Digital decision aid for prenatal counseling in imminent extreme premature labor: development and pilot testing.

BACKGROUND: In case of extreme premature delivery at 24 weeks of gestation, both early intensive care and palliative comfort care for the neonate are considered treatment options. Prenatal counseling, preferably using shared decision making, is needed to agree on the treatment option in case labor progresses. This article described the development of a digital decision aid (DA) to support pregnant women, partners and clinicians in prenatal counseling for imminent extreme premature labor. METHODS: This DA is developed following the International Patient Decision Aid Standards. The Dutch treatm

Explainable machine learning methods and respiratory oscillometry for the diagnosis of respiratory abnormalities in sarcoidosis.

BACKGROUND: In this work, we developed many machine learning classifiers to assist in diagnosing respiratory changes associated with sarcoidosis, based on results from the Forced Oscillation Technique (FOT), a non-invasive method used to assess pulmonary mechanics. In addition to accurate results, there is a particular interest in their interpretability and explainability, so we used Genetic Programming since the classification is made with intelligible expressions and we also evaluate the feature importance in different experiments to find the more discriminative features. Machine learning approaches to predict the 1-year-after-initial-AMI survival of elderly patients.

BACKGROUND: While multiple randomized controlled trials (RCTs) are available, their results may not be generalizable to older, unhealthier or less-adherent patients. Observational data can be used to predict outcomes and evaluate treatments; however, exactly which strategy should be used to analyze the outcomes of treatment using observational data is currently unclear. This study aimed to determine the most accurate machine learning technique to predict 1-year-after-initial-acute-myocardial-infarction (AMI) survival of elderly patients and to identify the association of angiotensin-converting

On usage of artificial intelligence for predicting mortality during and post-pregnancy: a systematic review of literature.

BACKGROUND: Care during pregnancy, childbirth and puerperium are fundamental to avoid pathologies for the mother and her baby. However, health issues can occur during this period, causing misfortunes, such as the death of the fetus or neonate. Predictive models of fetal and infant deaths are important technological tools that can help to reduce mortality indexes. The main goal of this work is to present a systematic review of literature focused on computational models to predict mortality, covering stillbirth, perinatal, neonatal, and infant deaths, highlighting their methodology and the descr

The CirCor DigiScope Dataset: From Murmur Detection to Murmur Classification.

Cardiac auscultation is one of the most cost-effective techniques used to detect and identify many heart conditions. Computer-assisted decision systems based on auscultation can support physicians in their decisions. Unfortunately, the application of such systems in clinical trials is still minimal since most of them only aim to detect the presence of extra or abnormal waves in the phonocardiogram signal, i.e., only a binary ground truth variable (normal vs abnormal) is provided. This is mainly due to the lack of large publicly available datasets, where a more detailed description of such abno

Building a Risk Prediction Model for Postoperative Pulmonary Vein Obstruction via Quantitative Analysis of CTA Images.

Total anomalous pulmonary venous connection (TAPVC) is a rare but mortal congenital heart disease in children and can be repaired by surgical operations. However, some patients may suffer from pulmonary venous obstruction (PVO) after surgery with insufficient blood supply, necessitating special follow-up strategy and treatment. Therefore, it is a clinically important yet challenging problem to predict such patients before surgery. In this paper, we address this issue and propose a computational framework to determine the risk factors for postoperative PVO (PPVO) from computed tomography angiog

Automatic Assessment of Pectus Excavatum Severity From CT Images Using Deep Learning.

Pectus excavatum (PE) is the most common abnormality of the thoracic cage, whose severity is evaluated by extracting three indices (Haller, correction and asymmetry) from computed tomography (CT) images. To date, this analysis is performed manually, which is tedious and prone to variability. In this paper, a fully automatic framework for PE severity quantification from CT images is proposed, comprising three steps: (1) identification of the sternum's greatest depression point; (2) detection of 8 anatomical keypoints relevant for severity assessment; and (3) measurements' geometric regularizati

On the analysis of data augmentation methods for spectral imaged based heart sound classification using convolutional neural networks.

BACKGROUND: The application of machine learning to cardiac auscultation has the potential to improve the accuracy and efficiency of both routine and point-of-care screenings. The use of convolutional neural networks (CNN) on heart sound spectrograms in particular has defined state-of-the-art performance. However, the relative paucity of patient data remains a significant barrier to creating models that can adapt to a wide range of potential variability. To that end, we examined a CNN model's performance on automated heart sound classification, before and after various forms of data augmentatio

Multi-label annotation of text reports from computed tomography of the chest, abdomen, and pelvis using deep learning.

BACKGROUND: There is progress to be made in building artificially intelligent systems to detect abnormalities that are not only accurate but can handle the true breadth of findings that radiologists encounter in body (chest, abdomen, and pelvis) computed tomography (CT). Currently, the major bottleneck for developing multi-disease classifiers is a lack of manually annotated data. The purpose of this work was to develop high throughput multi-label annotators for body CT reports that can be applied across a variety of abnormalities, organs, and disease states thereby mitigating the need for huma

Comparison of different feature extraction methods for applicable automated ICD coding.

BACKGROUND: Automated ICD coding on medical texts via machine learning has been a hot topic. Related studies from medical field heavily relies on conventional bag-of-words (BoW) as the feature extraction method, and do not commonly use more complicated methods, such as word2vec (W2V) and large pretrained models like BERT. This study aimed at uncovering the most effective feature extraction methods for coding models by comparing BoW, W2V and BERT variants. METHODS: We experimented with a Chinese dataset from Fuwai Hospital, which contains 6947 records and 1532 unique ICD codes, and a public Sp

Joint Localization and Classification of Breast Cancer in B-Mode Ultrasound Imaging via Collaborative Learning With Elastography.

Convolutional neural networks (CNNs) have been successfully applied in the computer-aided ultrasound diagnosis for breast cancer. Up to now, several CNN-based methods have been proposed. However, most of them consider tumor localization and classification as two separate steps, rather than performing them simultaneously. Besides, they suffer from the limited diagnosis information in the B-mode ultrasound (BUS) images. In this study, we develop a novel network ResNet-GAP that incorporates both localization and classification into a unified procedure. To enhance the performance of ResNet-GAP, we

A Photogrammetric Method for the Measurement of Three-Dimensional Cervical Range of Motion.

Cervical spondylosis has gradually become a high-incidence disease in today's society. The cervical range of motion (CROM) is widely used as the evaluation criterion of cervical status, whereas the existing methods of CROM measuring are not humanized enough. Consequently, the purpose of this study was to develop a novel photogrammetric method to assess three-dimensional CROM. Three smartphone cameras were controlled to simultaneously capture three-direction photographs of the subject wearing the special designed device with three mark lines. The obtained photographs were uploaded to a PC and t

Attention Gate Based Dual-Pathway Network for Vertebra Segmentation of X-Ray Spine Images.

Automatic spine and vertebra segmentation from X-ray spine images is a critical and challenging problem in many computer-aid spinal image analysis and disease diagnosis applications. In this paper, a two-stage automatic segmentation framework for spine X-ray images is proposed, which can firstly locate the spine regions (including backbone, sacrum and ilium) in the coarse stage and then identify eighteen vertebrae (i.e., cervical vertebra 7, thoracic vertebra 1-12 and lumbar vertebra 1-5) with isolate and clear boundary in the fine stage. A novel Attention Gate based dual-pathway Network (AGNe

Bayesian Comorbidity Network and Cost Analysis for Asthma.

The evolving disease spectrum poses significant challenges to the asthma management, thus worsening health quality and increased financial burden on patients. However, potential dependency pattern in comorbidity spectrum remains unclear. We built comorbidity networks based on Bayesian networks utilizing 19604 asthma-patient hospitalization data to investigate dependency patterns among asthma comorbidities. We analyze static properties and trajectory behaviors of gender- and age-stratified asthmatic comorbidity networks. Results suggest that chronic obstructive pulmonary disease, respiratory fa

Evaluation of an ARF diagnosis calculator: a survey and content analysis.

BACKGROUND: Acute Rheumatic Fever (ARF) is a critically important condition for which there is no diagnostic test. Diagnosis requires the use of a set of criteria comprising clinical, laboratory, electrocardiographic and echocardiographic findings. The complexity of the algorithm and the fact that clinicians lack familiarity with ARF, make ARF diagnosis ideally suited to an electronic decision support tool. The ARF Diagnosis Calculator was developed to assist clinicians in diagnosing ARF and correctly assign categories of 'possible, 'probable' or 'definite' ARF. This research aimed to evaluate

Noise-Immune Extreme Ensemble Learning for Early Diagnosis of Neuropsychiatric Systemic Lupus Erythematosus.

Early diagnosis is currently the most effective way of saving the life of patients with neuropsychiatric systemic lupus erythematosus (NPSLE). However, it is rather difficult to detect this terrible disease at the early stage, due to the subtle and elusive symptomatic signals. Recent studies show that the 1H-MRS (proton magnetic resonance spectroscopy) imaging technique can capture more information reflecting the early appearance of this disease than conventional magnetic resonance imaging techniques. 1H-MRS data, however, also presents more noises that can bring serious diagnosis bias. We hen

Stochastic Petri net model describing the relationship between reported maternal and congenital syphilis cases in Brazil.

INTRODUCTION: Syphilis is a sexually transmitted disease (STD) caused by Treponema pallidum subspecies pallidum. In 2016, it was declared an epidemic in Brazil due to its high morbidity and mortality rates, mainly in cases of maternal syphilis (MS) and congenital syphilis (CS) with unfavorable outcomes. This paper aimed to mathematically describe the relationship between MS and CS cases reported in Brazil over the interval from 2010 to 2020, considering the likelihood of diagnosis and effective and timely maternal treatment during prenatal care, thus supporting the decision-making and coordina
