BACKGROUND CONTEXT: Posterior decompressions in the form of laminectomies for vertebral body tumors have poor outcomes. Surgical management typically requires anterior decompression and reconstruction; however, these procedures can be associated with significant morbidity and mortality.
PURPOSE: To evaluate the feasibility of anterior spinal column reconstruction using an expandable cage through a posterior approach.
STUDY DESIGN/SETTING: Multicenter consecutive case series of 21 prospectively followed patients.
PATIENT SAMPLE: Twenty-one patients with vertebral body tumors treated with ant

BACKGROUND: The authors propose an expanded technique of the temporoparietal facial flap of sufficient size to provide complete coverage in a single-stage procedure for the projected three-dimensional autogenous cartilage framework, including the tragus or lobule.
METHODS: A temporoparietal fascial flap measuring 9 x 9 cm was prepared and wrapped around an expander. The expander was gradually infused with saline solution. Approximately 6 months after the first operation, a projected three-dimensional auricular framework was covered by the expanded temporoparietal fascial flap. Nine patients u

Anomalies of the urogenital sinus have been described in association with male pseudohermaphroditism, bladder and uterine duplication, and a spectrum of external genital anomalies, including a female phallic urethra. The evaluation and optimal surgical management of these anomalies can be complex. We describe a case of a female patient with an accessory phallic urethra and a urogenital sinus anomaly. The presence of stenosis of the accessory urethral meatus led to the formation of a giant diverticulum with unusual features. The complete repair of the anomaly incorporated creation of labia mino

The possibility of transmission of slow virus infection (HIV) and Creutzfeld-Jakob disease by cadaveric dura implants makes it necessary to find synthetic, absorbable materials for the reconstruction of the dura mater. Various procedures with autologous or alloplastic material are described. Four commercially available biomaterials were choosen to study the proliferation rate and the biocompatibility of human osteoblast-like cells (HOB-like cells) on 2- dimensional material by biochemical analysis. With a proliferation assay, the viability and the proliferation capacity of osteoblast-like cell

OBJECTIVE: To study the clinical effects, the merits and shortcomings of the hard palate flap in repairing postoperative defects of oral soft tissue.
METHODS: The whole-palate flap pedicled with the unilateral greater palatine artery was used to repair 25 cases of oral soft tissue defects.
RESULTS: Of the 25 cases, 22 flaps survived completely and 2 flaps survived partially. The postoperative appearance and functions were satisfactory.
CONCLUSION: Compared with other flaps, the hard palate flap is easier to harvest and use. It has reliable blood supply and does not harm the patients' appear

BACKGROUND: Reconstruction in the periorbital area is challenging owing to the complex function of the eye, relative lack of adjacent loose tissue, free anatomic margin, central facial location, and the need to maintain symmetry with the contralateral eye. Reconstructive options risk crossing anatomic margins, deviation of the lid margin (ectropion), persistent lymphedema, and repair with skin of dissimilar color, texture, and thickness.
OBJECTIVE: The purpose was to describe a reconstructive option that would avoid crossing cosmetic units or subunits, minimize the risk of ectropion, repair w

An intraoral prosthesis was fabricated to restore more normal contour to a patient's lower lip that was compromised following a marginal mandibulectomy performed to eradicate a malignant neoplasm. The surgical resection compromised the lower lip on the affected side because the marginal mandibular branch of the facial nerve was damaged during the surgical resection. The prosthesis successfully restored the lost lip support, reduced the incidence of lip biting, and improved the patient's oral competency. This clinical report describes the procedure for making an intraoral lip plumper prosthesis

PURPOSE OF REVIEW: Rehabilitation of the congenitally anophthalmic orbit is frustrating to both the parents and physician. Traditional methods involve using progressively enlarging static acrylic conformers to expand the conjunctival socket, followed by placement of conventional static spherical orbital implants, dermis-fat grafts, or inflatable balloon expanders for orbital enlargement. Limitations of these methods typically result in less-than-optimal cosmetic outcomes, with retardation of bony orbital and overlying soft tissue growth adversely affecting midfacial growth and symmetry. Recent

OBJECTIVE: To evaluate the clinical efficiency of fresh amniotic membrane transplantation in treatment of stenosis of conjunctival sac.
METHODS: Thirty cases (30 eyes) of stenosis of conjunctival sac were treated with fresh amniotic membrane transplantation. Amniotic membrane was obtained under sterile conditions after elective cesarean delivery. The woman's serum was negative for HBsAg, syphilis, and human immunodeficiency virus. The placenta was first washed free of blood clots with sterile saline. Under sterile conditions, the inner amniotic membrane was separated from the chorion by blunt

Patent microvascular anastomoses are mandatory for a successful free tissue transfer. Dextran 40 is widely used by reconstructive microsurgeons in conjunction with free tissue transfer, to prevent flap loss. Unfortunately, dextran-induced adverse reactions, such as anaphylactoid reactions, adult respiratory distress syndrome, cardiac overload, hemorrhage, and renal damage, remain the major risks in routine use of dextran 40. The authors retrospectively analyzed the patency rates of 55 microvascular tissue transfers of a single microsurgeon after tumor ablation of malignancies of the upper aero

A vascularized bone segment of the ulna together with a posterior interosseous fasciocutaneous flap is harvested, including a cuff of the extensor pollicis longus muscle. The authors treated five male patients with metacarpal bone and soft-tissue defects of the hand using a distally based island osteocutaneous posterior interosseous flap. Their ages at the time of surgery ranged from 15 to 37 years (mean, 24 years). The bone defects were in the first metacarpal in three cases, the fourth metacarpal in one, and the fifth metacarpal in one. The length of the donated ulna ranged from 3 to 7 cm (m

A systematic approach of the surgical management of a calcaneal fracture can minimize the potential of soft tissue complications. When reducing a closed calcaneal fracture, the incision used affects the postoperative complications. The L-shaped incision with the horizontal limb lying on the lateral glabrous junction ensures maximum blood flow to either side of the incision. Whether or not the wound can be closed primarily depends on the preexisting edema, the lost calcaneal height, and the delay between the fracture and reduction (Fig. 20). The wrinkle test is a good indicator that the incisio

Reconstruction surgery for a female to male transsexual usually involves mastectomy, hysterectomy and creating an aesthetically appealing neophallus. We have successfully inserted an inflatable prosthesis using the AMS CX prosthesis in a 45 year old transsexual, who had a large bulky neophallus constructed from the anterior abdominal subcutaneous fat, about 9 years ago. The single cylinder CX prosthesis was well anchored to the symphysis pubis using a dacron windsock tubing, the activation pump was placed in the dependent pouch of the right labium and the reservior in the usual perivesical spa

Since 1986, five cases of one-stage reconstruction of the injured penis using microsurgical techniques have been successfully carried out. A piece of ilium based upon the deep circumflex iliac vessels is used to form the penis prop and a skin flap of the forearm is transferred to form the urethra and foreskin to encircle the prop. Then the vessels and nerves of the skin flap of the forearm are anastomosed with the vessels and nerves of the thigh. Postoperative follow-up showed the constructed penis to be satisfactory in contour, allow free flow of urine, provide cutaneous sensory recovery and

Exstrophy-epispadias complex has been a difficult disease to treat. As in the minds of the public and most physicians alike, these children are crippled with life-long ailments and multiple operations. Much of the morbidity of this condition relates to failure to preserve urogenital tract function in these children. In recent years, through better understanding of the exstrophic anatomy and improved surgical techniques, complete repair of the exstrophic anomaly has become possible as soon as the child is born. Results of such closure indicate that such repair is not only feasible but mandatory

This article presents a comprehensive review of contralateral C7 (cC7) transfer surgery, tracing its evolution from treating brachial plexus injuries to its transfer to ipsilateral C7 transfer surgery in treating upper motor neurone injuries. The cC7 was initially postulated to restore function by replacing injured nerves at the peripheral level, but dynamic cortical reorganization has since been demonstrated post-surgery, which potentially allows harnessing of the cC7 procedure to expand the span of control of the uninjured hemisphere in conditions like hemiplegic stroke. By integrating princ
