OBJECTIVE: Sacral insufficiency fracture after lumbosacral (LS) arthrodesis is an uncommon complication. The objective of this study was to report the authors' operative experience managing this complication, review pertinent literature, and propose a treatment algorithm.
METHODS: The authors analyzed consecutive adult patients treated at their institution from 2009 to 2018. Patients who underwent surgery for sacral insufficiency fractures after posterior instrumented LS arthrodesis were included. PubMed was queried to identify relevant articles detailing management of this complication.

BACKGROUND CONTEXT: In the lumbar spine osteoarthritis (LS-OA) population having surgery for lumbar spinal stenosis (LSS) symptoms, a significant proportion of patients experience limited benefit following the intervention. Thus, identifying contributing factors to this is important. Individuals with OA often have multiple joint symptoms, yet this has received limited attention in this population.
PURPOSE: Document the occurrence of joint symptoms among patients undergoing surgery for LS-OA, and investigate the influence of these symptoms on disability postsurgery.
DESIGN: Prospective study

BACKGROUND CONTEXT: Revision posterior decompression and fusion surgery for patients with symptomatic adjacent segment degeneration (ASD) is associated with significant morbidity and is technically challenging. The use of a stand-alone lateral lumbar interbody fusion (LLIF) in patients with symptomatic ASD may prevent many of the complications associated with revision posterior surgery.
PURPOSE: The objective of this study was to assess the clinical and radiographic outcomes of patients who underwent stand-alone LLIF for symptomatic ASD.
STUDY DESIGN: This is a retrospective case series.

BACKGROUND: While low back pain is the number one cause of disability in the United States, lumbar spinal stenosis along with intervertebral disc herniation and degenerative spondylolisthesis is one of the 3 most common diagnosis of low back and leg pain for which surgery is performed. Numerous modalities of treatments including drug therapy and complex surgical fusions have been recommended for treatment of central spinal stenosis. Epidural injections are one of the commonly performed nonsurgical interventions in managing central spinal stenosis; however, there has been paucity of literature

OBJECT: Cervical stenosis is a defining feature of cervical spondylotic myelopathy (CSM). Matsunaga et al. proposed that elements of stenosis are both static and dynamic, where the dynamic elements magnify the canal deformation of the static state. For the current study, the authors hypothesized that dynamic changes may be associated with myelopathy severity and neck disability. This goal of this study was to present novel methods of dynamic motion analysis in CSM.
METHODS: A post hoc analysis was performed of a prospective, multicenter database of patients with CSM from the AOSpine North Ame

PURPOSE: Lumbar fusion has been shown to be effective in treating a variety of degenerative spinal conditions, though significant differences exist in the magnitude of clinical improvement across different surgical diagnoses. With modern, minimally disruptive approaches for fusion, diagnosis-specific differences in clinical improvement may be reduced. The purpose of this study is to report and compare interim clinical improvements in patients treated with XLIF for various degenerative lumbar conditions.
METHODS: 160 patients underwent XLIF for either degenerative spondylolisthesis (n = 68), d

BACKGROUND CONTEXT: The correlation between radiographic solid fusion and favorable clinical outcome has not been fully established. Many surgeons believe that patients who achieve a radiographic solid fusion will exhibit a more positive clinical outcome than those getting an unsuccessful fusion. To our knowledge, there is no study that has evaluated whether a solid fusion influences clinical outcome after minimally invasive lumbar fusion.
PURPOSE: This study was designed to evaluate the effect of radiographic solid fusion on clinical outcome after minimally invasive transforaminal lumbar int

A minimum 2-year follow-up retrospective review was undertaken to assess our experience with an anterior paramedian muscle-sparing approach to the lumbar spine for anterior spinal fusion (ASF). The records of 28 patients (November 1991 through January 1996) undergoing ASF via a left lower quadrant transverse skin incision (6-10 cm) with a paramedian anterior rectus fascial Z-plasty retroperitoneal approach were reviewed. Diagnosis, number, and level of lumbar interspaces fused, types of fusion, estimated blood loss, length of procedure, length of hospital stay, and complications were analyzed.

Since the Food and Drug Administration approved the Babgy-Kuslich BAK cage for anterior lumbar interbody fusion (ALIF), various threaded interbody fusion devices have been introduced. Bone dowels offer several theoretical advantages over cages with regard to stress shielding; the purpose of this study was to assess the efficacy of bone dowels in interbody fusion. All patients undergoing ALIF performed by the senior author between 1998 and 2001 were retrospectively reviewed. Patients' preoperative and postoperative neurologic status, functional status, and both back and radicular pain were asse

This article hypothesizes that while the principles of evidence-based practice should serve as a guiding ideal for physicians, the manner in which this practice style is commonly implemented may be insufficient to the provision of truly informed decision-making and consent - especially in cases where outcomes are inextricably tied to technique, such as in the surgical subspecialities. Specifically, current methods of 'shared decision making' fail to adequately incorporate patient-specific expectations and physician-specific outcomes. A 'next-step', aimed at matching such expectations with outc

OBJECTIVES: To investigate the radiological outcome of the use of a new Cape Town-developed spinal fixation system.
DESIGN: One hundred and fifty-five patients underwent posterior lumbar spinal fusions with this fixation system and autogenous bone graft more than a year ago. Of these 121 were available for radiological follow-up.
SETTING: Spinal pedicle fixation systems are in common use in spinal fusion surgery. Most systems use rigid screws with a high rate of implant failure. In South Africa most spinal implants are imported and expensive, and this prompted the development of a locally ma
