Side effects and time course of response in a placebo-controlled trial of fluoxetine for the treatment of geriatric depression.

A high rate of improvement among patients who receive placebo in controlled trials of antidepressants can complicate the evaluation of true drug effect. Placebo response may be a reaction to the psychosocial factors of study participation or a function of changes in the natural course of depression. Drug side effects may also influence patients' expectations, and they should be distinguished from the somatic symptoms associated with major depression. The authors reanalyzed data from a large, multicenter, placebo-controlled clinical trial of fluoxetine treatment of geriatric depression to evalu

Two-year outcome in first-episode psychosis treated according to an integrated model. Is immediate neuroleptisation always needed?

In this multicentre study the two-year outcome of two groups of consecutive patients (total N = 106) with first-episode functional non-affective psychosis, both treated according to the 'need-specific Finnish model', which stresses teamwork, patient and family participation and basic psychotherapeutic attitudes, was compared. No alternative treatment facilities were available in the study sites. The two study groups differed in the use of neuroleptics: three of the sites (the experimental group) used a minimal neuroleptic regime whilst the other three (the control group) used neuroleptics acco

Patient presentation, interview content, and the detection of depression by primary care physicians.

This study was undertaken in order to better understand the detection of depression by primary care physicians. Specifically, we investigated the relationship between information gathered during the course of the medical interview and the subsequent diagnosis of depression. Forty-seven community-based primary care physicians, unaware of the mental health focus of this research, were videotaped in the office setting, as they interviewed two "typical" standardized patients who met DSM-III-R criteria for major depression. One patient presented with headaches and the other presented with palpitati

The salvage of open grade IIIB ankle and talus fractures.

Between 1983 and 1989, 11 open grade IIIB ankle or talus fractures were treated according to protocol including debridement, temporary placement of antibiotic beads, soft tissue coverage (including seven free vascular tissue transfers), intravenous antibiotics and fusion using an anterior plate, and bone graft. All patients had a minimum of three separate hospitalizations. Each had at least five operative procedures performed with an average of 8.2/patient (range: 5-12). The total in-patient hospital stay averaged 61.6 days (20-107 days) and in patient costs averaged $62,174.43/patient (range:

Rehabilitation of the adult with head trauma.

The changes incurred as a result of brain injury are often devastating. The approaches outlined in this article merely scratch the surface because the list of possible deficits is inexhaustible. The behavioral and cognitive capacity during the various phases of recovery dictate the intervention method that will be appropriate and successful, with concepts and tasks progressing from the simple to the more complex. When confusion, agitation, and inappropriate behavior are evident, safety is a prime concern. Some individuals do not progress beyond this stage. Placement and appropriate future plan

Total care for juvenile diabetics--a Swedish experiment.

A Swedish Study Group for Childhood Diabetes has recently prepared a national treatment and care programme for diabetes in children and adolescents, which is briefly presented in this paper. The programme is based on the working hypothesis that long-term vascular and neurological complications - still the most serious threats to the well-being and survival of juvenile diabetics - may be prevented by continuously maintaining a normal or near-normal blood glucose level. Four levels (I--IV) of metabolic control are being defined, of which level I is the optimal goal and level II acceptable, while

Functional and structural alterations in different durations of untreated illness in the frontal and parietal lobe in major depressive disorder.

Major depressive disorder (MDD) is one of the most disabling illnesses that profoundly restricts psychosocial functions and impairs quality of life. However, the treatment rate of MDD is surprisingly low because the availability and acceptability of appropriate treatments are limited. Therefore, identifying whether and how treatment delay affects the brain and the initial time point of the alterations is imperative, but these changes have not been thoroughly explored. We investigated the functional and structural alterations of MDD for different durations of untreated illness (DUI) using regio

Convergent Microglial and Astrocytic Responses across Diverse Stress Paradigms: Integrating Inflammation, Metabolism, and Behavior.

The central nervous system responds to diverse challenges through convergent glial mechanisms, yet most reviews examine single stressor paradigms in isolation. This review uniquely integrates current knowledge across five major stress categories (psychosocial, immune/inflammatory, infectious, injury/damage, and metabolic), revealing both shared pathways and stimulus-specific glial responses that have not been systematically compared. Because microglia and astrocytes are the principal immune-metabolic sensors mediating these responses, this review is centered on these two cell types, while ackn

Embodied robots in rehabilitation-oriented interventions for older adults with disability or cognitive impairment: a scoping review.

BACKGROUND: Robots are increasingly used in later-life care. However, existing reviews often combine applications such as companionship, telepresence, medication support, monitoring, psychosocial support, traditional AI, and rehabilitation within a single body of evidence. This conceptual overlap makes it difficult to interpret the rehabilitation-specific evidence base. OBJECTIVE: This study aimed to map empirical studies on embodied robots used in rehabilitation-oriented interventions for older adults with disability or cognitive impairment and to clarify the operational boundary between reh

Prevalence of work-related health conditions among hotel housekeepers: a systematic review and meta-analysis.

BACKGROUND: Hotel housekeepers are exposed to high physical workload, repetitive tasks, time pressure, and limited social recognition. These occupational conditions may increase the risk of work-related health problems, particularly musculoskeletal disorders, and negatively affect functional capacity and long-term work sustainability. The objective of this systematic review was to synthesise the available evidence on the most prevalent work-related health conditions among hotel housekeepers and to examine their association with work-related risk factors. METHODS: A systematic review was condu

Higher social support is associated with lower tinnitus-related disability among institutionalized older adults: a cross-sectional study.

BACKGROUND: Tinnitus-related disability among older adults in long-term care facilities may reflect not only auditory symptoms but also functional and psychosocial vulnerability. This study examined the association between perceived social support and Tinnitus Handicap Inventory (THI)-defined tinnitus-related disability among older long-term care residents with chronic subjective tinnitus. METHODS: We conducted a cross-sectional study between July and November 2025 among 390 residents aged 60 years or older with chronic subjective tinnitus in long-term care facilities in urban Wuhan, China. Participants' Perspectives on the iCareBreast Mobile-Based Perioperative Care Program for Women Undergoing Breast Cancer Surgery: Qualitative Process Evaluation.

BACKGROUND: Breast cancer treatment, particularly during the perioperative period, is often accompanied by significant psychological distress, including anxiety and uncertainty. Mobile health (mHealth) interventions have emerged as promising tools to provide timely psychosocial support through convenient, flexible, and personalized platforms. While research has explored the use of mHealth in breast cancer prevention, care management, and survivorship, few studies have examined patients' experiences with mobile interventions during the perioperative phase of breast cancer treatment. OBJECTIVE:

Multidimensional assessment of exertional dyspnea in young healthy males and females who select unsatisfied inspiration at peak exercise.

Historically, it was thought that healthy humans predominantly described their breathing as a sense of increased work or effort (W/E) during maximal exercise. However, emerging data show that many healthy adults select unpleasant dyspnea descriptors such as "unsatisfied inspiration" (UI), with relatively more females selecting UI than males. We hypothesized that males and females who select UI would report higher dyspnea intensity ratings during exercise, select more distressing dyspnea qualities post exercise, and have greater inspiratory constraints than those who do not. Sixty-four healthy,

The effects of a family-centered psychosocial-based nutrition intervention in patients with advanced cancer: the PiCNIC2 pilot randomised controlled trial.

BACKGROUND: Malnutrition in advanced cancer patients is common but limited and inconclusive data exists on the effectiveness of nutrition interventions. Feasibility and acceptability of a novel family-based nutritional psychosocial intervention were established recently. The aims of this present study were to assess the feasibility of undertaking a randomised controlled trial of the latter intervention, to pilot test outcome measures and to explore preliminary outcomes. METHODS: Pilot randomised controlled trial recruiting advanced cancer patients and family caregivers in Australia and Hong K

Exercise in children with joint hypermobility syndrome and knee pain: a randomised controlled trial comparing exercise into hypermobile versus neutral knee extension.

BACKGROUND: Knee pain in children with Joint Hypermobility Syndrome (JHS) is traditionally managed with exercise, however the supporting evidence for this is scarce. No trial has previously examined whether exercising to neutral or into the hypermobile range affects outcomes. This study aimed to (i) determine if a physiotherapist-prescribed exercise programme focused on knee joint strength and control is effective in reducing knee pain in children with JHS compared to no treatment, and (ii) whether the range in which these exercises are performed affects outcomes. METHODS: A prospective, para

[The relationship between depression, anxiety and heart disease - a psychosomatic challenge].

BACKGROUND: Depressive and cardiological disorders present a major comorbidity. Their manifold interrelations may be best analysed within a biopsychosocial model of disease. METHODS: A systematic research was done on empirical studies published during the last 15 years and dealing with epidemiological, etiopathogenetic and therapeutic dimensions of the comorbidity of depression, anxiety and heart disease. RESULTS: From an epidemiological perspective recurrent depressions are associated with a significantly increased risk of coronary heart disease. Depressive disorders play a major role in tr

Psychological adjustment and suicidal ideation in patients with AIDS.

This article assesses the psychosocial adjustment to illness and examines the relationship between adjustment and psychosocial and medical variables in 91 ambulatory HIV-infected patients. The 91 subjects were receiving ambulatory medical care in hospitals (Memorial Hospital, New York Hospital, and St. Vincent's Hospital) and in private medical consult (Gay Men's Health Crisis) in New York. The majority (74.5%) of subjects had AIDS. The sample was composed principally of white Roman Catholic homosexual men living alone. However, 49.5% were black or hispanic, 31.9% had intravenous drug use as t
