Effects of Tai Chi on fatigue and resting-state brain activity in chronic fatigue syndrome: a randomized controlled fMRI study.

BACKGROUND: Chronic fatigue syndrome (CFS) is a complex disorder characterized by persistent fatigue, sleep disturbance, and cognitive impairment. It substantially impairs both physical and mental health. Tai Chi, a traditional Chinese mind-body exercise, may improve symptoms in patients with CFS. However, its neural mechanisms remain unclear. This study investigated the effects of Tai Chi on spontaneous brain activity in patients with CFS using resting-state functional magnetic resonance imaging (rs-fMRI) and amplitude of low-frequency fluctuations (ALFF). METHODS: Sixty patients with CFS we

Electricity generation: options for reduction in carbon emissions.

Historically, the bulk production of electricity has been achieved by burning fossil fuels, with unavoidable gaseous emissions, including large quantities of carbon dioxide: an average-sized modern coal-burning power station is responsible for more than 10 Mt of CO(2) each year. This paper details typical emissions from present-day power stations and discusses the options for their reduction. Acknowledging that the cuts achieved in the past decade in the UK CO(2) emissions have been achieved largely by fuel switching, the remaining possibilities offered by this method are discussed. Switching

Triple Antithrombotic Therapy versus Dual Antiplatelet Therapy Alone Following Anterior STEMI, a Systematic Review and Meta-Analysis With GRADE Assessment.

BackgroundLeft ventricular thrombus (LVT) remains an important complication following anterior ST-elevation myocardial infarction (STEMI), despite widespread adoption of primary percutaneous coronary intervention (pPCl). Current guidelines supporting prophylactic oral anticoagulation in high-risk patients are based on limited evidence, and the balance between thromboembolic prevention and bleeding risk remains uncertain. PubMed, Embase and etc were searched for studies comparing triple antithrombotic therapy (TAPT; anticoagulation plus DAPT) versus DAPT alone in patients with anterior STEMI un

Barriers to and enablers of childhood immunization uptake in Ethiopia's Amhara, Oromia, and Somali Regions: A multi-perspective qualitative study.

Ethiopia has the fourth-highest number of zero-dose children globally. Negative experiences and perceptions of immunization are recognized barriers to vaccination uptake but warrant context-specific investigation. We explored barriers and enablers to immunization uptake in selected regions of Ethiopia. We conducted a formative qualitative study involving 18 focus group discussions with men and women, in-depth interviews with 23 mothers of children with varying immunization statuses, and 42 key informant interviews with religious and community leaders and health workers in eight districts in Am

The impact of universal credit on mental, physical, and financial well-being: longitudinal evidence from the UK household longitudinal survey.

BACKGROUND: Universal Credit (UC) is a major UK welfare reform that consolidates six means-tested benefits into a single monthly payment, aiming to simplify benefits delivery and incentivize labor market participation. However, concerns have emerged regarding its potential adverse consequences on recipients' mental and physical well-being. Existing evidence is limited by methodological weaknesses, short follow-up time, and a narrow focus on psychological distress. METHODS: Applying the heterogeneous difference-in-differences approach developed by Callaway and Sant'Anna, we used waves 6-14 of

Effect of restrictive fluid balance in critically ill patients with acute kidney injury: a target trial emulation.

PURPOSE: To estimate the effect of restrictive fluid management on renal function and mortality in critically ill patients with acute kidney injury (AKI) at high risk of developing positive fluid balance. METHODS: We conducted a target trial emulation using data from 12 intensive care units in Queensland, Australia (2015-2021). Adults with AKI within 72 h of intensive care unit (ICU) admission and an AKI-Fluid Balance Risk Score ≥ 32 were included. The intervention comprised conservative crystalloid administration, restricted nutritional fluid intake, and diuretic use for 72 h. We estimated p

Pediatric head injury in the emergency department: balancing radiation risk, missed diagnoses, and decision-rule evolution.

BACKGROUND: Pediatric head injury is a frequent cause of emergency department neuroimaging worldwide and a major contributor to health care utilization. Although most children with minor head trauma do not sustain clinically important traumatic brain injury (ciTBI), the risk of acute complications necessitates accurate risk stratification. A central challenge is balancing the need to reliably exclude ciTBI against the potential harms of ionizing radiation. Despite high-sensitivity clinical decision rules, computed tomography (CT) remains the dominant imaging modality, with utilization rates ap

Lymphoid-like Suppressive Microglia in Alzheimer's Disease: A New Neuroimmune Regulatory Axis?

Microglia are central regulators of Alzheimer's disease pathogenesis, but their roles cannot be reduced to a simple protective-versus-harmful dichotomy. Genetic, single-cell, and spatial studies have shown that Alzheimer 's-associated microglia occupy diverse disease-linked states shaped by amyloid plaques, tau pathology, lipid stress, complement activation, astrocyte signaling, aging, and immune genetic risk. Among the regulatory nodes controlling these states, SPI1, which encodes the myeloid transcription factor PU.1, has emerged as a key determinant of microglial identity and disease respon

Point-of-care ultrasound during trauma and critical care transport: a practical, safety-gated review.

BACKGROUND: Air medical and critical care transport teams often treat severely injured or critically ill patients when imaging, laboratory testing, and specialist support are limited. Point-of-care ultrasound (POCUS) may support early recognition of reversible conditions, but its value depends on strict indication selection, operator competence, and interpretation rules that prevent false reassurance. METHODS: This structured narrative review summarizes evidence and practical considerations for transport POCUS in trauma and critical care retrieval. The review is organized around a safety-gate

Motion preserving C1 ring lateral mass screw fixation with transverse rod for isolated unstable Jefferson fracture of atlas vertebra: a multicentric study.

PURPOSE: Some cases of isolated unstable Jefferson's fracture of the Atlas verbebra require surgical fixation. Historical constructs with occipitocervical fixation or C1C2 posterior Harms-Goel fixation remain associated with functional impairment owing to limited postoperative cervical range of motion. Since 2004, motion-preserving C1 ring lateral mass screws with transverse rod (C1 LMF) has emerged as a possible technical alternative. METHODS: Retrospective multicentric study of patients operated on for isolated unstable Jefferson's fracture of the Atlas vertebra using C1 LMF in two tertiary

Short-Term Success, Long-Term Failure: Strain Turnover and Virulence Re-Emergence May Drive Relapse in Pouchitis.

BACKGROUND & AIMS: Pouchitis, de-novo small intestinal inflammation is the most common complication developing in patients with ulcerative colitis after total large bowel resection and ileal pouch-anal anastomosis (IPAA) reconstruction. While the first line treatment is antibiotics, the microbial properties underlying flare, remission, and relapse remain vague. We aimed to investigate how antibiotic treatment drives microbial shifts that underlie remission and contribute to relapse. METHODS: Patients after IPAA were prospectively recruited during clinical flare (active pouchitis defined by th

Motivational interviewing for substance use among adults in low- and middle-income countries: A systematic review.

BACKGROUND: Harmful substance use (HSU) poses a significant global health challenge, impacting social, economic, and healthcare systems. Low- and middle-income countries (LMICs) are particularly burdened as a result of limited infrastructure for managing substance-related issues. While evidence-based brief interventions like motivational interviewing (MI) are widely used in high-income countries, research on their effectiveness in LMICs is limited. AIM: This systematic review aims to evaluate studies employing MI as part of interventions for adults with HSU in LMIC settings. SETTING: This re

Effect of denture and soft tissue disinfection on edentulous biofilms.

Oral biofilms play a central role in oral and systemic diseases. Periodontal pathogens such as Porphyromonas gingivalis and Aggregatibacter actinomycetemcomitans have been detected on dentures and oral mucosa, suggesting denture wearers remain at risk. This study aimed to evaluate whether combined denture and soft tissue disinfection reduces microbial biofilm more effectively than denture cleaning alone. Sixty-one complete-denture wearers were randomly assigned to a test group (n = 31; denture disinfection plus soft tissue chlorhexidine treatment) or a control group (n = 30; denture disinfecti

Equity-focused analysis of tailoring strategies for overcoming risk factors in ethnic minorities (EQUATOR): a protocol for a scoping review with a realist analysis.

INTRODUCTION: Chronic diseases driven by modifiable risk factors (MRFs), such as poor diet, physical inactivity, tobacco use and harmful alcohol consumption, disproportionately affect ethnic minority groups worldwide and within multi‑ethnic societies such as Singapore. Although culturally tailored interventions and implementation strategies exist, there is limited synthesis of what has been used, for whom and under which contextual conditions these approaches produce equitable reductions in MRFs. EQUATOR aims to identify and synthesise policies, programmes and implementation strategies that ha

Evaluation of hepatic cancer stem cells (CD73+, CD44+, and CD90+) induced by diethylnitrosamine in male rats and treatment with biologically synthesized silver nanoparticles.

BACKGROUND: Cancer stem cells (CSCs) play a critical role in the initiation and heterogeneity of a variety of cancers due to their pluripotent nature and capacity for asymmetric cell division. Therefore, uncovering the carcinogens that increase the CSC population in target tissues is crucial for understanding the genesis of cancer. The therapeutic potential of Operculina turpethum (OT) derived silver nanoparticles (AgNPs) was assessed in diethylnitrosamine (DEN)-induced CSC populations; CD73+, CD44+, and CD90 + of hepatic tissues in male rats. Sodium benzoate exposure disrupts HPG-axis in male rats: insights into oxidative stress, hormonal dysregulation, histopathology and kisspeptin/RFRP-3 expression.

The reproductive effects related to sodium benzoate (SB) are increasingly recognized as concerns for public health. To elucidate the underlying mechanistic pathway in SB-induced reproductive impairment, we evaluated its dose-dependent effects by assessing the key elements of HPG- axis, encompassing wide range of doses from dietary to presumably harmful levels. Thirty-five adult male rats (Sprague-Dawley) split into seven groups (n = 5/group); control, SB10, SB50, SB100, SB500, SB1000mg/kg and the co-treated group (SB + EB). SB and distilled water were gavage for 90 consecutive days. The co-tre

Exercise attenuates inflammaging and testosterone decline in aged and morphine-treated rats: a role for NF-κB signaling.

Aging reduces testicular function by lowering sperm quality and testosterone, worsened by diseases. Morphine addiction harms male reproduction by disrupting hormonal balance and increasing testicular oxidative stress and inflammation. Regular physical exercise can help counteract these effects by boosting antioxidants and supporting sperm production. This study explores how exercise mitigates inflammaging and testosterone decline by modulating the NF-κB signaling pathway in aged male rats with morphine addiction. A total of 56 male Wistar rats were divided into eight groups, with four groups f

Identification of Anatoxin-a and Related Metabolites in Exposed Mice Samples with a High-Resolution Mass Spectrometry Discovery Workflow.

Harmful cyanobacterial blooms (HCBs) are a public health concern and require ongoing surveillance to monitor the negative water quality effects and cyanotoxins associated with these blooms. (+)-Anatoxin-a (ATX) is a potent neurotoxin produced by select cyanobacteria during HCB formation. Many HCB toxins are commonly associated with discolored water; however, ATX can be present in clear water, which results in a high risk of exposure by accidental ingestion for humans and animals. In this work, we used a qualitative, semitargeted liquid chromatography high resolution mass spectrometry (LC-HRMS)
