The surgical results in patients with severe congenital heart disease, who underwent surgical treatment between 1978 and 1981 at the National Cardiovascular Center, Japan, were analyzed. Six children with congenital heart disease are described in whom poor growth was found to be due to small-bowel villous atrophy; none was in heart failure and only one was severely cyanosed. Growth improved in all 6 on a gluten-free diet. Gluten enteropathy may be more common than is realised in children with congenital heart disease, and jejunal biopsy should be undertaken early in any patient with poor growth and no heart failure to exclude the coexistence of the condition.

Ten patients suffering from panic attacks were studied in an effort to test the feasibility of measuring panic attacks via 24-h ambulatory heart rate monitoring. Subjects were also asked to record their activity level, anxiety level, and level of panic at 15-min intervals. Seven of the 10 patients reported panic attacks during the 24-h recording period; one patient reported two panic attacks. Captopril (Lopirin, Squibb, von Heyden) is an inhibitor of the angiotensin-I converting enzyme. In congestive heart failure angiotensin mediated vasoconstriction and aldosterone secretion is reversed by captopril. Both effects of captopril induce reduction of cardiac preload and afterload. The clinical state of patients with congestive heart failure therefore improves since cardiac output increased and pulmonary pressures decrease. A vast experience of over 1500 patients with prostheses of the heart valves is being summarized. A method of physical and psychological rehabilitation of these patients is described and data are given proving the efficacy of such measures: increased physical endurance in work by 92%, decrease of psychic disorders by 41.5%. The efficacy of the sanatorium stage of post-operative treatment is shown. Not infrequently the degree of invalidity of patients with cardiac valve prostheses is exaggerated, it is stressed that the use of exercise tests by the rehabilitation commission is essential.

The epidemiologic association between hypertension and left ventricular failure is well established, and the hemodynamics of this relationship are well defined. From a clinician's standpoint, perhaps the most important fact to keep in mind is that effective control of the predisposing hypertensive disease can reduce the consequent congestive heart failure by as much as 90%.

From October 1, 1979 to June 30, 1980, 890 2D echocardiographic examinations were performed. Of these, 221 showed valvular lesions with predominant rheumatic aetiology. The valvular lesions encountered were pure mitral stenosis, pure mitral insufficiency, mixed mitral stenosis and insufficiency, aortic incompetence, aortic stenosis, mitral valve prolapse and left atrial thrombus. It is concluded that 2D echo imaging should in due time provide the "gold standard" for the evaluation of valvular lesions.

Experience in surgical correction of aortic valvular disease with extreme calcification of valves in 145 patients is generalized. The method for valve prosthetics preventing the development of specific complications is described. On the basis of analysis of the immediate and late postoperative results, it is recommended to use cold cardioplagia for protection of the myocardium. Hospital postoperative mortality was 13.9% in the last 4 years and 10.5% in the last 18 months. The left ventricle in left heart failure can be elliptical, spherical or funnel shaped. There is no firm correlation between the different shapes and the hemodynamics. Left ventricular failure results in dilatation, deformation and loss of funnel function of the left atrium. Two cases of thyrotoxicosis including a first-degree heart block are reported; a study of action potentials of the bundle of His was achieved in the second case. The association atrioventricular block-hyperthyroidism is unusual but does not seem to be a pure coincidence. The direct effect of thyroid hormon on the myocardium is supposed to be responsible for the conduction disturbance; its disappearance with the cure of the thyrotoxicosis is a favourable argument for a relation of causality. The therapeutic consequence of this association is to exclude completely the use of betablockers.
