Community-associated methicillin-resistant Staphylococcus aureus.

Community-associated methicillin-resistant Staphylococcus aureus (CA-MRSA) has emerged over the last decade across the United States and the world, becoming a major pathogen in many types of community-acquired infections. Although most commonly associated with minor skin and soft tissue infections, such as furuncles, CA-MRSA also can cause necrotizing fasciitis, pyomyositis, osteoarticular infections, and community-acquired pneumonia. This article discusses the epidemiology, diagnosis, and management of these infections from the perspective of the emergency physician.

Controlling tungiasis in an impoverished community: an intervention study.

BACKGROUND: In Brazil, tungiasis is endemic in some resource-poor communities where various domestic and sylvatic animals act as reservoirs for this zoonosis. To determine the effect of control measures on the prevalence and intensity of infestation of human and animal tungiasis, a repeated cross-sectional survey with intervention was carried out. METHODOLOGY/PRINCIPAL FINDINGS: In a traditional fishing community in Northeast Brazil, humans and reservoir animals were treated, and premise-spraying using an insecticide was done, while a second fishing community served as a control. Both communi

Epidemiology of STD disparities in African American communities.

This article reviews the epidemiology of sexually transmitted disease (STD) disparities for African American communities in the United States. Data are reviewed from a variety of sources such as national case reporting and population-based studies. Data clearly show a disproportionately higher burden of STDs in African American communities compared with white communities. Although disparities exist for both viral and bacterial STDs, disparities are greatest for bacterial STDs such as gonorrhea, chlamydia, and syphilis. Gonorrhea rates among African Americans are highest for adolescents and you

Community-acquired acute kidney injury in Asia.

Asia, the largest continent in the world, is heterogeneous in the ethnic, socioeconomic, and developmental status of its populations. A vast majority of it is poor with no adequate access to modern health care, making an accurate estimation of the nature and extent of acute kidney injury (AKI) difficult. Community-acquired AKI in otherwise healthy individuals is common, and the population developing AKI is younger compared with its counterparts in Europe or North America. The etiologic spectrum varies in different geographic regions of Asia depending on environmental, cultural, and socioeconom

Microsurgical management of anterior communicating artery aneurysms.

OBJECTIVE: Aneurysms of the anterior communicating (ACOM) artery are the most frequently occurring type of ruptured intracranial aneurysms. The peculiar anatomy of the anterior communicating artery complex, its anatomic variations, and its multiple perforators, along with the deep location of these aneurysms and our difficulty accessing them, pose challenging anatomic problems in their surgical treatment. METHODS: We present our operative techniques for microsurgical treatment of ACOM artery aneurysms. Special, complex situations that may arise during treatment of these aneurysms and their so

Valvular heart disease in the community: a European experience.

The Euro Heart Survey on valvular heart disease included 5001 patients from 92 centers in 25 European countries in 2001: 71.9% had native valve disease and 28.1% had previously undergone valve surgery. Aortic stenosis (AS) and mitral regurgitation (MR) accounted for 43.1 and 33.6%, respectively, of single-valve diseases and were mostly caused by degenerative diseases. Mean age was 69 and 65 years, respectively, and at least one comorbidity was present in 36.3% of patients with AS and 41.7% with MR. Analysis of the therapeutic decision in patients with severe valve diseases showed that symptoma

Community-acquired Haemophilus influenzae meningitis in adults.

Haemophilus influenzae is an uncommon cause of bacterial meningitis in adults. This report describes a prospective evaluation of 16 episodes of community-acquired H. influenzae meningitis in a nationwide study on bacterial meningitis. Predisposing conditions were present in eight (50%) of the 16 episodes; the most common predisposing conditions were otitis or sinusitis (five episodes; 31%) and remote neurosurgery or head trauma (three episodes; 19%). One (6%) episode was fatal and hearing loss occurred in four (25%) episodes. It was concluded that H. influenzae meningitis in adults is a diseas

Is infection in the community a real threat? The example of CAUTI.

Targeting health care associated infections (HCAIs) is a key priority of current health policy. This applies as much to the community as to hospitals, although the latter receive far more media attention. One of the commonest HCAIs in the community is catheter-associated urinary tract infection (CAUTI). Cutting infection rates is a challenge. Many CAUTIs are the result of inappropriate catheterization, often combined with reduced sensitivity of bacteria to antibiotics given prophylactically. The government's rapid reaction task force has recommended use of a silver-alloy coated catheter, but c

Coccidioidomycosis as a common cause of community-acquired pneumonia.

The early manifestations of coccidioidomycosis (valley fever) are similar to those of other causes of community-acquired pneumonia (CAP). Without specific etiologic testing, the true frequency of valley fever may be underestimated by public health statistics. Therefore, we conducted a prospective observational study of adults with recent onset of a lower respiratory tract syndrome. Valley fever was serologically confirmed in 16 (29%) of 55 persons (95% confidence interval 16%-44%). Antimicrobial medications were used in 81% of persons with valley fever. Symptomatic differences at the time of e

[Management of community-acquired pneumonia in the physician's office].

The classical form of a community-acquired pneumonia is that caused by infection with pneumococcus, and differs clinically from atypical pneumonia in particular by fever-related differences. The diagnosis is based on the five cardinal symptoms: fever, cough, sputum production, pleuritic chest pain and a pulmonary infiltrate. Depending on whether there are further risk factors involved, a more or less comprehensive clinical and laboratory diagnostic work-up is needed. As a rule, oral antibiotic treatment with aminopenicillin, macrolides, fluorchinolones, ketolides or cephalosporins is applied,

The cognitive effects of electroconvulsive therapy in community settings.

Despite ongoing controversy, there has never been a large-scale, prospective study of the cognitive effects of electroconvulsive therapy (ECT). We conducted a prospective, naturalistic, longitudinal study of clinical and cognitive outcomes in patients with major depression treated at seven facilities in the New York City metropolitan area. Of 751 patients referred for ECT with a provisional diagnosis of a depressive disorder, 347 patients were eligible and participated in at least one post-ECT outcome evaluation. The primary outcome measures, Modified Mini-Mental State exam scores, delayed rec

Psychosocial factors and health: community and workplace study.

The effects of psychosocial factors on health have drawn growing attention. An important prerequisite for epidemiologic studies is that instruments to measure psychosocial factors be reliable and valid based on psychometric properties. The introduction of occupational stress models has made breakthroughs in conceptualizing real-life complex phenomena in the workplace. This article describes some trials that explore the associations between psychosocial factors and health in the community and workplace. Scales for measuring social support and psychosocial job characteristics were developed, and

[Catheter-related urinary tract infections in the community].

The prevalence of urinary catheterization in the community ranges from 0.02% to 0.07%. Despite the generalized use of closed systems, the risk of bacteriuria in patients with urethral catheters is 3%-10% per day and its presence is universal when the device remains in place for 30 days or longer. Although most of these episodes of bacteriuria are asymptomatic, up to 30% of them lead to clinical symptoms and complications, including severe sepsis and death. The microorganisms infecting the urine of catheterized patients frequently belong to species less susceptible to antibiotics and form biofi
