The beta-subunit of chorionic gonadotrophin (beta HCG) was measured by radioimmunoassay in the serum of 190 consecutive patients admitted to hospital with the suspicion of ectopic pregnancy. The detection limit was set at 1 ng/ml. A urine sample was also taken for pregnancy testing on admission. 96 women with threatened abortion were monitored by repeated ultrasonic examinations. Simultaneously plasma HCG, progesterone, estradiol and estriol levels were measured. 12 patients with blighted ovum had hormonal levels in the low normal range before the 8th week and abnormal values after the 10th week. 31 patients had abnormal ultrasonic morphology and absence of fetal heart movements. One hundred patients admitted to hospital with a diagnosis of threatened abortion were assessed by means of sonar, urinary oestrogen, pregnanediol and human chorionic gonadotrophin (HCG) assays and clinical examination. Assay of oestrogen excretion was the most accurate (86-5 per cent) in predicting the ultimate outcome of pregnancy, but did not give as much information as sonar examination which gave an accurate prognosis in 84 per cent of cases and was much quicker to perform The reasons for the sonar failures are discussed. A case of ectopic gonadotropin production by a bronchogenic carcinoma is presented. Eleven similar cases have been previously reported. Urine gonadotropin titres appeared related to the size of the tumour mass since they decreased with response to treatment and increased when the tumour started progressing again. The hormone is most probably elaborated and secreted by the tumour cells.
