A reconstructive technique for treatment of Achilles tendon rupture with an extensive defect is presented. The primary goal of this salvage procedure is to create functional length and anatomic continuity among muscle, tendon, and bone. The procedure uses a combination of fascial advancement techniques: the V-Y advancement and the gastrocnemius-soleus fascia turndown graft. In this technique, the inverted "V" inscribed on the proximal gastrocnemius fascia is partially elevated from the underlying soleus and twisted 180 degrees on itself to augment the repair. Advancing the musculotendinous jun

The sublabral foramen is considered to be a clinically asymptomatic, isolated variant of the anterior-superior capsulo-labral-complex. It may be observed during shoulder arthroscopy and may implicate problems in differential diagnosis of traumatic lesions. In an anatomic study on 89 macroscopically healthy shoulder specimens the age distribution and the incidence in relation to the varying shape of the glenoid as feasible factors of influence for a sublabral foramen (SF) were analyzed in order to elucidate the unknown pathogenesis of SF. In addition histologic sections of three SF samples were

INTRODUCTION: This is a report of 2 consecutive cases of the Galeazzi-equivalent fracture in children.
CLINICAL PICTURE: In both cases, the mechanism of injury was falls onto the outstretched hand. Radiographs showed fracture of the radius with separation of the distal ulnar physis.
TREATMENT: Initial treatment by closed manipulation failed; hence, open reduction and internal fixation was required. The cause of failure of closed manipulation in both cases was interposition of soft tissue into the ulnar fracture site.
OUTCOME: Both cases showed good functional outcome after open reduction an

This article presents a case of entrapment of the flexor hallucis longus tendon after open reduction and internal fixation of a Weber C ankle fracture resulting in interphalangeal joint contracture of the hallux. Pathology involving other tendons at the foot and ankle associated with ankle fractures is reviewed. Other scenarios of flexor hallucis longus pathology are discussed. Flexor hallucis longus anatomy, as related to distal fibular fractures, is outlined, and a recommendation is made to consider flexor hallucis longus entrapment as a cause of hallux dysfunction after open reduction and i

Soleus muscle injuries are frequently unrecognized, representing a common cause of sports inactivity. This is mainly because little is known about the anatomy of the soleus muscle and the clinical manifestations of injury. Unlike other muscles, the soleus muscle has a complex myoconnective structure with three intramuscular tendons, which makes the interpretation of muscle pathologic conditions challenging. Soleus muscle injuries can be acute or chronic and are usually considered to be a minor discomfort by both the patient and the sports medicine physician, leading to a relatively quick retur

Although rotator cuff repair is often successful at relieving pain, the repaired insertion site frequently fails. Mechanical properties of the repair improved when the shoulder was immobilized in an animal model, but joint stiffness and range of motion were not evaluated. The objective of this study was to measure rotational mechanics before and after shoulders were immobilized after cuff injury and repair, not immobilized after cuff injury and repair, and immobilized without injury and repair. Humeral rotation was significantly less 4 and 8 weeks after injury and repair but did not decrease s

Musculoskeletal pain occurs in 50% to 81% of people living with spinal cord injury. Pain can have a significantly limiting impact. Pain can lead to decreased mobility, endurance, social participation, and depression. Additionally, many people living with spinal cord injury (SCI) rely on their upper body for ADLs (activities of daily living) which may result in decreased independence in cases of severe pain. Musculoskeletal impairments associated with spinal cord injury include decreased muscle mass and bone density, spasticity, contractures, as well as overuse injuries in muscles, tendons, and

The aim of this study is present a case of isolated injury to the branch of posterior interosseous nerve. A 27-year-old gentleman was presented with an inability to extend the ulnar three fingers of his left hand after sustaining closed fractures of the left ulna and radius after a motorbicycle accident. The patient recovered partially over the course of 3 months but continues to be functionally compromised by the persistent radial deviation of his left wrist and incomplete mobility of the ulnar 3 fingers upon extension. On second exploration, we found the denervated muscles supplied by the re

Brachial plexus birth injury is an upper-extremity paralysis that occurs from a traction injury to the brachial plexus during birth. Approximately 10% to 30% of children with a brachial plexus birth injury have residual neurologic deficits with associated impact on upper-limb function. Management of brachial plexus birth injuries with a multidisciplinary team allows optimization of functional recovery while avoiding unnecessary intervention. Early occupational therapy should be initiated with a focus on range of motion and motor learning. The need for microsurgical reconstruction of the brachi

Patients with complex upper extremity injuries represent a unique subset of the trauma population. In addition to extensive soft tissue defects affecting the skin, bone, muscles and tendons, or the neurovasculature in various combinations, there is usually concomitant involvement of other body areas and organ systems with the potential for systemic compromise due to the underlying mechanism of injury and resultant sequelae. In turn, this has a direct impact on the definitive reconstructive plan. Accurate assessment and expedient treatment is thus necessary to achieve optimal surgical outcomes

Hamstring muscle injuries are common in athletes and mostly consist of sprains at the myotendinous junction, which often respond well to conservative treatment. Proximal hamstring avulsion injuries, though less common, can be severely debilitating. This injury is often seen in water skiers but has been described in many other sports and in middle-aged patients. Complete avulsions in young and active individuals do not respond well to conservative treatment and may require surgical repair. In contrast, many partial tears may be treated nonoperatively. However, when symptoms continue despite a t

Ulnar-sided wrist pain is one of the most common symptoms in athletes of baseball, racket sports, golf, and wrestling where there is frequent use of the hands as well as in soccer and running, where hand use is minimal. Compared with all wrist injuries, ulnar-sided wrist injury is a relatively serious condition for athletes because it plays an important role in performing a strong grip and in the rotation of the forearm. Ulnar-sided wrist pain in athletes can be related to acute trauma or chronic overuse. Acute trauma can lead to bone fractures and sprains/tears of ligaments. Repetitive mechan

BACKGROUND: Comprehensive studies evaluating quadriceps tendon (QT) autograft for anterior cruciate ligament (ACL) reconstruction are lacking. The optimal choice of graft between bone-patellar tendon-bone (BPTB), hamstring tendon (HT), and QT is still debatable.
HYPOTHESIS: The current literature supports the use of QT as a strong autograft with good outcomes when used in ACL reconstruction.
STUDY DESIGN: Meta-analysis; Level of evidence, 2.
METHODS: A systematic search of the literature was performed in PubMed, MEDLINE, Cochrane, and Ovid databases to identify published articles on clinica

BACKGROUND: It was the objective of this study to evaluate the contact area and pressure between a repaired rotator cuff tendon and bone for several different rotator cuff repair techniques, with and without a subacromial balloon spacer. It was hypothesized that the contact area and pressure at the rotator cuff footprint would increase with a subacromial balloon spacer following all repair techniques evaluated.
METHODS: Ten cadaveric shoulders were mounted onto a biomechanical shoulder simulator. A full-thickness 2-cm rotator cuff tendon tear was simulated, and repaired using single-row, doub

BACKGROUND: The double-row suture bridge (SB) is an effective technique for rotator cuff repair. However, because of high-tension sutures and tendon vascular insufficiency, nonunion or retearing of the rotator cuff after SB repair is common. The low-tension suture and microfracture features of the SCOI Row (SR) technique may help address these issues.
PURPOSE: To compare the biomechanical and histological outcomes of SR and SB techniques in treating rotator cuff tears in rabbits.
STUDY DESIGN: Controlled laboratory study.
METHODS: The study utilized a model of acute bilateral supraspinatus

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association2024eng
Subscapularis pathology is difficult to diagnose, in part because of decreased sensitivity and accuracy in identifying tears with magnetic resonance imaging (MRI) when compared to other cuff tendons. Artificial intelligence evaluation of patient physical examination and MRI data using a machine learning model shows that arthroscopically confirmed partial- or full-thickness subscapularis tears are highly associated with abnormal subscapularis tendon length, long head of the biceps tears, and subscapularis fatty atrophy, and on physical examination, with weakness with internal rotation and posit

BACKGROUND: While suture anchors are widely used in medical procedures for their advantages, they can sometimes lead to complications, including anchor prolapse. This article presents a unique case of suture anchor prolapse at the base of the distal phalanx of the little finger after extensor tendon rupture reconstruction surgery.
CASE PRESENTATION: A 35-year-old male, underwent extensor tendon rupture reconstruction using a non-absorbable suture anchor. After seven years the patient visited our outpatients complaining of stiffness, pain, and protrusion at the surgical site. Initial X-ray ima
