The authors describe the technique of anastomosis without vesicourethral suture after radical prostatectomy for cancer. The operative details are described with a brief analysis of the principal complications observed in the first 14 patients operated with preservation of the bladder neck. These results are compared with the in the literature concerning this old technique recently revised by the Mayo Clinic surgical team in 1988.

A new one-stage urethroplasty operation is presented. The author called it "double purpose penile skin flap technique". It is recommended for the substitution of distal urethral defect when its length is equal to or less than half the circumference of the penile shaft in cases following multiple failed hypospadias repairs.

Our previous work has shown that the CO2 laser can be successfully used in urethral reconstruction in a rat model. This new experiment investigates the use of the CO2 laser to perform a patch graft urethroplasty in the rabbit, as a preclinical model to its use in the repair of hypospadias in humans. The relationship between age and urodynamic parameters was studied cross-sectionally in a group of 28 women in whom clinically and urodynamically both stress incontinence and detrusor instability were excluded (no proven incontinence, NPI) and in a group, matched for age, with genuine stress incontinence (GSI). At all ages the maximal urethral closing pressure (MUCP) was higher in the NPI than in the GSI group; in both groups there was, however, a similar decrease in MUCP with age. Thirty-four women with genuine stress incontinence and low urethral closure pressure were studied pre- and postoperatively. A modified Burch retropubic urethropexy was performed in 29 patients with a cure rate of 78%, improvement in 7%, and failure in 15%. The modified Stamey procedure was performed in 5 patients with significant pelvic floor relaxation with a cure rate of 40% and improvement in 60%. If urethral mobility is present, traditional anti-incontinence procedures appear to be a reasonable alternative in patients with low urethral pressure.

To study the acute effect of norfenefrine, given orally in aqueous solution, on the urethral closure function, profilometry was performed before and after administration of placebo and increasing doses of norfenefrine in 6 females with genuine stress incontinence. A weak increase in urethral pressure was observed after administration of 90 mg of norfenefrine. No changes in heart rate and blood pressure occurred. The results indicate that the direct sympathomimetic effect of norfenefrine is weak. It is suggested that norfenefrine mainly acts via indirect mechanisms.

A new technique of onlay flap urethroplasty is described. The cosmetic and functional results of this logical extension of our one-stage urethroplasty with parameatal foreskin-flap have been good. The onlay urethroplasty with parameatal foreskin flap was recommended for the repair of distal hypospadias without chordee.

We report the technique and late results of a procedure for selected cases of distal hypospadias called sleeve advancement urethroplasty. This procedure incorporates correction of chordee, distal urethral mobilization and glans plasty in patients with coronal or distal subcoronal hypospadias. A bladder mucosal graft for urethral reconstruction was performed on 79 patients who had complicated conditions in which local penile or preputial skin was not available. The major and minor complication rates for this procedure were 15.2 and 43 per cent, respectively, the latter representing mucosal glanular protrusion obviated by a technical alteration. Given the difficulty of these cases these complication rates are deemed acceptable in such patients.

From July 1986 to Dec 1987, 78 women complaining of frequency, urgency, dysuria and suprapubic discomfort were evaluated at our clinic. Their ages ranged from 20 to 68 (mean age: 38.2). We used the Arap hypospadias repair in 31 patients with distal hypospadias with considerable success, achieving an excellent result in 93 per cent of our patients. Improved cosmesis, extension of the scope of the problems that can be addressed with this repair (including treatment of a distal urethrocutaneous fistula) and the ease with which the Arap procedure can be performed are the advantages that this operation has over other 1-stage distal hypospadias repairs.

A case of urethral diverticulum with calculus in a 53-year-old woman is reported. She was admitted to Isahaya General Hospital complaining of a localized swelling in the external genitalia. We found a urethral diverticular calculi by urethrography. Urethral diverticulectomy was carried out without any complications. The removed stones were in 2 pieces and they were composed of calcium phosphate and ammonium urate.
