Clinical and anatomical aspects of possible mylohyoid nerve injury during genioplasties.

Damage to the cutaneous branch of the mylohyoid nerve (CBMN) during genioplasties is possible but rare. The aim of this cadaveric study was to demonstrate possible mechanisms of injuring the CBMN during genioplasties. Mandibulotomy access was used to carry out sublingual and lateral cervical dissections on 10 formalinized cadavers with a mean age of 65 years. The length of the mylohyoid nerve was measured at three sections on the mandible. The posterior section of the mylohyoid nerve was 8.7+/-0.5 mm; the middle section was 14.5+/-0.9 mm and the anterior section was 15.6+/-1.2 mm in length. Th…

Anatomical bases of left recurrent nerve lesions during mediastinoscopy.

PURPOSE: The mechanism of left recurrent laryngeal nerve palsy after cervical mediastinoscopy remains unclear. Our aim is to describe the anatomical bases of this complication. METHODS: Video-assisted mediastinoscopy was performed on ten fresh human cadavers with simultaneous dissection of the left recurrent nerve. RESULTS: Three dissection areas could be described regarding the risk of left recurrent nerve damage: a low risk area, along the right wall and before the upper part of the anterior wall of the trachea; an area at high risk of indirect stretch-induced lesions, between the lower pa…

[Surgical-anatomical study as a part of operative treatment of primary hyperparathyroidism].

Surgical- anatomical studies improve experience of a surgeon and technique of the parathyroids preparation. The authors conducted a study aimed at standardization of the parathyroidectomy procedure and collection of the parathyroids from cadaverous donors for allotransplantation. The total of 101 cadavers (51 females, 50 males) were sectioned. Parathyroid regions were closely preparated, the glands were collected and histologically verified. Anatomical differences of this region were recorded. The parathyroid tissue was confirmed microscopically in 75%. Mostly, the parathyroids were mistaken f…

Exposure of the V1-V3 segments of the vertebral artery via the posterior cervical triangle: a cadaveric feasibility study.

OBJECT: Surgical exposure of the extracranial part of the vertebral artery (VA) is occasionally necessary. Historically, the greater portion of the extracranial portion of the VA has been approached by traversing the anterior cervical triangle. The authors speculated that this entire segment of the VA could be reached with equal efficacy via the posterior cervical triangle (PCT). METHODS: Six adult cadavers underwent dissection of the left and right VAs via the PCT. The entire extracranial VA was easily exposed through this approach. Only three of 12 sides required the transection of the clav…

Effects of X-STOP device on sagittal lumbar spine kinematics in spinal stenosis.

The X-Stop device is designed to distract the posterior elements of the stenotic segment and place it in flexion to treat neurogenic claudication. Previous biomechanical studies on X Stop have been done in vitro on cadavers looking at disc pressures and segmental range of movements. The objective of this study is to understand the sagittal kinematics in vivo of the lumbar spine at the instrumented and adjacent levels. Twenty-six patients with lumbar spine stenosis underwent 1 or 2 level X-Stop procedure. All had pre- and postoperative positional magnetic resonance imaging (MRI) in standing, su…

Possible anatomic structures causing entrapment neuropathies of the median nerve: an anatomic study.

This study aimed at identifying the anatomic structures which may be responsible for entrapment neuropathies of the median nerve. Thirty upper extremities of 15 formalin fixed adult cadavers were dissected from the axilla to the distal forearm under Zeiss Opmi 9--FC microscope. We encountered seven different anatomic structures that may compress the median nerve. These structures are the brachialis muscle, Struther's ligament, the bicipital aponeurosis, pronator teres, flexor digitorum superficialis, the accessory head of plexor pollicis longus (Gantzer's muscle) and vascular structures. The s…

Lack of the complete circular rhabdosphincter and a distinct circular smooth muscle layer around the proximal urethra in elderly Japanese women: an anatomical study.

Histological observations of semiserial sections obtained from the cadavers of 30 elderly Japanese women (aged 61-93 years) found that the urethral rhabdosphincter (URS) usually occurred as a ventrally localized structure instead of exhibiting the normal completely circular configuration. The superoinferior length, thickness and muscle fiber density of the URS area showed significant interindividual variation. A thick fascicle, extending posterolaterally from the URS and radiating to the lateral vaginal wall and other perineal tissues, was usually observed. A circular smooth muscle layer, whic…

The dorsal scapular island flap: an alternative for head, neck, and chest reconstruction.

The back has become an increasingly popular donor site for flaps because it can provide thin, pliable tissue, with minimal bulk, and the scar can be easily hidden under clothing. The authors performed a cadaveric and clinical study to evaluate the anatomy of the dorsal scapular vessels and their vascular contribution to the skin, fascia, and muscles of the back. On the basis of anatomical studies in 28 cadavers and clinical experience with 32 cases, it was concluded that the dorsal scapular vessels provide a reliable blood supply to the skin of the medial back, making it a versatile flap to us…

A morphological study of the thoracic duct at the jugulo-subclavian junction.

Chylous fistulae are uncommon but serious complications of neck surgery, occurring with an incidence of 1-3% after radical neck dissection. The majority occur on the left side (75-92%) and are due to damage to the terminal segment of the thoracic duct as it drains into the great veins of the neck in the region of the venous angle. The risk of trauma to the terminal thoracic duct may be influenced by anatomical variations. The macroscopic arrangement of the termination of the thoracic duct in the left neck was examined in 24 UK cadavers. Twenty-one ducts terminated as a single vessel, two ducts…

[Plantar dislocation of the tarso-metatarsal articulation (Lisfranc articulation). Apropos of a case].

Plantar fracture-dislocation of the tarso-metatarsal joint (Lisfranc plantar fracture-dislocation) is rather unusual. The authors described a case sustained in a 39 years old man injured in a motorcycle road traffic accident. Mechanism was recognized as plantar hyperflexion combined with an axial foot compression. Open reduction, and K-wires fixation were performed. An anatomical feature was restored. In an additional anatomical study on 8 cadavers, the plantar direction of a fracture-dislocation of the tarso-metatarsal joint was observed when the injury axis followed the scapho-cuneiform join…

Touch allodynia following endoscopic (single portal) or open decompression for carpal tunnel syndrome.

We investigated if single-portal endoscopic carpal tunnel decompression equipment (Agee, 3M, USA) would cause increased carpal tunnel pressure during the release and if endoscopic release would reduce postoperative touch allodynia. Measurements on cadavers of the pressure produced during endoscopic release showed similar pressures to those produced during maximal range of motion. One hundred patients underwent either open or endoscopic decompressions. Twenty normal individuals served as controls. At 1 month after surgery both groups had significant allodynia compared with the controls, but at …

Relief of intractable perineal pain by coccygeal nerve block in anterior sacrococcygeal ligament after surgery for rectal cancer.

Intractable perineal pain often appears in the anal region following abdominoperineal resection for the treatment of rectal cancer. In such cases, a subarachnoid block or transsacral block is generally used to control pain. However, these procedures sometimes cause complications such as dysuria or sensory paralysis of the pudendum. A new method of pain control is presented herein using absolute alcohol as a neurolytic agent to relieve localized perineal pain through a coccygeal nerve block in the anterior sacrococcygeal ligament. Five cadavers were necropsied to localize the coccygeal nerve. A…

[Topology of the carpal tunnel in dynamic stress of the wrist joint].

Alterations of the spatial geometry of the carpal tunnel and its contents during dynamic stress situations of the wrist are of importance in clarifying morphological causes of a carpal tunnel syndrome. In order to demonstrate the changes occurring in the form of the carpal tunnel and the location of its contents during wrist movement, casts of the carpal tunnel were produced from the hands of cadavers in flexed and extended positions. The casts were then morphometrically analyzed and the movements of the median nerve and the flexor tendons were examined radiologically. The investigation showed…

[Experimental laser surgery of the ossified cochlea].

A pulsed holmium: YAG-laser (lambda = 2120 nm) was used to reopen the basal turn of artificially obliterated human cochleas in freshly dissected cadavers for intracochlear insertion of the stimulation electrode of a cochlear implant under simulated surgical conditions. Laser energy was transmitted through a 400 microns nylon fibre via the opened facial recess directly to the round window niche. At an energy level of 500 mJ/2.5 microseconds pulse, a repetition rate of 2 Hz, and an exposure time of 20-30 s, the photo-ablative mechanism of laser-bone interaction, which has only a limited thermal …

[Morphological investigation of external sphincter muscle of the male posterior urethra].

For investigation of function of the male posterior urethra, morphological study was performed in terms of the muscle architecture including sphincter muscle of the posterior urethra. The muscle localization was demonstrated by microdissection in eight male cadavers. Histological examination was also employed with HE staining and immunohistochemical method to detect the nature of these muscles. At the dorsal side, the muscle fibers of the deep trigone were located from the inter-ureteral ridge beyond the bladder neck to the vermontanum surrounding the posterior urethra. They were regarded as t…

The anatomic basis and clinical applications of flaps based on the posterior tibial vessels.

The vascular anatomy of the posterior tibial vessels has been studied in 20 legs of 10 cadavers. The number, size and distribution of the direct cutaneous and direct muscle branches of the posterior tibial vessels were recorded. For analysis, the leg was divided into four equal segments, Zone I being the most distal and Zone IV the most proximal. The direct cutaneous branches were found to cluster mainly around Zone II. The direct muscle branches to the soleus and flexor digitorum longus muscles arose chiefly in Zones II and III. Based on this knowledge, we performed fasciocutaneous, island so…

[Anatomic bases of the surgical approach to the carpal tunnel. A propos of 50 dissections].

After a classic dissection of 50 wrists in black african cadavers, we have studied carpal tunnel anatomy for therapeutic deductions. By a wide anterior incision following the longitudinal axis of the third finger remounting the inferior third of the forearm we studied both the anatomical limits specially the transcarpal ligament, the volare carpal ligament and also median nerve and flexores digitorum tendons. We have observed in 2 cases an ectopic second lumbrical muscle inside the carpal tunnel; we found numerous variations of the collateral and terminal branches of median nerve; we concluded…
