PURPOSE: The postoperative course of median nerve decompression in carpal tunnel syndrome may be associated with complications. The aim of this study was to explore the possible effects of alpha-lipoic acid (ALA) in the postoperative period after surgical decompression of the median nerve at the wrist.
METHODS: We conducted a double-blind prospective, randomized, controlled trial. A total of 64 patients with proven carpal tunnel syndrome were enrolled and randomly assigned into 1 of 2 groups: group A (n = 32) patients had surgical decompression of the median nerve followed by ALA for 40 days,

Krabbe disease, or globoid cell leukodystrophy, is a rare disorder caused by deficient galactosylceramidase activity and loss of myelin-forming oligodendrocytes, resulting in progressive demyelination and severely impaired motor function. Disease symptoms in humans appear within 3-6 months of age (early infantile) and manifest as marked irritability, spasticity, and seizures. The disease is often fatal by the second year of life, with few effective treatment options. Herein we evaluated the therapeutic potential of mesenchymal stem cells (MSCs) administered intracranially to a 1-month-old rhes

The distinction between myasthenia gravis and Lambert-Eaton myasthenic syndrome is based on clinical, neurophysiological and immunological features. We hereby report two cases with a clinical diagnosis of myasthenia gravis and neurophysiological features consistent with a pre-synaptic neuromuscular transmission defect. Both patients had increased anti-acetylcholine receptor antibody titres and showed a good response to cholinesterase inhibitors, along with a >100% facilitation of the compound muscle action potential on electrophysiological studies. We provide a review of English literature stu

OBJECTIVE: Although patients with acute inflammatory demyelinating polyneuropathy (AIDP) are frequently admitted a few days after symptoms onset, electrodiagnostic (EDX) abnormalities in early AIDP are not well characterized. Our aim was to determine the EDX pattern of early AIDP, and, if needed, to propose new EDX diagnostic criteria.
METHODS: In this monocentric study, we retrospectively reviewed the clinical and EDX data of 58 consecutive AIDP patients in whom EDX studies had been performed within 7 days after disease onset, representing 46% of all GBS patients admitted in our hospital in

The objective of this study was to analyze Charcot-Marie-Tooth disease type 1A (CMT1A) evolution. We conducted a 2-year longitudinal study in 14 CMT1A patients and 14 age- and sex-matched controls. In the patients, we performed neurological examination with hand-held dynamometry, electrophysiology, and lower-limb muscle MRI, both at baseline and 2 years later, while controls were examined at baseline only. Patients' ages ranged from 12 to 51 years. Outstanding manifestations on initial evaluation included pes cavus, areflexia, lower-limb weakness, and foot hypopallesthesia. In evaluating muscl

BACKGROUND: The Semmes-Weinstein Monofilament Test (SWMT) is a clinical widely used test to quantify the sensibility in patients with Carpal Tunnel Syndrome (CTS). No study has investigated the relationship between the SWMT and sensory nerve conduction studies (SNCS) in patients with CTS.
OBJECTIVE: To assess the relationship between the SWMT and SNCS findings in patients with CTS.
METHODS: This cross-sectional clinical measurement study included 35 patients with CTS (55 hands) with a mean age of 45 ± 12 years. The outcome measures were the SWMT and SNCS measures of distal latency (DLs), amp

INTRODUCTION: L5 radiculopathy has characteristic clinical and electrodiagnostic features including: radicular pain; weakness or denervation of hip abductors, ankle dorsiflexors, and inverters; and pre-ganglionic dorsal foot sensory loss. It is unknown how often patients with this distinctive clinical-electrodiagnostic presentation have isolated L5-root compression on neuroimaging or more widespread, possibly age-related, lumbar neuroforaminal or spinal stenosis.
METHODS: A study-blinded neuroradiologist quantitated lumbosacral neuroforaminal, lateral recess, and spinal stenosis in 26 consecu

OBJECTIVES: To determine whether repetitive tibial nerve stimulation (RTNS) affects neurogenic claudication and F-wave conduction in lumbar spinal stenosis.
DESIGN: An intervention study: before/after trial.
SUBJECTS: Data for 12 central lumbar spinal stenosis patients were compared with 13 age- and sex-matched healthy volunteers.
METHODS: A conditioning RTNS at the ankle, 0.3-ms duration square-wave pulses with an intensity 20% higher than the motor threshold, was applied at a rate of 5/s for 5 min. We assessed the effects of RTNS on the claudication distance at which the lumbar spinal ste

West Nile virus infection is asymptomatic in most cases. West Nile virus neuroinvasive disease includes encephalitis, meningitis, and/or acute flaccid paralysis. In children, acute flaccid paralysis as the solo presentation of West Nile virus disease is rare. It develops abruptly and progresses rapidly early in the disease course. We report on a 10-year-old child who presented with a slowly progressive left leg flaccid paralysis over 4 weeks. He tested positive for West Nile virus in both blood and cerebrospinal fluid. Spinal MRI showed enhancement of the ventral nerve roots. This was also sup

Behavioral function lost in mammals (including humans) after peripheral nerve severance is slowly (weeks to years) and often poorly restored by 1-2-mm/day, nonspecifically directed outgrowths from proximal axonal stumps. To survive, proximal stumps must quickly repair (seal) plasmalemmal damage. We report that, after complete cut- or crush-severance of rat sciatic nerves, morphological continuity, action potential conduction, and behavioral functions can be consistently (>98% of trials), rapidly (minutes to days), dramatically (70-85% recovery), and chronically restored and some Wallerian dege

INTRODUCTION: In this study we investigated the changes in axonal excitability and the generation of neurological symptoms in response to focal nerve compression (FNC) of the median nerve in carpal tunnel syndrome (CTS).
METHODS: Sensory excitability recordings were undertaken in 11 CTS patients with FNC being applied at the wrist using a custom-designed electrode.
RESULTS: During FNC, refractoriness increased significantly (62.4 ± 3.4%; P < 0.001), associated with a rapid reduction in superexcitability (16.9 ± 2.8%; P < 0.001) and sensory nerve action potential amplitude (SNAP) (32.4 ± 3.9%

BACKGROUND: Guillain-Barriota syndrome (GBS) is an acute neurological illness affecting the peripheral nervous system causing significant morbidity. The syndrome has been classified electrophysiologically into demyelinating and axonal forms. The clinical features of the axonal variant (acute motor axonal neuropathy--AMAN) are not well characterized, particularly in the Indian context.
OBJECTIVE: To assess the incidence, clinical presentation and disease course of patients with AMAN.
MATERIALS AND METHODS: This is an observational study done at a tertiary referral center in South India. The s

Early forms of complex regional pain syndromes (CRPS) are characterized by severe pain and autonomic dysfunction in a limb, both of which seem out of proportion to the inciting event. While often caused by obvious nerve injury, the syndromes also occur following relatively trivial trauma. Persistent inflammation has been implicated in the etiology of CRPS. We hypothesized that inflammation of a nerve proximal to the symptoms could lead to neural changes consistent with clinical CRPS. Using a rat model of neuritis, the activity of sensory and autonomic neurons was recorded distal to the inflame
