BACKGROUND: Hemodialysis-associated hypersensitivity reactions are well documented in the literature. Ethylene oxide sensitization and activation of complement are important factors involved during such reactions. The majority of severe hypersensitivity reactions in dialysis patients, however, is due to sensitization to ethylene oxide.

We experienced a case of serious adverse reaction following the administration of Gd-DTPA (gadolinium with diethylenetriamine pentaacetic acid). The patient showed sneezing and hoarseness when Gd-DTPA was administered for the first time. At the second injection, when hydrocortisone was used in advance to prevent allergic reaction, she lapsed into a pre-shock state and anaphylactic reaction was strongly suggested by the laboratory data. The possibility that Gd-DTPA may cause the severe side effects must be noted.

The antianaphylactic activity of inhaled frusemide was studied in ovalbumin-sensitized guinea-pigs. The exposure of the animals to frusemide aerosol (1% solution for 20 min) attenuated the respiratory response to ovalbumin challenge (aerosol 1% solution) and was associated with a significant reduction of blood histamine (70%; P less than 0.01) and thromboxane-B2 (35%; P less than 0.01) compared to control animals. A case of severe anaphylactoid reaction following injection of intravenous contrast is described, and a discussion of the treatment of severe allergic reactions follows. The patient's presentation was unusual in that she rapidly developed massive non-cardiac pulmonary edema as the primary symptom, along with hypotension and hypokalemia, a syndrome similar to adult respiratory distress syndrome. A protocol for large-scale screening of blood donors for IgA deficiency using a tanned cell passive haemagglutination-inhibition (PHI) technique and enzyme-linked immunosorbent assay (ELISA) is described. Selective deficiency of serum IgA is the most common immunodeficiency in humans; when immunodeficient individuals receive blood transfusions a severe anaphylactoid reaction can develop. The present report describes such a patient.
