Antenatal care trial interventions: a systematic scoping review and taxonomy development of care models.

BACKGROUND: Antenatal care models vary widely around the world, reflecting local contexts, drivers and resources. Randomised controlled trials (RCTs) have tested the impact of multi-component antenatal care interventions on service delivery and outcomes in many countries since the 1980s. Some have applied entirely new schemes, while others have modified existing care delivery approaches. Systematic reviews (SRs) indicate that some specific antenatal interventions are more effective than others; however the causal mechanisms leading to better outcomes are poorly understood, limiting implementat

Clinical Nurse Specialists' Perceptions of Competency and Self-Efficacy in Evidence-Based Practice and Implementation Science.

BACKGROUND: Research and demonstration projects have shown that the Clinical Nurse Specialist (CNS) role is uniquely suited to lead implementation of evidence-based clinical practice changes for improved outcomes and reduced costs to healthcare systems. CNS practice competencies closely align with the design, development, and implementation of evidence-based practice change. National data on CNS's knowledge and use of implementation science to guide EBP implementation is lacking. AIMS: A CNS professional organization implementation science team and a university evidence-based practice center

Factors Associated with Professional Mental Health Counseling Among Korean Adults: An Age-Stratified Analysis Using the Andersen Behavioral Model.

Background/Objectives: Mental health service utilization remains considerably lower than the level of mental health need, and the factors related to utilization may differ across the life cycle. Applications of the Andersen Behavioral Model often underrepresent service availability and responsiveness. Based on the Andersen Behavioral Model, this study aimed to identify factors related to professional mental health counseling among Korean adults within three age strata. Methods: Using nationally representative, cross-sectional data from the 2021 National Mental Health Survey of Korea, this seco

History of Infertility and Plasma Markers of Inflammation, Cholesterol, and Adipokines Among Participants in the Nurses' Health Study II.

INTRODUCTION: There is increasing evidence that women who have experienced infertility are at greater risk for several chronic conditions. However, the mechanisms underlying these associations are not clear. Pathophysiology may be illuminated through observation of aberrant systemic biomarkers. However, there are limited data on infertility history and midlife biomarkers of inflammation, lipids, and adipokines. MATERIALS AND METHODS: Among participants with biomarker measurements in the Nurses' Health Study II, we used generalized linear models to assess history of infertility and plasma C-re

Shaping a knowable event or embracing a mysterious journey: A mixed methods study on palliative care clinician views on voluntary assisted dying.

OBJECTIVES: This study explored Australian palliative care clinicians' perspectives on the legalization of voluntary assisted dying (VAD), aiming to identify variables associated with clinicians' views and understand challenges of its implementation. METHODS: An online survey exploring support for legalization of VAD was sent to palliative care clinicians in Queensland and New South Wales and followed up with semi-structured interviews. Support was categorized as positive, uncertain, or negative. An ordinal logistic regression model was used to identify variables independently predictive of e

Strengthening Transitions in Care for Patients with ST-Elevation Myocardial Infarction: A Theory-Based Qualitative Study.

BACKGROUND: Despite strong evidence for secondary prevention after ST-elevation myocardial infarction (STEMI), adherence to pharmacotherapy and participation in cardiac rehabilitation remain suboptimal. Fragmented transitions from hospital to outpatient care contribute to early discontinuation, inadequate self-management support, and delayed functional and psychological recovery, particularly in regional systems without standardized follow-up care. This study examined barriers and enablers to post-STEMI transitions in care using a theory-informed qualitative approach. METHODS: Semi-structured

The Role of the Family and Community Nurse in Improving Quality of Life and Optimizing Home Care Post-COVID: A Systematic Review with Meta-Analysis.

Background/Objectives: The COVID-19 pandemic accelerated the shift toward community- and home-based care models. Within this transformation, Family and Community Nurses (FCNs) have become key in bridging hospital and primary care, supporting continuity, self-care, and quality of life (QoL). Despite increasing recognition, evidence on FCN-led interventions remains fragmented. This systematic review and meta-analysis aimed to synthesize evidence on the impact of FCN interventions on QoL and clinical outcomes in post-COVID and people living with chronic conditions managed in community and home se

Hyperuricemia in Chinese Patients with Mood Disorders: Prevalence, Related Factors, and Predictive Model.

PURPOSE: Previous studies have shown that serum uric acid (UA) levels are significantly higher in patients with bipolar disorder (BD) than in patients with depressive disorder (DD), schizophrenia, and healthy controls. Currently, studies generally report that there is a complex bidirectional interaction between mood disorders (MD) and hyperuricemia (HUA). We investigated the prevalence and related factors of hyperuricemia in Chinese patients with mood disorders to find out potential mechanisms and build a predictive model. PATIENTS AND METHODS: A total of 771 patients with mood disorders who

Actor sensemaking and its role in implementation of the decentralized drug-resistant tuberculosis policy in South Africa.

South Africa has a high burden of drug-resistant tuberculosis (DR-TB). A policy to decentralize DR-TB treatment from specialized central hospitals to more accessible district facilities was introduced in 2011, but to date implementation has been suboptimal, with variable pace, coverage, and models of care emerging. This study explored multilevel policy implementation of DR-TB decentralization in two provinces of South Africa, Western Cape and KwaZulu-Natal. Applying interpretive policy analysis, this paper describes how actors across health system levels and geographies made sense of the DR-TB

Clinical practice guidelines of foot care practice for patients with type 2 diabetes: A scoping review using self-care model.

BACKGROUND: Type 2 diabetes can lead to severe foot complications, making self-care education, guided by clinical guidelines, crucial. However, fragmented and dispersed recommendations challenge effective implementation of these guidelines. Bringing together recommendations and presenting them according to a self-care model can provide a solid framework and facilitate the interpretation of results. AIMS: to map the international guidelines that provide recommendations to nurses to enable people with type 2 diabetes for foot self-care and synthesize the recommendations according to the key con

Improving communicative access and patient experience in acute stroke care: An implementation journey.

INTRODUCTION: Patient experience for people with aphasia/families in acute care is frequently reported as negative, with communication barriers contributing to adverse events and significant long-term physical and psychosocial sequelae. Although the effectiveness of providing supported communication training and resources for health care providers in the stroke system is well documented, there is less evidence of implementation strategies for sustainable system change. This paper describes an implementation process targeting two specific areas: 1) improving Stroke Team communication with patie

NEO-SAFE: a clinical model for patients and healthcare personnel safety in primary level hospitals.

Patient safety is a major concern in medicine. Approximately, 4 million infants die each year worldwide and 23% of these deaths are caused by perinatal asphyxia. To prevent the long-term damage of asphyxia, the resuscitation flowchart must be perfectly and promptly performed. However, high effectiveness in performing resuscitation can only be achieved and maintained if the algorithm is frequently executed. Therefore, maintaining a high level of patient care is difficult in some remote centres. The aim of this study was to evaluate the effectiveness of a new organizational model of care-network
