[High exposure of the distal internal carotid artery].

High exposure of the internal carotid artery is a challenging procedure even for experienced surgeons. Access to the distal internal carotid artery is impeded by progressive encroachment of the mastoid process and the angle of the mandible and furthermore at this level the artery is intimately associated with the hypoglossal and glossopharyngeal nerves. If high exposure is needed we prefer preparation of the distal internal carotid artery by dissection of the venter posterior of the digastric muscle and the styloid process including the stylohyoid and stylopharyngeus muscles. This procedure ca

Carcinoma of the buccal mucosa metastasizing to the talus.

Head and neck squamous cell carcinomas (HNSCCs) are tumors with propensity mostly for locoregional spread. Most frequent sites of metastasis from cancer of the oral cavity include lung, bone, liver, adrenal, heart, and kidney. The advent of newer surgical techniques and an improved understanding of head and neck cancer have further improved control of cancer above the clavicles. Furthermore, the overall survival rate in patients with advanced head and neck cancer has not improved significantly. This has been partly because of the emergence of second primary cancers and the development of dista

[Surgical treatment of laryngotracheal cicatricial stenoses].

An analysis of results of 18 laryngotracheal resections in patients after prolonged artificial pulmonary ventilation with circular, A-shaped or mixed forms of stenosis has shown that one-stage sleeve resection of the larynx below the glottis and upper third of the trachea with the formation of primary anastomosis is possible without placing a stent when they have cicatricial lesion. In patients with laryngotracheal stenosis the sleeve laryngotracheal resection is recommended with primary thyrocricoid tracheal anastomosis as a comparatively safe and effective reconstructive operation giving goo

Complications after superficial plane cervicofacial rhytidectomy: a retrospective analysis of 178 consecutive facelifts and review of the literature.

PURPOSE: It is well-recognized that postoperative hematoma is the most common complication of cervicofacial rhytidectomy (facelift surgery), and several strategies aimed at reducing the incidence of hematoma have been developed and studied. Other common complications include skin necrosis, seroma, motor and sensory nerve injury, alopecia, auricular deformity/displacement, and dyspigmentation. The purpose of this study was to retrospectively analyze the incidence of complications after superficial cervicofacial rhytidectomy performed by a single surgeon and his fellows and to compare our findin

[Surgical management of aged patients with hypopharyngeal carcinoma and preservation of laryngeal function].

BACKGROUND & OBJECTIVE: The prolongation of life expectancy results in an increasing number of aged patients with hypopharyngeal carcinoma who have poor surgical tolerance because of degenerative organ functions. This study was to explore the surgical methods and preservation of laryngeal function for aged patients with hypopharyngeal carcinoma. METHODS: Clinical data of 84 patients with hypopharyngeal carcinoma who aged above 60 years, treated from 1996 to 2001, were retrospectively reviewed. Of the 84 cases, 68 were originated from pyriform sinus, 5 from postcricoid area, 11 from posterior

Total thyroidectomy for differentiated thyroid carcinoma: primary and secondary operations.

AIMS: There is considerable controversy concerning the most appropriate surgical treatment of patients with differentiated thyroid carcinoma (DTC). Although some authors have advocated subtotal thyroidectomy because of the decreased surgical morbidity and the lack of improved survival with a more extensive procedure, total thyroidectomy has been defended by others as a treatment of choice with lower morbidity. METHODS: We reviewed 106 consecutive patients who had been treated with total thyroidectomy for DTC to determine the complication rate. Forty-seven patients had primary operations and 5

Pattern of recurrence after extended radical esophagectomy with three-field lymph node dissection for squamous cell carcinoma in the thoracic esophagus.

Factors responsible for recurrence of esophageal cancer were investigated in 90 patients who underwent extended radical esophagectomy with three-field dissection for a squamous cell carcinoma in the thoracic esophagus. The initial tumor recurrence was grouped as either locoregional (site of the primary tumor, anastomotic site, or lymph nodes) or as distant (distant organs, pleura, or peritoneum). Nineteen patients (21%) developed a locoregional recurrence, and 19 (21%) developed a distant recurrence. One (1%) developed both recurrences simultaneously and was classified as a distant recurrence.

Infection-associated cervical artery dissection. Three cases.

BACKGROUND: The pathogenesis of cervical artery dissection remains unknown. Infection-mediated damage of the arterial wall may be one contributing mechanism. We present three male patients with respiratory infection prior to cervical artery dissection. CASE DESCRIPTIONS: Case 1: During an upper respiratory tract infection, a 49-year-old patient developed bilateral carotid and vertebral artery dissection with complete vessel restitution. Case 2: Within 3 years, a 40-year-old patient experienced two episodes of bilateral internal carotid artery dissection, both preceded by febrile upper respira

Squamous carcinoma of the gums.

We reviewed a 20-year experience with squamous carcinoma of the gums in 347 patients who received definitive therapy. More than three-quarters of the lesions involved the lower gum and all but 37 patients were previously untreated. The proportion of patients with localized tumors (N0) remained the same (64 percent). Surgery continued to be the treatment of choice in 97 percent of patients, but proportionately more patients had a more conservative procedure which preserved lower jaw continuity. The 5-year determinate survival rate was little changed (54 percent). Advanced clinical stage (stages

Free jejunal graft for reconstruction of oral, oropharyngeal, and pharyngoesophageal defects.

A free jejunal graft was used either as a patch or as a conduit in 40 cases of reconstruction of the oral cavity, hypopharynx, and upper esophagus. Compared with previous intraoral reconstructive techniques using skin or myocutaneous flaps, one-stage free jejunal grafts result in low morbidity, rapid postoperative recovery, and no additional head and neck scars. Because of secure vascularization, the retention of temporary mandible implants is made safer. In addition to tongue reconstruction, facial contouring can also be achieved by the use of mesenteric fatty tissue. The well-vascularized ph

Squamous-cell carcinoma of the tongue: preoperative interstitial radium and external irradiation. Part I: Local and regional control.

Results of therapy in 300 patients with squamous-cell carcinoma of the anterior two thirds of the tongue are presented in terms of control of the primary tumor (local) and metastases to the lymph nodes of the neck (regional). Correlation between clinical stage and control was good, with the best results being obtained in stage T1N0 (84% control of tongue, 69% control of neck) and the poorest results in TX N2N3 (9% and 11%, respectively). Different methods of treatment were analyzed separately for various stages of disease and compared with results from other institutions. Possible methods of i

Cervical arterial dissection: time for a therapeutic trial?

BACKGROUND AND PURPOSE: Cervical arterial dissection is a major cause of stroke in young adults, yet despite standard treatment with anticoagulants or antiplatelet drugs, its management remains uncertain. The goal of this study was to assess the natural history of the disorder and to decide on the feasibility of a therapeutic trial. METHODS: Collaborating members of the Canadian Stroke Consortium prospectively enrolled consecutively referred patients with angiographically proven acute vertebral or carotid arterial dissection. Data recorded included clinical and radiological details, recurrenc

Transpectoral ultrasound-guided catheterization of the axillary vein: an alternative to standard catheterization of the subclavian vein.

Subclavian vein catheterization is associated with failure and complications because of injury to the nearby lung and subclavian artery. Its position, sandwiched between the clavicle and the first rib, makes sonographic imaging difficult. The medially pointed sonography probe makes it difficult to align the needle as well as image the entire needle. The axillary vein lies outside of the thoracic cage and can be easily imaged in its longitudinal view along with the entire needle, guidewire, dilator, and catheter in real-time. All described techniques of venous access using sonography have used

[Palliative chemotherapy of recurring squamous cell carcinomas of the ENT field with the combination cis-diamminodichloroplatinum (II)/adriamycin].

23 patients with recurrent squamous cell carcinoma of head and neck (SCCHN) were treated with a combination of cisplatin 120 mg/m2 and adriamycin 60 mg/m2, both administered by I.V. infusion over 4 hours and 15 minutes, respectively, at day 1. The courses were repeated at 3-weeks interval. Hydration and diuresis forced via mannitol were much lower than usual with such high doses of cisplatin. All patients were intensively pretreated by surgery and/or radiotherapy, further local treatment being impossible; all had loco-regional recurrence, 2 distant metastases additionally. 21/23 patients could

Carotid chemodectomas: questions to ensuring radicality, efficacy, and safety of surgical treatment.

BACKGROUND: In Russia and the world, there is an increase in the number of cancer patients with localization on the head and neck. According to the Global Cancer Observatory (GLOBOCAN), 890.000 new cases of head and neck cancer and 450.000 deaths were registered in 2018, with a forecast of a 30% increase in the incidence by 2030. Carotid chemodectomas (CC) account for 0.5% of all head and neck tumors. The frequency of occurrence is estimated as 1 per 30000-100000 people. CCS in most patients are benign, but 10% may be malignant. The aim to determine the rational surgical tactics in patients wi
