Inflammasome-independent IL-1β mediates autoinflammatory disease in Pstpip2-deficient mice.

Chronic recurrent multifocal osteomyelitis (CRMO) is a human autoinflammatory disorder that primarily affects bone. Missense mutation (L98P) of proline-serine-threonine phosphatase-interacting protein 2 (Pstpip2) in mice leads to a disease that is phenotypically similar to CRMO called chronic multifocal osteomyelitis (cmo). Here we show that deficiency of IL-1RI in cmo mice resulted in a significant reduction in the time to onset of disease as well as the degree of bone pathology. Additionally, the proinflammatory cytokine IL-1β, but not IL-1α, played a critical role in the pathology observed …

Chronic infections of the spine: surgical indications and treatments.

UNLABELLED: Chronic vertebral osteomyelitis is a disease of substantial morbidity. Although uncommon to most spinal surgeons, the incidence of pyogenic and granulomatous spondylitis worldwide is on the rise. Although antibiotic therapy remains the initial treatment for most patients, surgical debridement with or without stabilization may be required for effective eradication of the disease. Indications for surgery in pyogenic and granulomatous osteomyelitis include the need to obtain a bacteriologic diagnosis when other methods have failed, the presence of a clinically significant abscess, an …

Cefsulodin treatment for serious Pseudomonas aeruginosa infections.

Cefsulodin, a narrow-spectrum cephalosporin with excellent antipseudomonal activity was used to treat 48 patients with 51 Pseudomonas aeruginosa infections. These included osteomyelitis, infected prostheses, post-operative and post-traumatic superficial wounds, decubitus and stasis ulcers, lower respiratory tract infections and infections of the urinary tract. Many of the patients were compromised by underlying debilitating conditions such as severe trauma, diabetes mellitus, vascular impairment, and abuse of alcohol and drugs. In cases of polymicrobial infections, a concomitant non-antipseudo…

[Osteomyelitic pseudarthrosis of the tibia with loss of substance treated with a graft of the opposite fibula].

Dragan Dj./Re. No. 2755/76, 5,5-year-old boy, referred to this hospital with signs of bipolar osteomyelitis, destruction and fistulisation of the left tibia and talocrural arthritis. Staphylococcus albus and streptococcus haemoliticus were isolated from the fistula. Osteomyelitis was treated with an adequate therapy including long usage of Lincocin. In Feb. and Oct. of 1977 a sequestrotomy of the mid portion of the tibia and transmedular fixation using Ratschmann's nail were performed. The process resulted in pseudarthrosis of the mid portion of the tibia with loss of bone substance in the len…

Brodie's Abscess of the Thumb: A Rare Complication of a Dog Bite.

Brodie's abscess is a rare form of subacute osteomyelitis, most commonly affecting the metaphysis of long bones in pediatric patients. Involvement of the hand is exceptional, and its indolent course combined with nonspecific imaging features can make diagnosis challenging. Here, we describe a case of Brodie's abscess following an animal bite and compare it with the few reported cases of Brodie's abscess of the hand in the literature.

Congenital insensitivity to pain with anhidrosis: a case report.

Although it is very rare, the syndrome of Congenital Insensitivity to Pain with Anhidrosis (CIPA) should be known to anaesthetic practitioners due to its perioperative implications. We report the first case of this in the British Isles, in a 5-year-old girl, whose osteomyelitis required surgical intervention. The genetics, the cellular basis and pathophysiology of the condition are outlined, and the relevance of analgesia in this challenging setting is considered in this report.

Paradoxical keratitis and dermatitis following adalimumab treatment.

Anti-tumor necrosis factor monoclonal antibodies are an important tool in the management of rheumatologic disease. However, paradoxical inflammation can be precipitated by their use. This case report describes the development of keratitis and dermatitis following adalimumab treatment for chronic recurrent multifocal osteomyelitis in a 6-year-old girl. The keratitis and dermatitis subsided once topical steroids were started and the adalimumab was held.

Reactivated Moraxella osteitis presenting as granulomatous disease.

Granulomatous infections are commonly associated with mycobacteria, brucellosis, actinomycosis, nocardiosis, spirochetes, and fungi. Rarely, granuloma formation is a host response to other bacterial infection. Osteomyelitis and osteitis that reactivate many years after the primary episode is a known phenomenon. A reactivation that presents as a granulomatous disease is rare. We present a case of reactivated osteitis due to Moraxella osloensis with consecutive granuloma formation.

Diffuse large B-cell lymphoma of the maxillary sinus.

We report the case of a 25-year-old man with a history of recent facial trauma who presented with a mildly painful swelling of the left maxilla and nasal obstruction. The patient was initially diagnosed with left maxillary sinus osteomyelitis secondary to undiagnosed and untreated left maxillary sinus fractures. However, further evaluation revealed that the patient had diffuse large B-cell lymphoma.

Atypical Pott's disease: localized infection of the thoracic spine due to Mycobacterium avium-intracellulare in a patient without human immunodeficiency virus infection.

Mycobacterium avium-intracellulare (MAI) rarely causes disease of the spine in healthy individuals. We describe an elderly woman who had isolated skeletal involvement with MAI, mimicking Pott's disease. She responded well to surgical excision of the inflamed tissue and antibiotic therapy. Osteomyelitis due to MAI must be differentiated from that due to Mycobacterium tuberculosis because the treatment regimens are different.

Osteopetrosis is a rare inherited bone disease that affects both humans and various mammals. The authors report on two cases of osteopetrosis with otolaryngological complications. [An unusual complication after tracheal resection for cicatricial stenosis].

The authors describe the case of a patient where after segmental resection of the trachea on account of cicatricial postcannulation stenosis of the trachea osteomyelitis of the sternum developed with a tracheocutaneous fistula. The cause of the fistula was probably a small dehiscence not detected after operation at the site of the anastomosis with subsequent development of chronic inflammatory changes in the anterior mediastinum and sternum. The authors describe the diagnostic procedure and surgical solution of this uncommon complication.

Recurrent brain abscess due to an unexpected foreign body.

We report an exceptional case of a patient with chronic frontal sinusitis complicated by chronic osteomyelitis and a cutaneous fistula. A recurrent brain abscess developed and was only cured after a very unusual wooden retained foreign body was removed at surgery. The hazards of wood as a foreign body are discussed and it is stressed that the possibility of a retained foreign body, even unsuspected, must always be borne in mind.

[Cranial osteitis: a report on four cases, including a Gradenigo-Lannois syndrome (author's transl)].

Three cases of hematogenous cranial osteitis are described. The etiology in two cases was a staphylococcus aureus infection from meningeal septicemia. One of these patients died and chronic osteomyelitis developed in the second case. The third case was one of multifocal tuberculous infection, localized in the cranium but asymptomatic. The last case was a Gradenigo-Lannois syndrome, and its etiology, either chronic otitis or sphenoidal sinusitis, is discussed.
