Laserinterferometric assessment of pilocarpine-induced movement of an accommodating intraocular lens: a randomized trial.

PURPOSE: To measure the axial movement of an accommodating intraocular lens (IOL) induced by ciliary muscle contraction after application of pilocarpine. DESIGN: Randomized, controlled, patient- and examiner-masked trial with intrapatient comparison. PARTICIPANTS AND CONTROLS: One hundred ten eyes of 55 patients with age-related bilateral cataract. METHODS: This study was divided into 3 parts. In the first, the accommodating IOL (1CU) was compared with a 3-piece open-loop acrylic IOL that served as the control. In the second, to assess the effect of capsule fibrosis on the potential accommo

The Neisseria meningitidis serogroup A capsular polysaccharide O-3 and O-4 acetyltransferase.

Neisseria meningitidis serogroup A capsular polysaccharide (CPS) is composed of a homopolymer of O-acetylated, alpha1-->6-linked ManNAc 1-phosphate that is distinct from the capsule structures of the other meningococcal disease-causing serogroups, B, C, Y, and W-135. The serogroup A capsule biosynthetic genetic cassette consists of four open reading frames, mynA-D (sacA-D), that are specific to serogroup A, but the functions of these genes have not been well characterized. mynC was found to encode an inner membrane-associated acetyltransferase that is responsible for the O-acetylation of the C

Molecular analysis of two group B streptococcal virulence factors.

Molecular biology has provided new technology for evaluating the traits of bacterial pathogens that are important in the pathogenesis of infections. The ability to derive isogenic strains that differ by a single trait provides a powerful tool for investigating the interaction of a putative virulence factor with the host at any of the various steps in pathogenesis. Recombinant DNA techniques afford the opportunity to clone the genes involved in the biosynthesis of a particular virulence factor. Once the gene(s) are cloned, a vast amount of information can be learned about their composition, str

Extracapsular cataract extraction and pseudophakos implantation in primates: a clinico-pathologic study.

Eighteen rhesus monkeys underwent lens implantation with Choyce Mark VIII, Binkhorst iridocapsular, or Shearing posterior chamber lenses. They were sacrificed 4-28 months postoperative. The eyes were compared clinically and histologically. Controls included unoperated eyes and eyes with lens extraction without implantation. Several histologic findings pertained equally to cataract extraction with or without lens implantation. Late opacification of the posterior capsule was caused by migrations and fibrous metaplasia of the lens epithelial cells. These cells appeared to undergo such metaplasia

Regional distribution prevalence of heterotopic ossification in the elbow joint: a 3D study of patients after surgery for traumatic elbow injury.

BACKGROUND: Heterotopic ossification (HO) is a common complication after elbow fracture surgery and can lead to severe upper extremity disability. The radiographic localization of postoperative HO has been reported previously. However, there is no literature examining the distribution of postoperative HO at the three-dimensional (3D) level. This study aimed to investigate 1) the distribution characteristics of postoperative HO and 2) the possible risk factors affecting the severity of postoperative HO at a 3D level. METHODS: A retrospective review was conducted of patients who presented to ou

Effects of Different Concentrations of Injectable Collagenase Enzyme on Capsular Tissue Around Silicone Implants: A Preliminary Experimental Study for the Development of a New Treatment Strategy.

OBJECTIVES: In recent studies, collagen organization was blamed for the formation of capsular contracture which is still a challenging problem after silicone implant-based breast operations. In this study, effects of different concentrations of collagenase enzyme derived from Clostridium histolyticum on the capsular tissue formation around the silicone implants were investigated. The injectable form of collagenase has a routine clinical use in the treatment of both Dupuytren's and Peyronie's diseases. MATERIALS AND METHODS: Thirty-two Wistar albino rats were randomized into four groups. A 2 ×

[Prevention of prothesis dislocation after the revision of total hip arthroplasty].

OBJECTIVE: To explore the role of reconstruction of the posterior capsule and external rotators in prevention of postoperative dislocation in total hip arthroplasty revision following the posterolateral approach. METHODS: Forty-five patients (47 hips) with the mean age of 65 years (55 to 78 years)of failed total hip arthroplasty were revised following the posterolateral approach. Posterior capsule was sutured to the anterosuperior portion of the capsule from where it had been detached, and the external rotators were then reattached to the soft tissue at the tip of the greater trochanter using

Enteroscopic diagnosis of intestinal malabsorption of a rare etiology: diaphragm disease.

We present the case of a 59-year-old patient with malabsorption syndrome which started with polyneuropathy. Capsule endoscopy and enteroscopy show villous denudation. Histological study confirms diaphragm disease of the small bowel. Diaphragm disease is a rare disease characterized by the presence of multiple thin diaphragms, such as septa, that narrow the intestinal lumen. The clinical features can be iron deficiency anemia, intestinal obstruction, change in bowel habits or acute abdomen secondary to perforation. It is usually generally with sustained use of high doses of non-steroidal anti-i

Frozen shoulder: overview of clinical presentation and review of the current evidence base for management strategies.

Adhesive capsulitis of the shoulder (ACS) is a condition with significant clinical and economic implications. The etiology of adhesive capsulitis is not clearly understood and there remains lack of consensus in clinical management for this condition. It can occur as a primary idiopathic condition or secondary to medical conditions or trauma. The hallmarks of ACS are pain and stiffness, caused by formation of adhesive or scar tissue in the glenohumeral joint. Management strategies vary depending on stage of presentation, patient factors and clinician preferences, and can range from conservative

Cochlear-Internal Canal and Cochlear-Facial Dehiscence: A Novel Entity.

Different types of otic capsule dehiscence restricted to the cochlea have been described. Here we describe the case of a patient with a cochlear-internal auditory canal dehiscence associated with a cochlear-facial dehiscence not reported before. A 53-year-old patient with severe to profound sensorineural hearing loss due to bilateral Meniere's disease underwent a cochlear implant surgery on the right ear. Preoperative brain magnetic resonance imaging findings were reported to be normal; during surgery, a cerebrospinal fluid gusher occurred at the time of round window opening. Postoperative com

Use of biodegradable collagen-glycosaminoglycan copolymer matrix implant to reduce postoperative fibrosis in strabismus surgery.

The occurrence of restrictive strabismus after conjunctival surgeries is widely described in the literature. Fibrosis causes adherence of the conjunctiva to the Tenon's capsule, intermuscular membrane, adipose orbital tissue, sclera, and extraocular muscle tissue. Fibrosis causes adherence of the conjunctiva to the Tenon's capsule, intermuscular membrane, adipose orbital tissue, sclera, and extraocular muscle tissue. We report a case of the use of a biodegradable collagen-glycosaminoglycan copolymer matrix implant (Ologen®) as an alternative adjuvant, used as a spacer to mechanically separate

Jellyfish sign for intraoperative identification of posterior lenticonus.

Posterior lenticonus is a rare progressive disease characterized by protrusion of posterior lens capsule along with lens cortex into the vitreous cavity. It may be associated with local thinning or absence of posterior lens capsule. It generally occurs sporadically, but familial cases have also been reported. If visually significant or if amblyopia is present, lens removal is indicated. Treatment consists of clear or cataractous lens extraction, optical correction along with prompt amblyopia therapy. In this case, we propose a "jellyfish sign" seen intraoperatively, which is referred to the ch

Histopathologically proven siderotic cataract with disintegrated intralenticular foreign body.

Cataract formation may be an indicator of early siderosis and has been associated with intralenticular foreign bodies. We report a unique case of histopathologically proven lens siderosis in a young man with a preceding history of trauma but no signs of retained intraocular foreign body. He presented with a total white cataract with brownish deposits on anterior capsule and underwent cataract surgery for same followed by histopathological staining of anterior capsule for iron deposits. This case illustrates the importance of close monitoring of patients with history of trauma or previous penet

Use of a hip arthroscopy flexible radiofrequency device for capsular release in frozen shoulder.

Adhesive capsulitis is a common and challenging condition to treat. Arthroscopic capsular release is usually contemplated when conservative treatment fails or when there is severe and/or chronic loss of range of motion. This procedure can be difficult to perform because of difficult access to the joint, poor visualization, and loss of working space from retraction of the joint capsule. The articular surfaces and the axillary nerve are also at higher risk of injury. Arthroscopic scissors, shavers, and electrocautery are typically used to perform the capsular release. To perform a safer and more

X-linked hereditary hemihypotrophy hemiparesis hemiathetosis.

Hereditary hemihypotrophy hemiparesis hemiathetosis syndrome (HHHH; OMIM 306960) was first reported in a family with congenital left hemiplegia in two males and moderately affected females. We describe a family with three males demonstrating congenital right hemiplegia with porencephalic lesions of the left internal capsule and putamen, or the periventricular white matter just above the internal capsule. Clinical findings within each family are limited to the same side, though sidedness differed between families. Both pedigrees were most consistent with X-linked inheritance. Genome-wide linkag

Incudostapedial joint arthroplasty using temporalis fascia/perichondrium ties.

Incudostapedial joint dislocation is the most frequently found ossicular chain defect. In these cases reconstitution of joint capsule is important in maintaining joint integrity. But reconstruction of dislocated incudostapedial joint is a challenging procedure as this joint is devoid of any muscular or soft tissue support. Here we describe a technique designed to reposition the incudostapedial joint in its anatomical position using temporalis fascia or perichondrium. Data have been collected and analyzed from 42 patients with incudostapedial joint discontinuity. The fascial ties used for recon

Osteogenesis imperfecta of the temporal bone: CT and MR imaging in Van der Hoeve-de Kleyn syndrome.

We report the progressive otic capsule demineralization around the membranous labyrinth and facial nerve in an adult patient with osteogenesis imperfecta tarda. Whereas the initial CT scan showed bandlike, undermineralized pericochlear areas, 2 years later, repeat CT performed because of hearing deterioration showed progression of these findings to the promontory, the round window niche, and the labyrinthine and tympanic segments of the facial nerve canal. MR imaging demonstrated enhancement of the abnormal otic capsule and of the intratemporal and canalicular facial nerve. The differential di
