A dot enzyme immunoassay (EIA) using 50-kD outer-membrane proteins (OMPs) of Salmonella typhi was compared with the Widal test for the serodiagnosis of typhoid fever in 109 febrile children admitted to a hospital in an endemic area. In the culture-positive typhoid group, the initial dot EIA was positive in 40 of 42 cases and the initial Widal test was positive in 41. Valvar heart disease is a rare complication of juvenile rheumatoid arthritis (JRA), the aortic valve being most commonly affected. Reported cases with symptomatic mitral involvement are rare. An analysis of 108 cases of children's death which occurred in 1981-1991 from hypertoxic meningococcal infection (MI) in St. Petersburg is presented. Serogroup B meningococcus as a causative agent was found in 44% of the cases. The role in the thanatogenesis of inflammatory and immune reactions, viral-bacterial associations was shown on the basis of detailed study of 47 cases. Bone affectation in systemic lupus erythematosus (SLE) is caused both by the disease itself and by the treatment used. We report the case of a woman diagnosed of SLE, who, in the course of her illness, develops multiple aseptic osteonecrosis (AON) and laminar periostitis, radiologically compatible with the diagnosis of hypertrophic osteoarthropathy (HOA), with no evidence of acropaquia. In this case, the patient shows all the risk factors involved in the pathogenesis of the development of ischemia in bone microcirculation.

The direct wet mount examination of vaginal secretion, widely applied for the diagnosis of Trichomonas vaginalis infection in woman patients, is rapid and economical, however, the sensitivity of this technique is not so high. IgM antibodies to the phenolic glycolipid-I (PGL-I) antigen of Mycobacterium leprae were detected by different ELISA techniques in three laboratories (in New York, Colorado, Seattle, U.S.A.). Specimens from 490 patients with suspected legionellosis from many parts of New Zealand were studied. Most of these were sera, but 36 specimens including material from lungs, pleural fluid and sputa were also examined. The sera were tested for the presence of antibodies to Legionella pneumophila serogroups 1 to 6 and Legionella micdadei. Serological evidence of legionellosis was found in 49 patients. The damp environment and density of animal population in Camargue are good arguments for the study of leptospirosis. A serological and epidemiological survey on 642 blood samples revealed 36 (5.6%) with significant antibody titers. Icterohaemorrhagiae is followed by Canicola, Grippotyphosa, Australis and Ballum... The most endemic area is not Camargue but the area situated at the northeast (Basse-Durance). Women are twice more affected than men. These two results are not consistent with our initial hypothesis. It is always advisable to seek the etiology of fevers of unknown origin.

In a study of obscure fevers in Cairo, Egypt, 24 cases of hepatic amoebiasis were diagnosed during a 3-year period from 1977-1980. The counterimmunoelectrophoresis amoebic serologic test was positive in 20 of the 24 patients and was very useful in confirming the diagnosis.
