Global and US standards for intrapartum surveillance of the pregnant and the fetal patient.

OBJECTIVE: Cardiotocography is a dual-signal physiologic modality monitoring the pregnant and fetal patient. The objective of the present study was to compare global guidelines with the recently updated US Clinical Practice Guideline. METHODS: A comparative analysis was conducted across global guidelines and the US guideline, evaluating three domains: uterine physiology, fetal heart rate interpretation, and management models. Definitions, interpretive structures, and triggers for intervention were compared without attempting to reconcile divergent frameworks. RESULTS: Global guidelines (card…

Impaired thyroid hormone receptor signaling in anterior hypothalamic parvalbumin neurons causes bradycardia in male mice.

Thyroid hormone receptor α1 (TRα1) regulates body temperature and heart rate in humans and mice. In addition to its direct actions in target tissues, it also affects peripheral functions indirectly through the brain. While these central actions on peripheral tissues have been demonstrated for liver and brown fat, the consequences for cardiac functions are still enigmatic. Recently, a population of parvalbumin neurons has been discovered in the anterior hypothalamic area that depends on TRα1 for correct development and controls heart rate in a temperature-dependent manner. Here we test the hypo…

Bradycardia after pulse methylprednisolone therapy in a child-Uncommon side effect of a frequently used drug: A case report.

Corticosteroids are potent anti-inflammatory agents used as a mainstay of therapy in most of the rheumatologic disorders. Common side effects of pulse steroid therapy include hypertension, hyperglycemia, seizure, hypokalemia, and infection. We report a case of an 11-year-old girl with polyarticular Juvenile idiopathic arthritis who developed bradycardia following pulse methylprednisolone therapy. On day 2 of methylprednisolone infusion, she developed bradycardia with a heart rate between 50 and 60/min. ECG was suggestive of sinus bradycardia. There was no evidence of dyselectrolytemia (Na-141 …

The impact of central and obstructive respiratory events on cerebral oxygenation in children with sleep disordered breathing.

STUDY OBJECTIVES: Both central and obstructive respiratory events are features of sleep disordered breathing. The repetitive hypoxia associated with obstructive events is believed to underpin the adverse neurocognitive and cardiovascular sequelae of this disorder, however whether central events contribute to this has not been investigated. To compare changes in cerebral and peripheral oxygenation, blood pressure and heart rate, associated with central and obstructive events in children aged 3-6 and 7-12 years. METHODS: Sixty children referred for suspected sleep disordered breathing underwent…

Chronic fluoxetine treatment enhances sympathetic activities associated with abnormality of baroreflex function in conscious normal rats.

Data regarding the effects of selective serotonin reuptake inhibitors (SSRIs), which are a common type of antidepressants, on cardiovascular autonomic function are inconsistent. The present study was conducted to determine the effects of chronic fluoxetine, an SSRI, on blood pressure, cardiac autonomic nervous activities and baroreflex control of heart rate. Male Sprague-Dawley rats were treated with fluoxetine (10mg/kg day, p.o.) or saline for 14 weeks. Baroreflex function was determined by the sigmoid logistic method based on the heart rate responses to changes in blood pressure elicited by …

Cardiovascular responses elicited during binge administration of cocaine.

Cocaine use is often characterized by a repeated pattern of frequent administrations (binge) followed by periods of abstinence. The repeated binge administration of methamphetamine and 3,4-methylenedioxymethamphetamine (MDMA) alters cardiovascular function and the arterial pressure and heart rate responses elicited by these drugs. Whether repeated binges of cocaine similarly affect cardiovascular function and cardiovascular responsiveness is unknown. Radiotelemetry was used to record the cardiovascular responses elicited during three successive cocaine binges (5 mg/kg, t.i.d., for 4 days) in c…

Influences of the sympathetic and parasympathetic nerve transection on cardiovascular reflexes induced by volleys in the IXth nerve fibers of rat.

The effect of vagotomy and sympathectomy on cardio-acceleratory and arterial hypertensive reflexes evoked by electrical stimulation of the rat glossopharyngeal (IXth) nerve was studied in relation to changes in baseline heart rate and arterial blood pressure. Uni- and bi-lateral transection of the cervical vagal trunk brought about augmentation of baseline heart rate, accompanied by a depression in reflex tachycardia; the amount of depression being inversely related to that of an increase in baseline heart rate. The latter increased more after a right vagotomy than after a left vagotomy. No ap…

Is absolute hypovolemia a risk factor for vasovagal response to head-up tilt?

To test the hypothesis that hypovolemia is associated with an increased incidence of vasovagal syncope during head-up tilt (HUT) 45 patients … (16.7% and 26.7%, respectively, P = NS). In those who developed vasovagal response, the changes of heart rate and blood pressure during HUT were not significantly different between hypovolemics and normovolemics, neither at the endpoint (vasovagal response) nor immediately before the development of the vasovagal response. In patients with nonvasovagal event …

[Usefulness of carotid sinus massage for Doppler quantification of mitral stenosis in sinus rhythm].

The quantification of mitral stenosis by pressure half-time method may be difficult in sinus rhythm with short deceleration slopes. To evaluate the usefulness of carotid sinus massage (CSM) for measuring mitral valve area by pressure half-time, 80 consecutive patients with mitral stenosis in sinus rhythm underwent a echo-Doppler study at basal heart rate and during right CSM. In both situations we measured: heart rate, mitral area by pressure half-time and E-F interval (time from E peak velocity to onset of atrial contraction). In 38 consecutive patients we also determined the mitral area by p…

Ambulation in the adolescent with spina bifida. II. Oxygen cost of mobility.

This study was designed to determine the energy cost (measured as oxygen use) of walking and wheelchair propulsion in children aged 10 to 15 with myelomeningocele of thoracic to sacral levels, and to determine whether energy cost of mobility could be estimated from clinical measures. Oxygen consumption (measured with open circuit spirometry) and heart rate were measured during treadmill walking by 21 children, wheelchair use by eight children, and, for five children, in both modes. Speeds ranged from 27 to 134 m/min, with slopes up to 15%. Energy consumption for walking was linearly related to…

Time trends in mortality from heart failure and atherosclerotic cardiovascular disease in people with and without diabetes: a multi-national population-based study.

BACKGROUND: Contemporary trends in cardiovascular disease (CVD) cause-specific mortality by diabetes status are inadequately described. We examined trends by diabetes status in coronary heart disease (CHD), cerebrovascular disease, and heart failure mortality, and mortality rate ratios (people with diabetes versus those without diabetes) across nine high-income jurisdictions. METHODS: We assembled CVD cause-specific mortality data from nine administrative datasets (Europe [n=5], Australia [n=1], Canada [n=2], and South Korea [n=1]), spanning 2000-23. Using Poisson regression, we estimated mor

Linear increase in optimal pedal rate with increased power output in cycle ergometry.

This experiment was designed to estimate the optimum pedal rates at various power outputs on the cycle ergometer. Five trained bicycle racers performed five progressive maximal tests on the ergometer. Each rode at pedal rates of 40, 60, 80, 100, and 120 rev X min-1. Oxygen uptake and heart rate were determined from each test and plotted against pedal rate for power outputs of 100, 150, 200, 250, and 300 W. Both VO2 and heart rate differed significantly among pedal rates at equivalent power outputs, the variation following a parabolic curve. The low point in the curve was taken as the optimal p…

Reversal of Fetal Heart Block in Antibody-Positive Mother After Hydroxychloroquine and Dexamethasone.

Maternal autoantibody-related complete heart block in the fetus is considered irreversible. During prenatal care for a 25-year-old nulliparous Hispanic woman with newly diagnosed nephrotic-range proteinuria and positive anti-nuclear antigen antibody, complete fetal heart block with a ventricular rate of 60 beats per minute was detected on a fetal echocardiogram at 28-week gestation. A small pericardial effusion and ascites were noted consistent with fetal hydrops. Dexamethasone and hydroxychloroquine were initiated. Fetal rhythm improved to Mobitz type 1 second-degree heart block, with a ventr

Decoding cardiac reinnervation from cardiac autonomic markers: A mathematical model approach.

BACKGROUND: Although cardiac autonomic markers (CAMs) are commonly used to assess cardiac reinnervation in heart-transplant patients, their relationship to the degree of sympathetic and vagal cardiac reinnervation is not well understood yet. To study this relationship, we applied a mathematical model of the cardiovascular system and its autonomic control. METHODS: By simulating varying levels of sympathetic and vagal efferent sinoatrial reinnervation, we analyzed the induced changes in CAMs including resting heart rate (HR), bradycardic and tachycardic HR response to Valsalva maneuver, root m…
