Necator americanus infection is now rare in developed countries but remains an important cause of iron-deficiency anemia and abdominal symptoms in individuals with relevant epidemiological backgrounds. A 59-year-old Filipino man with long-standing unexplained iron-deficiency anemia was admitted to our hospital for epigastric pain associated with choledocholithiasis. Although biliary enzymes improved after endoscopic treatment, his abdominal pain persisted. Subsequent gastrointestinal investigations, including upper gastrointestinal endoscopy, colonoscopy, and small-bowel capsule endoscopy, rev

BACKGROUND: Opium and its derivatives are widely abused throughout the world. Recent case reports and a few limited studies have suggested that opiates cause dilation of the common bile duct of the abusers.
MATERIAL/METHODS: Our case-control study, lasting 7.5 months, investigated 121 male adult addicts and 142 non-addicted controls for biliary tract diameters, using ultrasonography. The study was conducted in Bahonar Hospital in Kerman. Neither the addiction cases nor the non-addict controls revealed any hepatobiliary tract symptoms. The subjects were asked to fill in a questionnaire. After …

Transdiaphragmatic rupture of hepatic hydatid cysts into the bronchial tree is a rare but severe complication that may mimic pulmonary infection. We report the case of a 61-year-old man who presented with fever, dyspnea, and right thoracic pain, with initial CT findings suggestive of pneumonia and pleural effusion. Despite antibiotic therapy and surgical drainage, clinical deterioration persisted. Bronchoscopy revealed biliary secretions, and thoracoabdominal CT demonstrated a calcified hepatic hydatid cyst communicating with the right lower lobe bronchial tree through a diaphragmatic defect,

Liver retraction-related injury is a rare complication of gastrointestinal surgery, usually clinically silent and not life-threatening. We present an 86-year-old male patient admitted to our hospital for a laparoscopic cholecystectomy that ended in a 300-minute laparotomy after surgical complications. Twenty-four hours after the surgery an MRI was performed due to suspected biliary tract injury. MRI revealed bile leak and a new liver lesion not present in preoperative MRI, it was initially interpreted as edema. Clinical course and follow up MRI eventually suggested de diagnosis of retractor-re…

INTRODUCTION: Ascaris lumbricoides is a common helminthic infection characterized by fecal-oral route of transmission. Commonly, it affects the gastrointestinal tract. However, in significantly rare cases, it can affect unexpected body regions, such as biliary tree, pancreas, and the lung.
CASE PRESENTATION: A 68-year-old female patient underwent PleurX tube insertion due to malignant pleural effusion. On the third week of insertion, she complained of itching and pain at the site of insertion, a grayish-white worm was visualized at the insertion site. Ascaris lumbricoides was identified by mi

Common bile duct duplications represent exceptionally rare congenital anomalies of the biliary tract. In this case report we document an unusual variant of common bile duct duplication in a 79-year-old man who underwent a pancreaticoduodenectomy for ampullary cancer. The duplication consisted of two unseparated, completely-layered, common bile ducts which originated above the cystic duct junction and terminated prior to the point of insertion into the pancreas, where the two lumens converged into a single duct. Duplication of the bile duct is rare and often goes undetected. In the present case…

The biliary tract's most frequent malignancy is gallbladder cancer (GBC). Due to the growing number of laparoscopic cholecystectomy procedures, the majority of GBC cases in the United States are diagnosed as incidental findings. Roughly 5% of the reported cases of gallbladder adenocarcinomas are of the mucinous type. Mucinous adenocarcinoma tumors contain more than 50% of mucin materials and are clinically characterized by poor prognosis. Initial presentation can mimic acute cholecystitis or cholelithiasis; meanwhile, histological findings post-cholecystectomy are the main diagnostic criteria …

Early-stage gallbladder cancer (GBC) is mostly discovered incidentally by the pathologist after cholecystectomy for a presumed benign disease. It is the most common malignancy of the biliary tract with a variable incidence rate all over the World. The majority of patients with GBC remain asymptomatic for a long time and diagnosis is usually late when the disease is at an advanced stage. Radical surgery consisting in resection of the gallbladder liver bed and regional lymph nodes seems to be the best treatment option for incidental GBC. However, recurrence rates after salvage surgery are still …

Gallstone ileus is an important form of small bowel obstruction that occurs in less than 0.5% of patients who present with obstruction. A biliary enteric fistula that evolves in the setting of chronic cholecystitis may allow the passage of a large gallstone into the gastrointestinal tract distal to the common duct. A single stone that is sufficient in size (at least 2-2.5 cm diameter) may then create a mechanical obstruction, most often at the ileocecal valve or the terminal ileum where the intestinal lumen narrows, and where peristalsis is less robust. We present an unusual case of gallstone

Primary sclerosing cholangitis (PSC) is a rare but prominent fibroinflammatory cholangiopathy which can affect individuals of essentially any age. It carries a median survival of 15-20 years, regardless of age at diagnosis, and is a foremost risk factor for cholangiocarcinoma. Given the chronic and progressive nature of PSC, its inherent risk for biliary tract and other complications, and the paucity of effective pharmacotherapies, endoscopy plays a major role in the care of many patients with this disorder. In this review, we discuss the endoscopic management of PSC, including established and…

Introduction. Pancreatic pseudocysts are a common complication of acute pancreatitis. Pancreatic pseudocyst's natural history ranges between its spontaneous regression and the settlement of serious complications if untreated, such as splenic complications, hemorrhage, infection, biliary complications, portal hypertension, and rupture. The rupture of a pancreatic pseudocyst to the peritoneal cavity is a dangerous complication leading to severe peritonitis and septic conditions. It requires emergent surgical exploration that is often of great technical difficulty and with important morbidity and

The etiology of the autoimmune liver disease primary biliary cirrhosis (PBC) remains largely unresolved, owing in large part to the complexity of interaction between environmental and genetic contributors underlying disease development. Observations of disease clustering, differences in geographical prevalence, and seasonality of diagnosis rates suggest the environmental component to PBC is strong, and epidemiological studies have consistently found cigarette smoking and history of urinary tract infection to be associated with PBC. Current evidence implicates molecular mimicry as a primary mec

The management of autoimmune liver disease can be very challenging. The presentation and natural history of these disorders is highly variable and can be asymptomatic, acute or chronic. Diagnosis requires the skilled interpretation of serological markers and histological changes and sometimes of biliary tract imaging. The major treatment options are immunosuppressive therapy (steroids and azathioprine) in autoimmune hepatitis (AIH) and ursodeoxycholic acid in cholestatic conditions although other treatments continue to be developed and tested. The complications of these diseases, in particular…

The mechanisms operating in lymphocyte recruitment and homing to liver are reviewed. A literature review was performed on primary biliary cirrhosis (PBC), progressive sclerosing cholangitis (PSC), and homing mechanisms; a total of 130 papers were selected for discussion. Available data suggest that in addition to a specific role for CCL25 in PSC, the CC chemokines CCL21 and CCL28 and the CXC chemokines CXCL9 and CXCL10 are involved in the recruitment of T lymphocytes into the portal tract in PBC and PSC. Once entering the liver, lymphocytes localize to bile duct and retain by the combinatorial

A multicenter, open labelled, randomized study was carried out to compare the prophylactic efficacy of Cefepime and Ceftriaxone in patients undergoing biliary tract surgery. Two hundred and nine patients were included in the study and randomized to receive preoperative infusion of 2 g Cefepime (n=107) or 2 g Ceftriaxone (n=102) both in a single i.v. administration. Antimicrobial prophylaxis was successful in preventing infections in 98.9% of patients in the Cefepime group and 97.7% in the Ceftriaxone group (p=0.3871). Both regimens were well tolerated without any adverse drug-related reactions…

Extrahepatic complications from hepatic cystic echinococcosis are rare and may be life threatening. Although the prevalence of echinococcosis in Spain had decreased, the number of cases of this disease and its severity has risen again due to immigration. We report the case of a patient with a giant hepatic echinococcus cyst diagnosed during investigation of an abdominal mass. The mass was associated with three other cysts: the first cyst was fistulized to the biliary tract, without clinical or laboratory alterations, the second cyst was fistulized to the right pleural cavity, leading to pleura…

A 50-year-old man with type B liver cirrohosis was admitted with jaundice and ascites. He had undergone an operation for esophageal variceal rupture at another hospital. Abdominal CT scan and ultrasonography showed cystic dilatation of the extrahepatic portal vein, and Doppler ultrasonography showed it to be a portal vein aneurysm. After admission, jaundice progressed, and not only liver failure but also biliary tract obstruction was suspected as its etiology. Therefore, endoscopic retrograde cholangiography was performed, and it showed a remarkably bent extrahepatic bile duct thought to be ca…

BACKGROUND: Choledochal cyst is an uncommon anomaly of the biliary tract. Experience with this disorder, with emphasis on the role of ERCP in management, is presented.
METHODS: All pertinent data for 13 patients with choledochal cyst seen between 1987 and 1998 were reviewed, retrospectively.
OBSERVATIONS: Cholangitis, either alone or associated with other findings, was the main presenting feature (69.23% of patients). Bilobar Caroli's disease was the most common cyst type (46.15% of patients). ERCP was diagnostic alone in 46.15% of patients and therapeutic in 53.85%.
CONCLUSIONS: Choledocha…
