The role of the cricothyroid joint anatomy in cricothyroid approximation surgery.

OBJECTIVES/HYPOTHESIS: Cricothyroid approximation (CTA) surgery aims at raising the voice pitch in male-to-female transsexuals. However, 30% of the patients are not satisfied with the result. The purpose of our study was to examine the cricothyroid joint (CTJ) biomechanics and to analyze if (and how) the CTJ anatomy influences the movement of the cricoid and, consequently, the elongation of the vocal fold and the voice pitch after CTA. METHODS: Twenty-four cadaver larynges were examined with high-resolution computerized tomography and MIMICS three-dimensional imaging software (Materialise Int

Joint modeling of longitudinal data in multiple behavioral change.

Multiple behavioral change is an exciting and evolving research area, albeit one that presents analytic challenges to investigators. This manuscript considers the problem of modeling jointly trajectories for two or more possibly non-normally distributed dependent variables, such as marijuana smoking and risky sexual activity, collected longitudinally. Of particular scientific interest is applying such modeling to elucidate the nature of the interaction, if any, between an intervention and personal characteristics, such as sensation seeking and impulsivity. The authors describe three analytic a

The intramandibular joint in Girella: a mechanism for increased force production?

Intramandibular joints (IMJ) are novel articulations between bony elements of the lower jaw that have evolved independently in multiple fish lineages and are typically associated with biting herbivory. This novel joint is hypothesized to function by augmenting oral jaw expansion during mouth opening, which would increase contact between the tooth-bearing area of the jaws and algal substratum during feeding, resulting in more effective food removal from the substrate. Currently, it is not understood if increased flexibility in a double-jointed mandible also results in increased force generation…

[Bone and joint diseases in children. Bisphosphonates in osteogenesis imperfecta].

Osteogenesis imperfecta (OI) is a genetic disorder characterized by fragile bone and reduced bone mineral density. Cyclic intravenous pamidronate is now the most widely used treatment for moderate to severe forms of OI. Oral bisphosphonates provide clinical benefit and convenience to the patients with mild form of OI. Recently, some studies demonstrated that intravenous zolendronate in children with OI is safe in short-term and similarly effective to parmidronate. Optimal dose, duration of treatment and long-term safety of newer bisphosphonates require further investigations.

MR imaging of atlantoaxial joint in early rheumatoid arthritis.

PURPOSE: This study was done to assess the involvement of the atlantoaxial joint in patients with early rheumatoid arthritis and evaluate the role of magnetic resonance (MR) imaging in depicting this early joint involvement. MATERIALS AND METHODS: Twenty patients (16 women and four men, mean age 55.0±12.9 years) with clinical and laboratory evidence of early rheumatoid arthritis (mean disease duration <12 months) were included in our study. MR imaging of the atlantoaxial joint was performed in all patients within 3 months from diagnosis. The MR features were correlated with clinical and bioch

Joint-preserving surgical treatment of spontaneous osteonecrosis of the knee.

BACKGROUND: To date, reports of surgical treatment of spontaneous osteonecrosis of the knee (SPONK) refractory to non-operative treatment have primarily focused on knee arthroplasty. This report presents an overview of the characteristics of SPONK and reports our experience with joint-preserving surgical treatment of this condition. METHODS: Fifteen patients who had joint-preserving surgery after failed non-operative modalities were studied. These patients were treated at a single center between January 1998 and September 2006 with a combination of arthroscopy and core decompression, or osteo

[Operative corrections of posttraumatic deformities of the elbow joint in children].

Posttraumatic deformities with need for surgical correction are rare and demanding in the pediatric population. The consequences of a bad outcome may last a life time. The best prevention of deformities around the elbow joint is proper initial treatment. The most common deformities are cubitus varus, cubitus valgus, chronic dislocation of the radial head and pseudarthrosis of the radial condyle. In contrast to the wide spread opinion to await the effect of the further growth, the deformity should be treated contemporarily to the underlying injury. This strategy will optimize the outcome. Human temporomandibular joint eminence shape and load minimization.

Analysis of previous data suggested the hypothesis that temporomandibular joint (TMJ) eminence shapes develop ideally to minimize joint loads. Hence, we tested this hypothesis in nine females and eight males in each of two groups, with and without TMJ disc displacement. Participants provided anatomical data used in a joint load minimization numerical model to predict, and jaw-tracking data used to measure, eminence shapes. Coordinate data (x,y) of shapes were fit to third-order polynomials for two sessions, sides, and methods (predicted, measured) for each participant. Inter-session data were

Osteomyelitis of the scapula with secondary septic arthritis of the shoulder joint.

We report a delayed diagnosis of osteomyelitis of the scapula involving the inferior angle and extending to the glenoid, with secondary septic arthritis of the glenohumeral joint in a 7-month-old female infant. The patient was treated with arthrotomy of the shoulder joint, anterior drainage of pus and intravenous antibiotics. The diagnosis was delayed as the patient was found to have bilateral lung abscess and other foci of infection, for which she was treated in the neonatal intensive care unit, with all the medical efforts directed towards saving her life. The diagnosis of septic shoulder ar

Blood-induced joint disease: the pathophysiology of hemophilic arthropathy.

Arthropathy is a frequent and serious complication of repeated joint bleeding in patients with hemophilia, resulting in pain, deformity, and disability. Although the pathogenesis of hemophilic arthropathy has not been fully elucidated, it appears to have similarities with the degenerative joint damage that occurs in osteoarthritis and the inflammatory processes associated with rheumatoid arthritis. This article reviews the potential actions of various blood constituents on joint components that culminate in the development of hemophilic arthropathy.

[Algorithm for the treatment of the haemophilic arthropathia of the upper ankle joint].

The upper ankle joint is one of the target-joints of the haemophilic patient. Therefore, the secondary arthritis of the upper ankle joint is one of the most frequent forms of haemophilic arthropathy. It is a secondary form of arthritis not only because of chronic synovitis and cartilage injury resulting from chronic recurrent intraarticular bleeds, but also due to the misalignment of the joint and abnormal joint stress. The consequences are manifest even in young patients and finally lead to upper ankle joint arthritis. In such clinical situations, the upper ankle joint-arthroplasty is a viabl…

Diagnosis and surgical management of synovial chondromatosis of the temporomandibular joint.

Synovial chondromatosis (SC) of the temporomandibular joint (TMJ) is an uncommon disease, which is characterized by chondrometaplasia of the synovial membrane. It is characterized by cartilaginous metaplasia of the mesenchymal remnants of the synovial tissue of the joints. Loose bodies can be found within the joint space. The cause of SC in the TMJ remains unclear. The nonspecific signs of SC often lead to a delay in diagnosis or a misdiagnosis. Removal of the loose bodies and of the synovial membrane involved is sufficient to control the disease in most cases. We present and discuss the diagn…

Benign joint hypermobility syndrome: a cause of childhood asthma?

Benign joint hypermobility syndrome (BJHS) is a hereditable disorder of connective tissue, which is characterized by the occurrence of multiple musculoskeletal problems in hypermobile individuals who do not have a systemic rheumatological disease. Rectal, uterine and mitral prolapses, varicose veins, myopia and recurrent urinary tract infections are more common in patients with BJHS, which indicates a diffuse anomaly in the structure of connective tissue rather than a limited involvement of the musculoskeletal system. Asthma, as a complex trait disease, develops after environmental exposure to

A novel in vivo skin extensibility test for joint hypermobility.

OBJECTIVE: The stress/strain curve derived from stretching skin is not linear, but follows a J-shaped curve. An initial generous yield is followed by a steep linear phase where considerable additional force is required to achieve modest increases in deformation. The former represents the taking up of slack resulting from the alignment of dermal collagen bundles in the line of force, while the gradient of the latter represents Young's modulus for skin. Skin hyperextensibility in Ehlers-Danlos syndrome (EDS) is limited to the initial phase of taking up slack. Skin hyperextensibility and joint hy

Chondrosarcoma of the temporomandibular joint. A case report and review of the literature.

Chondrosarcoma is a malignant tumor characterized by the formation of cartilage, but not bone, by tumor cells. Only 5% to 10% of chondrosarcomas occur in the head and neck, representing 0.1% of all head and neck neoplasms, with the larynx and the maxillo-nasal region being the most common sites. This report describes an unusual case of chondrosarcoma in a 54-year-old man who presented with pain and swelling in the left preauricular area. Computed tomography demonstrated a soft tissue mass in the left temporomandibular joint without causing erosion of the adjacent bony structures. The tumor was
