A prospective outcome evaluation of arthroscopic Bankart repairs: minimum 2-year follow-up.

BACKGROUND: Arthroscopic treatment of anterior shoulder dislocation has become possible through improvements in instruments and techniques. OBJECTIVE: To prospectively evaluate results of arthroscopic Bankart repairs at a minimum 2-year follow-up for patients with histories of shoulder dislocation and an anterior-inferior labral tear at the time of diagnostic arthroscopy. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: A consecutive series of 85 patients (70 men, 15 women; mean age, 26 years) with Bankart lesions were treated with arthroscopic repair using suture anchors; 18 patie

Soft tissue mineralization: roentgen analysis.

Numerous, incompletely understood, and undetermined physiologic factors may exert further but unappreciated influences on the development of ectopic calcification and ossification. In the former instance, in addition to serum calcium and phosphorous ion concentrations, tissue pH, blood supply, hormones, i.e., vitamin D, vitamin A, and various enzymes (e.g., alkaline phosphatase and pyrophosphatase) may all play significant, ancillary, time-dependent, but as yet undetermined roles. Ossification, like calcification, may occur in association with many types of disorders and under a variety of cir

Post-traumatic distal humerus non-union : Open reduction and internal fixation: long-term results.

The objective of this paper is to evaluate the long-term functional results achieved after open reduction and internal fixation of 24 distal humerus non-unions. Non-unions were extra-articular-extracapsular (11 cases), extra-articular-intracapsular (8 cases) and intra-articular (5 cases). Preoperative elbow range of motion averaged 45 degrees. Time between original trauma and revision surgery averaged 14 months. Stabilisation methods varied according to type and location of the non-union. Follow-up averaged 46 months (range: 18-108). Elbow range of motion at last examination averaged 98 degree

A comparison of proximal radioulnar synostosis excision after trauma and distal biceps reattachment.

We tested the hypothesis that the results of operative resection of a proximal radioulnar synostosis are better when the synostosis is due to a distal biceps reattachment (11 patients) than when it occurs after trauma (13 patients). Two patients in the trauma cohort had recurrence of the synostosis, and 1 had repeat resection. Two patients in the trauma cohort and 1 in the biceps cohort had substantial loss of forearm rotation due to regrowth of heterotopic ossification without synostosis, and 2 patients were addressed with subsequent surgeries. The arc of forearm rotation after the index surg

Quantitative measurement of radial head fracture location.

BACKGROUND: The most common location of a displaced fracture of part of the radial head is often described as the anterior lateral aspect of the radial head with the forearm in neutral position, based on observation rather than precise measurements. The purpose of our study was to measure the exact location of fractures involving part of the radial head using quantitative 3-dimensional computed tomography (CT). MATERIALS AND METHODS: We measured the fracture lines with respect to the biceps tuberosity in 24 patients with a displaced articular fracture of part of the radial head (Mason type 2)

Dome osteotomy for posttraumatic cubitus varus: a surgical technique to avoid lateral condylar prominence.

The indication for surgery in most children with posttraumatic cubitus varus is the presence of an unsightly deformity. The function of the limb is generally not impaired. Lateral closing-wedge supracondylar osteotomy, although a widely used corrective procedure, tends to produce lateral condylar prominence, thus jeopardizing the cosmetic outcome. The authors used the dome supracondylar osteotomy, as described by Tien et al, as the corrective procedure for cubitus varus in 12 consecutive children. The average follow-up was 2.3 (range 1-4) years. The objective evaluation was done by one of the

Treatment of primary total distal biceps tendon rupture using cortical button, transosseus fixation and suture anchor: A single center experience.

INTRODUCTION: There remains as of yet no consensus on the optimal treatment for total or partial distal biceps tendon repairs. As such, the purpose of this study was to assess functional outcome, the impact of complications and cost effectiveness, in patients undergoing primary distal biceps tendon repair by either cortical button (CB), transosseous suture (TO) or suture anchor (SA). HYPOTHESIS: There is no difference in functional outcome and cost effectiveness, in patients undergoing distal biceps tendon repair. MATERIAL & METHODS: A retrospective analysis was performed on prospectively co

A one-stop approach to the management of soft tissue and degenerative musculoskeletal conditions using clinic-based ultrasonography.

BACKGROUND AND OBJECTIVE: One-stop clinics aim at assessment, investigation and initiating treatment in a single hospital visit. They have been proven to be cost-effective and to increase patient satisfaction in various specialties. The aim of this study was to describe the one-stop approach to managing soft tissue and degenerative musculoskeletal conditions using clinic-based musculoskeletal ultrasonography (MSUS). METHODS: A retrospective case record review was carried out of patients assessed and managed in the musculoskeletal clinic by a musculoskeletal and sports physician over a 10-mont

Restoration of elbow function by intercostal nerve transfer for obstetrical paralysis with co-contraction of the biceps and the triceps.

Among the conservatively treated patients with obstetrical paralysis after delivery in the vertex presentation, cross reinnervation sometimes occurs in the course of their recovery and co-contraction of multiple muscles impairs smooth upper limb motion. Such co-contraction of the biceps and the triceps inhibits normal elbow motion, making it impossible to use the elbow effectively in daily activities, despite adequate strength in these muscles. To overcome biceps/triceps co-contraction, we performed intercostal nerve transfer to the musculocutaneous nerve for three patients of age 11 months, 6

Clinical and radiographic outcomes of cementless reverse total shoulder arthroplasty for proximal humeral fractures.

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) has demonstrated successful outcomes in the treatment of both acute and chronic proximal humeral fractures (PHFs). The traditional RTSA surgical technique uses a methyl methacrylate cemented humeral component to restore and maintain both humeral height and retroversion. However, use of humeral bone cement has been associated intraoperatively with cardiopulmonary risk, increased operative cost, and postoperatively with difficulty if revision arthroplasty is required. We report the clinical and radiographic outcomes of a completely cementles

Reconstruction of the interosseous membrane of the forearm with a graft substitute: a cadaveric study.

PURPOSE: Longitudinal radioulnar dissociation occurs when traumatic axial loading through the wrist disrupts the interosseous membrane (IOM) of the forearm and fractures the radial head (Essex-Lopresti injury). Proximal migration of the radius results in a wrist with a positive ulnar variance, which leads ultimately to painful ulnar-sided wrist degeneration and wrist pain during grasping activities that involve axial loading or ulnar deviation of the wrist. In theory reconstruction of the IOM with a graft substitute can limit proximal migration of the radius, thereby preserving wrist function.

Feasibility and impact of an expanded upper extremity musculoskeletal health for musicians (MHM) program on arm and hand pain in musicians.

BACKGROUND: This easibility cohort study follows a prior randomized control pilot trial that examined the feasibility and impact of a formal Musculoskeletal Health for Musicians (MHM) program. In this follow-up study, an expanded program was designed to target hand and arm pain by adding exercises for the wrist, elbow, forearm, and hand intrinsic muscles. The MHM program combines education with exercise instruction to prevent pain and injury in musicians. PURPOSE: The aim of this study was to design and implement an expanded MHM program targeted at arm and hand pain and formally assess feasib

[Tear of the Distal Biceps Brachii Tendon - Correlation of Ultrasound and Operative Findings, Surgical Therapy Results].

PURPOSE OF THE STUDY When treating tears of the distal biceps brachii muscle tendon, we repeatedly noticed a difference between the preoperative ultrasound findings and the operative findings. The aim of the study was to retrospectively correlate these findings in order to determine the sensitivity of the ultrasound examination in everyday orthopaedic practice. Moreover, we compared the results and complications of surgical treatment through two operative techniques used at our department. MATERIAL AND METHODS In the 2004-2016 period 20 patients underwent a surgery at our department for total

Functional Outcome of Radius and Ulnar Shaft Fractures Treated With Intramedullary Kirschner's Wire Fixation in Children.

INTRODUCTION: Fractures are a common occurrence in childhood, with approximately one-third of boys and girls sustaining at least one fracture before the age of 17. Both-bone forearm fractures, particularly those involving the radius and ulna, are more common in the non-dominant hand and in boys and usually involve the distal portions of both bones. If not properly treated, these injuries can have a significant impact on limb function. OBJECTIVE: This study aimed to determine the functional outcomes of radius and ulnar shaft fractures in children treated with intramedullary Kirschner's wire fi
