Case Report: An undescended intravagal parathyroid adenoma: a rare cause of refractory primary hyperparathyroidism and severe osteoporosis.

Ectopic intravagal parathyroid adenomas represent an exceedingly rare clinical entity. Embryologically, parathyroid glands develop from the third and fourth pharyngeal pouches, migrating to their definitive locations on the inferior and superior aspects of the thyroid. While ectopic locations are well described, intravagal variants are rare, with few documented cases in the literature. We report an additional unique case of a patient with persistent hypercalcemia and severe osteoporosis following a failed bilateral neck exploration for primary hyperparathyroidism. Following multimodal structur…

Independent Predictors of Hungry Bone Syndrome After Parathyroidectomy for Primary Hyperparathyroidism: Insights from a Large Cohort Study.

Background/Objectives: Hungry bone syndrome (HBS) is a clinically significant metabolic complication following parathyroidectomy for primary hyperparathyroidism (PHPT), characterized by profound and prolonged hypocalcemia resulting from rapid skeletal remineralization. Although multiple risk factors have been proposed, published data remain inconsistent regarding the most reliable predictors. This study aimed to evaluate potential risk factors associated with the development of HBS and to clarify controversial findings reported in the literature. Methods: Patients who underwent surgery for PHP…

Analysis of circulating bone-glucose profile and exerkine irisin across parathormone quartiles in patients with primary hyperparathyroidism.

BACKGROUND: Parathyroid tumors, mostly single or multiple adenomas, cause a hormonal imbalance via parathormone (PTH) excess, that further increases serum calcium (primary hyperparathyroidism) with both PTH- and calcium-related multi-organ consequences. Testing the bone turnover markers (BTM) profile might capture the essence of associated bone turnover anomalies with practical applications in the clinical setting. OBJECTIVE: Our objective was to compare the BTM across mineral metabolism panel, glucose profile, as well as circulating irisin levels in patients with primary hyperparathyroidism …

Case Series: Postoperative recurrent secondary hyperparathyroidism caused by ectopic submandibular parathyroid: 3 cases report and literature review.

Postoperative recurrence for secondary hyperparathyroidism (SHPT) remains a clinical challenge. Ectopic parathyroid glands are a major cause of recurrence, and those located in the submandibular region are rare. This report retrospectively analyzes three patients on maintenance dialysis who developed recurrent elevation of parathyroid hormone (PTH) levels 3 to 37 months after parathyroidectomy (PTX) for SHPT. Neck ultrasound revealed no definite lesions; however, 99mTc-methoxyisobutylisonitrile (99mTc-MIBI) single-photon emission computed tomography/computed tomography (SPECT/CT) fusion imagin…

Brown Tumors of Hyperparathyroidism: A 13-Year Retrospective Case Series from a Tertiary Center in South India.

INTRODUCTION: Brown tumors are rare, non-neoplastic osteolytic lesions resulting from hyperparathyroidism (HPT), often mimicking bone metastases. Although their incidence has declined in developed nations due to early biochemical screening, they remain clinically relevant in regions with prevalent Vitamin D deficiency. We present a 13-year retrospective analysis of brown tumor cases managed at a tertiary care center in South India. MATERIAL AND METHODS: Twelve patients diagnosed with brown tumors between 2012 and 2025 were retrospectively reviewed. Clinical presentation, biochemical findings,…

Causal validation between 179 lipids and hyperparathyroidism: A bidirectional Mendelian randomization combined meta-analysis with mediation factors.

Hyperparathyroidism, an endocrine disorder linked to hypercalcemia, increases with age, particularly in those over 60. Abnormal lipid metabolism may be closely related to its occurrence and progression. The study used Mendelian randomization (MR) analysis on 179 lipid traits against hyperparathyroidism in the Finngen and UK Biobank (UKB) databases. Meta-analysis of inverse variance weighted results followed, with significance P-values corrected for multiple comparisons. Causal validation was performed between positive lipids and renal failure, and MR analysis examined the link between renal fa…

Prolonged secondary hyperparathyroidism in adenine-induced CKD leads to skeletal changes consistent with skeletal hyporesponsiveness to PTH.

High circulating parathyroid hormone (PTH) leading to secondary hyperparathyroidism is proposed to be a key driver of the skeletal phenotype of chronic kidney disease-mineral bone disorder (CKD-MBD) leading to high bone turnover and cortical bone deterioration. The association between high PTH and the skeletal phenotype is typically clearly demonstrated in preclinical models of CKD; however, clinical studies show the relationship between PTH and skeletal outcomes is not as clear. The clinical data have led to a proposed hyporesponsiveness to PTH in the CKD setting with unclear causes. In the c…

Acute onset neurological deficits and hypocalcaemia associated with nutritional secondary hyperparathyroidism in three juvenile servals (Leptailurus serval).

Nutritional secondary hyperparathyroidism is a metabolic disorder caused by an imbalance in calcium homeostasis and is typically associated with the feeding of a calcium-deficient diet. Nutritional secondary hyperparathyroidism has been reported in domestic species, including cats and dogs, as well as captive carnivores and wildlife including lions, tigers, servals and skunks. Captive carnivores are typically fed a raw-meat diet, which can be severely deficient in calcium if not appropriately balanced or supplemented. Three juvenile servals (Leptailurus serval) presented with seizures, ataxia …

Hypercalcemia Secondary to Hyperparathyroidism Mimicking Eclampsia During Postpartum Period in a Young Hypertensive Woman: A Case Report.

We report a rare case of hyperparathyroidism due to parathyroid adenoma causing hypercalcemia, presenting as a generalized prolonged seizure on the seventh postpartum day mimicking eclampsia. Eclampsia developing on the seventh postpartum day in a woman without features of impending eclampsia is rare. Therefore, other differential diagnoses should be considered including primary hyperparathyroidism. Diagnosis is essential as complete cure is possible with surgery.

Quality of Life After Parathyroidectomy in Chronic Kidney Disease-Related Hyperparathyroidism: A Systematic Review and Meta-Analysis.

BACKGROUND: Secondary and tertiary hyperparathyroidism (SHPT and THPT) are frequent complications of chronic kidney disease and kidney transplantation, often impairing quality of life (QoL) through bone pain, fatigue, and pruritus. Parathyroidectomy is the definitive treatment for refractory cases, yet its impact on patient-reported QoL outcomes remains uncertain. METHODS: We conducted a systematic review and meta-analysis in accordance with PRISMA guidelines (PROSPERO CRD42025108038). Nine studies (n = 675) with validated QoL assessments and ≥ 6 months of follow-up were included. QoL was mea…

Parathyroid fine needle aspiration with PTH washout: Can it lead to parathyroid cell seeding in primary hyperparathyroidism?

Although parathyroid fine-needle aspiration (P-FNA) with parathyroid hormone (PTH) washout is effective in detecting preoperative parathyroi … e follow-up. Effective adenoma localization is crucial for successful minimally invasive surgery of hyperparathyroidism. Our study indicates that combining preoperative P-FNAB with PTH washout and imaging enhances adenoma detection, especially when intraoperative PTH measurements are not available, thus improving surgical outcomes. Notably, we found no evidence of ce …

Intraoperative parathyroid hormone assay benefits surgery for primary hyperparathyroidism when preoperative localisation is negative or not performed.

INTRODUCTION: Parathyroid localisation is now routine before first-time surgery for patients with primary hyperparathyroidism (PHPT). The aim of this study was to investigate the contribution of intraoperative parathyroid hormone (PTH) (ioPTH) in patients in whom localisation was either not undertaken or negative for a tumour. METHODS: This was a retrospective study of patients undergoing first-time parathyroidectomy for PHPT in a regional endocrine centre. Data were collected prospectively (Microsoft Excel) and the all-Wales electronic patient record portal was used to retrieve missing data.…

Parathyroid near-infrared autofluorescence use for parathyroidectomy in mild primary hyperparathyroidism: Results from a randomized monocentric trial.

BACKGROUND: Multiglandular parathyroid disease, which is particularly frequent in patients with mild primary hyperparathyroidism, is a surgical challenge requiring bilateral cervicotomy with 4-gland exploration. Near-infrared autofluorescence of the parathyroid is increasingly used to prevent hypocalcemia after total thyroidectomy. However, its utility in decreasing operating time and aiding parathyroid identification during bilateral 4-gland exploration remains debated. METHODS: In our prospective trial, we enrolled consecutive patients with sporadic mild primary hyperparathyroidism (serum c…

Clinical predictors of negative/equivocal SPECT imaging outcomes in primary hyperparathyroidism: Factors calling for 18F-choline-PET.

PURPOSE: For minimally invasive surgery of parathyroid adenomas, exact localization diagnostics are essential. Main imaging modalities used for diagnostics are sonography, SPECT with/without CT (traditional imaging) and 18F-choline-PET. The aim of our study was to identify predictors for inconclusive SPECT imaging and subsequently determine in which cases 18F-choline-PET is needed. METHODS: Retrospective analysis of 138 patients with histologically confirmed primary hyperparathyroidism (pHPT). After sonography, patients underwent SPECT or SPECT/CT imaging, with subsequent 18F-choline-PET in c…
