Differential cardiovascular and autonomic responses to structurally distinct intermittent hypoxia paradigms in rats.

Intermittent hypoxia (IH), the key physiological stressor in obstructive sleep apnea, is commonly quantified by respiratory event frequency. However, clinical heterogeneity in hypertension among patients with comparable apnea-hypopnea index (AHI) suggests that episode timing, including the duration and frequency of desaturation-reoxygenation cycles, may exert distinct biological effects even under equal cumulative burden. To test this, male Wistar-Kyoto rats were exposed for 21 days (8 h/day) to IH with either 10-s hypoxia duration at 30 cycles/h (10s-30c) or 5-s hypoxia duration at 60 cycles/

Evaluation of heart rate variability, QT dispersion, and Tp-e interval in pediatric subclinical hypothyroidism.

BACKGROUND: Clinical effects of subclinical hypothyroidism are not clearly understood. This study aims to investigate the effects of subclinical hypothyroidism (SH) on cardiac autonomic and conduction systems in children. METHODS: Forty-seven cases (25 female, 22 male) with SH aged between 3 and 17 years and 46 controls that were age, body mass index and sex matched, were included in the study. Heart rate variability (HRV) was used to evaluate cardiac autonomic function while QT dispersion, P dispersion and Tp-e measurements from ECGs to evaluate susceptibility to arrhythmia. RESULTS: Standa

Sphincter EMG as a diagnostic tool in autonomic disorders.

Multiple system atrophy (MSA) is a neurodegenerative disease presenting with a combination of parkinsonian, cerebellar, and autonomic (including cardiovascular, urinary, and anorectal) dysfunction. It is pathologically defined, but at present lacks a definitive clinical diagnostic test. The majority of patients with probable MSA have an abnormal sphincter EMG. Patients with idiopathic Parkinson's disease do not show marked sphincter EMG abnormalities. Therefore, these abnormalities can be used to distinguish MSA from idiopathic Parkinson's disease in the first 5 years after disease onset. In c

The Lewis family revisited: no evidence for autosomal dominant multiple system atrophy.

In 1964, Lewis reported a familial ataxia-dysautonomia syndrome reminiscent of Shy-Drager syndrome subsequently known as multiple system atrophy (MSA). Here we review this report and propose that the Lewis family may represent an unusual form of autosomal dominant cerebellar ataxia type I, which might be categorized either as SCA3 (Machado-Joseph disease) or a new SCA subtype. There remains no conclusive evidence to support the notion of hereditary MSA.

Electrocardiographic changes during postnatal development in conscious swine with cardiac autonomic imbalance.

Using a conscious swine model, we studied the effects of different patterns of cardiac autonomic denervation on alterations of R-R and Q T intervals for 8 postnatal weeks. Newborn pigs were assigned randomly to four different groups: sham-operated controls (C), stellate ganglion ablation (SGX), either left (LSGX) or right (RSGX), and the right cardiac vagus nerve (RCVX) transection. The ECGs were recorded by telemetry while animals rested quietly or were judged behaviorally to be asleep. Analyses of the ECG included measurements of R-R and Q-T intervals, as well as corrected Q-T intervals (QTc

Power spectral analysis of heart rate in hyperthyroidism.

The aim of the present study was to evaluate the impact of hyperthyroidism on the cardiovascular system by separately analyzing the sympathetic and parasympathetic influences on heart rate. Heart rate variability was evaluated by autoregressive power spectral analysis. This method allows a reliable quantification of the low frequency (LF) and high frequency (HF) components of the heart rate power spectral density; these are considered to be under mainly sympathetic and pure parasympathetic control, respectively. In 10 newly diagnosed untreated hyperthyroid patients with Graves' disease, we ana

Intrinsic electrophysiologic properties of reentrant supraventricular tachycardia involving bypass tracts.

This study evaluates the effects of autonomic blockade (propranolol, 0.2 mg/kg, and atropine, 0.04 mg/kg) in 20 patients with paroxysmal supraventricular tachycardia (SVT). In 8 patients the SVT circuit involved a concealed atrioventricular bypass for retrograde conduction (group I) and in 12 a concealed atrio-His pathway (group II). Autonomic blockade did not significantly change atrial and ventricular refractory periods, whereas it prolonged atrioventricular nodal refractoriness without varying AH interval. The ventriculoatrial interval did not change in any patient. The H2A2 interval was un

Bedside diagnosis of cardiac autonomic damage by computerized analysis of heart rate-respiration relationship.

In this study we propose a method for the analysis of the relationship between heart rate changes and respiration as a possible diagnostic tool for cardiac autonomic damage. The method consists in recording R-R intervals and respiratory amplitude by a suitably equipped personal computer, and by evaluating the cross-correlation peak between the two signals. This mathematical function appeared to be more sensitive to the degree of concordance between the two signals, rather than their absolute amplitude. The cross-correlation appeared to be lower in diabetics with autonomic dysfunction, markedly

An "autonomic" classification of antiarrhythmic drugs.

An alternative classification schema for antiarrhythmic drugs is proposed based primarily on the autonomic "side effects" of these drugs in addition to their electrophysiologic actions. In this new schema, Class I (local anesthetics) is subdivided into 1A (quinidine-like agents with cholinergic blocking actions) and 1B (agents such as lidocaine without autonomic activity). Class II comprises the digitalis glycosides which have vagotonic effects. Class III contains drugs with antiadrenergic activity, subdivided into IIIA (beta-blockers such as propranolol), IIIB (norepinephrine-release inhibito

Anatomy and physiology of genital organs - men.

Male sexual functions involve a number of organs and structures in genitalia whose role is to produce fertilizing gametes and to allow female-partner insemination. The testes belong to the reproductive and endocrine systems as they synthesize spermatozoa and androgens, and are under finely regulated hormonal control by the hypothalamopituitary axis. Sexual responses are controlled by a complex and coordinated interplay of both the somatic and the autonomic nervous system in multiple components of the brain, spinal cord, and relevant peripheral organs. Erectile bodies are an essential element o…

Piezo2 expressed in proprioceptive neurons is essential for skeletal integrity.

In humans, mutations in the PIEZO2 gene, which encodes for a mechanosensitive ion channel, were found to result in skeletal abnormalities including scoliosis and hip dysplasia. Here, we show in mice that loss of Piezo2 expression in the proprioceptive system recapitulates several human skeletal abnormalities. While loss of Piezo2 in chondrogenic or osteogenic lineages does not lead to human-like skeletal abnormalities, its loss in proprioceptive neurons leads to spine malalignment and hip dysplasia. To validate the non-autonomous role of proprioception in hip joint morphogenesis, we studied th

Beta-endorphin, ACTH, and catecholamine responses in chronic autonomic failure.

Plasma epinephrine (EPI), norepinephrine (NE), beta-endorphin, and corticotropin (ACTH) responses were measured during insulin-induced hypoglycemia in normal subjects and in patients with either multiple system atrophy (MSA) or idiopathic orthostatic hypotension (IOH). In normal subjects, there was a striking rise in EPI, NE, beta-endorphin, and ACTH following the nadir of hypoglycemia. Both beta-endorphin and ACTH responses were significantly lower than normal in patients with MSA, in contrast to normal levels in IOH patients. No correlation was observed between the degree of adrenergic insuf

Denzimol, a new anticonvulsant drug. II. General pharmacological activities.

The paper reports on the pharmacological properties of N-[beta-[4-(beta-phenylethyl)phenyl]-beta-hydroxyethyl]imidazole hydrochloride (denzimol, Rec 15-1533), a compound endowed with anticonvulsant properties, and its possible side effects. Effects on the CNS, effects on vigilance and general motility are similar to those of phenytoin and occur at doses much above anticonvulsant levels. The drug has good in vitro antihistamine, anticholinergic and anti-5-HT activity which accounts for the effects on the gastrointestinal system e.g. inhibition of gastric secretion and motility and anti-ulcer pr

Cystic Fibrosis and the Nervous System.

Cystic fibrosis (CF) is a life-shortening autosomal recessive disorder caused by mutations in the gene encoding the cystic fibrosis transmembrane conductance regulator (CFTR). CFTR is an anion channel that conducts bicarbonate and chloride across cell membranes. Although defective anion transport across epithelial cells is accepted as the basic defect in CF, many of the features observed in people with CF and organs affected by CF are modulated by the nervous system. This is of interest because CFTR expression has been reported in both the peripheral and central nervous systems, and it is well

Vagus nerve stimulation-induced bradyarrhythmias in rats.

The autonomic consequences of seizures can be severe. Death can follow from autonomic overactivity that causes a parasympathetically mediated bradyarrhythmia. We studied the cardiovascular consequences of unilateral and bilateral stimulation of the distal segments of transected vagus nerve in rats anesthetized with urethane. The range of stimulation rates tested is comparable to the firing rates observed in vagus nerve during seizures. There was a consistent inverse relation between stimulus rate and heart rate with nodal block appearing at 5-10 Hz and minimum HR levels (cardiac standstill) oc

Differentiation in the angiotensin II receptor 1 blocker class on autonomic function.

Autonomic function is disordered in cardiovascular disease states such as chronic heart failure (CHF) and hypertension. Interactions between the renin-angiotensin-aldosterone system (RAAS) and the sympathetic nervous system (SNS) may potentially occur at a number of sites. These include central sites (eg, rostral ventrolateral medulla), at the level of baroreflex control, and at the sympathetic prejunctional angiotensin II receptor 1 (AT(1)) receptor, which is facilitatory for norepinephrine release from the sympathetic nerve terminal. Therefore, drugs that block the RAAS may be expected to im

The influence of the psyche and the brain on immunity and disease susceptibility: a critical review.

In critically reviewing the sources of evidence connecting psyche and brain with the immune system, the authors include a brief review of current knowledge of the immune system, its interactions with the neuroendocrine system, and other factors influencing its regulation. These include developmental stages, aging, rhythmicity, and a variety of exogenous influences. The need for developing further information about normal base lines is emphasized. Against that background, many sources of data demonstrating connections between the central nervous system and the immune system are presented: indir
