[Emphasizing the critical issues in perioperative management of children with congenital cataract].

Cataract is the main cause of treatable visual impairment in children. Children face more risks during the perioperative period of cataract surgery than adults because of their anatomical, physiological, behavioral, and developmental characteristics; therefore, special attention and management are required. This article comprehensively discusses the perioperative management of children's cataract surgery on the base of relevant literatures and the authors' clinical experience, including the indications and timing of surgery, preoperative examination, prevention and treatment of surgical compli

Ocular toxicity after inadvertent overdose of intracameral cefuroxime during cataract surgery.

PURPOSE: To describe intracameral toxic effects of cefuroxime at a higher dose than recommended in cataract surgery. METHODS: Retrospective study of 8 eyes of 8 patients who inadvertently received 12.5 mg/0.1 ml of intracameral cefuroxime due to a dilution error, at the end of the cataract surgery. All patients underwent a strict ophthalmology follow-up for 6 months. RESULTS: All patients presented with a marked anterior segment inflammation with corneal oedema that resolved completely in all cases (between 5 days and 3 months). At 6 months of follow-up a statistically significant difference…

Glaucoma screening using an attention-guided stereo ensemble network.

Glaucoma is a chronic eye disease, which causes gradual vision loss and eventually blindness. Accurate glaucoma screening at early stage is critical to mitigate its aggravation. Extracting high-quality features are critical in training of classification models. In this paper, we propose a deep ensemble network with attention mechanism that detects glaucoma using optic nerve head stereo images. The network consists of two main sub-components, a deep Convolutional Neural Network that obtains global information and an Attention-Guided Network that localizes optic disc while maintaining beneficial

Chemical disinfectants in ophthalmic practice.

The instrumentation used in ophthalmic clinics can be a source of epidemics in health care set up. Contact tonometry with Schiotz or Applanation tonometer is associated with nosocomial epidemic keratoconjunctivitis outbreaks. Recently identified SARS-CoV-2 (COVID -19) spreads mainly via the respiratory route and fomites and can transmit through other body fluids, including tear film. Various ophthalmic instruments can become a common source of spreading cross infections. Chemical disinfection is one of the most common methods employed to decontaminate instruments and environmental surfaces and

Dexamethasone Intracanalicular Insert: A Review in Treating Post-Surgical Ocular Pain and Inflammation.

Dextenza®, an intracanalicular insert that is placed into the lower punctal opening of the eye, gradually releases dexamethasone for up to 30 days to alleviate pain and inflammation associated with ophthalmic surgery. A significantly higher proportion of patients treated with the dexamethasone intracanalicular insert than with the placebo insert had no pain at day 8 (co-primary endpoint, 7 days post-operation) across three pivotal phase III trials, and the inflammation co-primary endpoint (absence of anterior chamber cells) at day 14 (13 days post-operation) was met in two of three trials. Medical and surgical management of conjunctivochalasis.

Conjunctivochalasis (CCH) is a bilateral conjunctival condition characterized by loose, redundant conjunctival folds, typically in the inferior bulbar conjunctiva. It is a common cause of ocular irritation, especially in older age. For asymptomatic CCH, no treatment is necessary. For treatment of symptomatic CCH, however, a variety of medical and surgical approaches are currently available, which will be thoroughly appraised in this review article. The first step in the management is medical therapy, which involves enhanced lubrication and use of anti-inflammatory medications. In refractory ca

Imaging Amblyopia: Insights from Optical Coherence Tomography (OCT).

Amblyopia refers to visual impairment resulting from perturbations in visual experience during visual development, typically secondary to strabismus, uncorrected refractive error, and/or deprivation. Amblyopia has traditionally been considered a cortical disease, but the depth of our understanding of this complex neurodevelopmental condition is limited by our ability to appreciate structural pathophysiology in the visual pathway. Recent advances in Optical Coherence Tomography (OCT) have facilitated numerous studies of the structural changes in the retina and optic nerve, thereby expanding our

Neuroblastoma of the Iris in Children.

Neuroblastoma of the iris is an extremely rare clinical entity. An otherwise healthy 2-month-old male infant presented to the oncology clinic with a nodular whitish iris lesion in his right eye. The excisional tumor biopsy was consistent with a pathological diagnosis of neuroblastoma with differentiation and negative MYCN gene mutation. Further systemic evaluation revealed a right adrenal mass with no metastatic lesion. The biopsy of the adrenal lesion was also consistent with neuroblastoma. After four courses of chemotherapy, the adrenal mass was completely resected. The patient underwent two

Multi-indices quantification of optic nerve head in fundus image via multitask collaborative learning.

Multi-indices quantification of optic nerve head (ONH), measuring ONH appearance with multiple types of indices simultaneously from fundus images, is the most clinically significant tasks for accurate ONH assessment and ophthalmic disease diagnosis. However, no attempt has been reported due to its challenges of the large variation of fundus appearance across patients, heavy overlap and extremely weak contrast between optic nerve head areas. In this paper, we propose a multitask collaborative learning framework (MCL-Net) for multi-indices ONH quantification. The proposed MCL-Net, a two-branch n

Multicentric Mucinous Adenocarcinoma in the Periorbital Region.

Primary mucinous adenocarcinoma of the skin is an uncommon malignancy in clinical practice, but multicentric presentation of the malignancy is considered even more rare. In this case report, the authors present a 70-year-old woman with multicentric primary mucinous adenocarcinoma of the skin manifesting with 2 separate lesions located on the right eyelid and cheek. Lesion removal and immunohistochemical staining ruled out mucinous adenocarcinoma of the skin secondary to lung or thyroid carcinoma, however, was inconclusive for breast carcinoma. A negative breast examination and mammography dete

Plus Disease in Retinopathy of Prematurity: More Than Meets the ICROP?

Retinopathy of prematurity (ROP), a vasoproliferative retinal disease affecting premature infants, is a leading cause of childhood blindness throughout the world. Plus disease, defined as venous dilatation and arteriolar tortuosity within the posterior retinal vessels greater than or equal to that of a standard published photograph, is the most critical finding in identifying treatment-requiring ROP. Despite an internationally accepted definition of plus disease, there is significant variability in diagnostic process and outcome, producing variable levels of reported intra- and interexpert agr
