A rare complication of extracapsular cataract extraction is hypotony due to capsular shrinkage, with consequent ciliary body detachment. A classic hypotony syndrome with progressive loss of visual acuity was observed in two patients, one with extracapsular aphakia and one with extracapsular pseudophakia.
