Case Report: Cutaneous Mycobacterium chelonae infection in a kidney transplant recipient with long-term immunosuppression and eculizumab therapy.

Mycobacterium chelonae is a rapidly growing non-tuberculous mycobacterium (NTM) that causes skin and soft tissue infections, particularly in immunocompromised patients. We present the case of a 51-year-old woman receiving eculizumab therapy after kidney transplantation, who developed painful nodules and ulcers on the lower leg. Standard laboratory and imaging findings were unremarkable. Initial cultures were negative, but biopsy and subsequent wound cultures identified M. chelonae. Repeat testing confirmed the pathogen. The patient was successfully treated with a 12-month course of combination…

Pharmacological Stimulator of Interferon Genes Activation Enhances Autophagy-Mediated Clearance of Mycobacterium tuberculosis in Human Macrophages.

INTRODUCTION: The cyclic GMP-AMP synthase (cGAS)-stimulator of interferon genes (STING) pathway is a critical sensor in the innate immune response to intracellular pathogens, yet its therapeutic potential for augmenting macrophage-mediated control of Mycobacterium tuberculosis (Mtb) remains incompletely understood. This study investigated whether pharmacological activation of the STING pathway could enhance autophagy to promote Mtb clearance in human macrophages. METHODS: Human THP-1 monocytes were differentiated into macrophages and infected with Mtb. The effects of the STING agonist MIW815 …

Rapid drug resistance prediction in positive Mycobacterium tuberculosis clinical samples using an extensive targeted next-generation sequencing panel.

Tuberculosis (TB) remains a global health challenge, exacerbated by the emergence of drug-resistant Mycobacterium tuberculosis strains. Most methods for drug susceptibility testing (DST) are culture-dependent and time consuming, possibly delaying optimal TB-treatment. This study aimed to develop an extensive targeted next-generation sequencing (tNGS) approach for rapid genotypic DST directly from clinical samples. We designed a tNGS panel comprising 30 amplicons targeting 19 genomic regions associated with resistance to 20 antibiotics. This method was applied to 71 smear-positive (0-3+) pulmon…

Improved immune responses and tuberculosis protection by aerosol vaccination with recombinant BCG expressing ESX-1 from Mycobacterium marinum.

The currently licensed anti-tuberculosis (TB) vaccine, Mycobacterium bovis BCG, provides limited protection against pulmonary TB in adolescents and adults, the main cause of TB transmission and mortality. To obtain an improved BCG-based vaccine candidate with increased immune signalling but still low virulence, we have previously generated a recombinant BCG strain named BCG::ESX-1Mmar, which is heterologously expressing ESX-1 functions of Mycobacterium marinum and thereby modulates the host innate immune responses via phagosomal rupture-associated induction of type I interferon production and …

6-methylflavone exerts selective antimycobacterial activity with membrane energetics disruption and demonstrates therapeutic potential against Mycobacterium abscessus infection.

UNLABELLED: Treatment of nontuberculous mycobacteria (NTM) diseases, especially Mycobacterium abscessus pulmonary disease, remains a major clinical challenge due to its broad antibiotic resistance and limited therapeutic options. To search for new candidates, we established a synthetic flavonoid library and screened it using a bioluminescent M. abscessus reporter strain. Among the compounds tested, 6-methylflavone (6-MF) was identified as a selective inhibitor of mycobacterial growth. 6-MF showed consistent activity against multiple clinical isolates of M. abscessus with minimum inhibitory con…

Domain arrangement-driven immunogenicity of a computationally designed mRNA vaccine targeting PPE68, IrtA, and PE9 of Mycobacterium tuberculosis.

Tuberculosis (TB), caused by Mycobacterium tuberculosis, remains a major global health challenge due to the limited efficacy of the Bacillus Calmette-Guérin (BCG) vaccine. Using an immunoinformatics-driven strategy, we designed and evaluated three distinct multi-epitope vaccine constructs (MEVCs) derived from PPE68, IrtA, and PE9, which were subsequently developed into an mRNA vaccine construct. T-cell and B-cell epitopes were predicted using IEDB tools and BepiPred-3.0, and the designed constructs were systematically evaluated for antigenicity, allergenicity, toxicity, and physicochemical cha…

Hybrid sequencing reveals incompleteness of the H37Rv reference genome and highlights lineage-specific genomic divergence in Mycobacterium tuberculosis.

Background. The Mycobacterium tuberculosis H37Rv reference genome (Lineage 4), sequenced in 1998, may be incomplete and inaccurate due to earlier technology. Moreover, its use is problematic in regions like East Asia where the dominant Beijing lineage (Lineage 2) is prevalent, as it may introduce biases due to lineage-specific genetic differences.Methods. We performed hybrid sequencing to generate high-quality assemblies of a clinically isolated Beijing family strain (A2018772) and a Chinese laboratory-adapted H37Rv strain (H37Rv_CN). Comparative genomic analysis was conducted against the stan…

Therapeutic drug monitoring of intravenous amikacin during peritoneal dialysis for Mycobacterium abscessus exit-site infection: a case report.

BACKGROUND: Patients with peritoneal dialysis (PD) often develop exit-site infections (ESI) caused by nontuberculous mycobacteria. Antimicrobial therapy, including intravenous amikacin (AMK), is combined with PD catheter removal to control the source of infection. However, the efficiency of AMK removal using PD remains unclear. In this report, we describe AMK pharmacokinetics before and after PD catheter removal in a patient diagnosed with Mycobacterium abscessus from ESI. CASE PRESENTATION: A 73-year-old female PD patient weighing 42.4 kg who received intravenous AMK + clarithromycin + imipe…

mNGS-Identified Mycobacterium porcinum Infection in a Newly Diagnosed Person With HIV Presenting With Recurrent Suppurative Cervical Lymphadenitis.

Although reports of human infection caused by Mycobacterium porcinum (M. porcinum) have gradually increased in recent years, cases occurring in people with HIV (PWH) remain rare, and the association between M. porcinum infection and suppurative cervical lymphadenitis in PWH has not been previously reported. In this case, metagenomic next-generation sequencing was used to rapidly identify M. porcinum from a pus specimen obtained from a newly diagnosed person with HIV presenting with suppurative cervical lymphadenitis as the initial manifestation. Recognition of these rare clinical features may …

Comparative Phenotypic Screening Identifies Protein Synthesis Inhibitors as Compounds That Enhance Early Acidification of Mycobacterium tuberculosis in Macrophages.

Treatments for tuberculosis disease (TB) are failing on a global scale, necessitating new drug discovery efforts to identify effective therapies. Mycobacterium tuberculosis (Mtb), the obligate human pathogen and causative agent of TB, resides within alveolar macrophages and combats acidic stress by inhibiting phagosome maturation. Understanding the role of acidification in Mtb pathogenesis can aid the development of improved TB therapies. Aligning with the hypothesis that acidification affects Mtb intracellular survival, we developed a phenotypic assay to identify compounds that enhance acidif…

Single-cell transcriptomics reveal PD-1-loss-driven immune dysregulation of pulmonary lymphocytes during early Mycobacterium tuberculosis infection.

Although PD-1 blockade shows strong antitumor efficacy and is being considered for chronic infections including Mycobacterium tuberculosis (M.tb), its influence on early lymphocyte responses to M.tb remains poorly understood. In this study, we characterized the transcriptional and developmental states of pulmonary B and T cells in mice lacking PD-1 after 7 days post-M.tb infection. We found that loss of PD-1 disturbed pulmonary lymphocyte homeostasis, characterized by abnormal regulatory T cell (Tregs) expansion, early exhaustion-like differentiation of cytotoxic T cells, and impaired maturati…

Understanding Mycobacterium tuberculosis host immune responses and identifying synergistic autophagy-inducing compounds as potential nanomedicines for tuberculosis treatment.

Tuberculosis (TB), caused by Mycobacterium tuberculosis (M. tb), poses a significant global health challenge and is currently the most fatal disease resulting from a single infectious agent. The development of drug-resistant strains, thin antibiotic pipeline, and limitations of current treatment regimens necessitate the development and exploration of new anti-TB treatmnt options. Host-directed therapies (HDTs) have emerged as a promising treatment option, due to their ability to modulate the host's immune system and target cellular processes vital for the replication and pathogenesis of invadi…
