(18)F-FDG PET/CT imaging of critical ischemia in the diabetic foot.

A 72-year-old patient with type 2 diabetes mellitus underwent F-FDG PET/CT for diabetic foot infection. No increased focal uptake was seen. F-FDG uptake was absent in the second, third, and fourth toes of the right foot. Angiography demonstrated severe stenosis in the proximal anterior tibial artery and segmental stenosis on the posterior tibial artery. The patient was diagnosed as having dry gangrene of the second to fourth toes of the right foot due to ischemia. He underwent a selective amputation because of the lack of revascularization possibilities.

Deep wound cultures correlate well with bone biopsy culture in diabetic foot osteomyelitis.

BACKGROUND: Osteomyelitis is a major complication in patients with diabetic foot ulceration. Accurate pathogenic identification of organisms can aid the clinician to a specific antibiotic therapy thereby preventing the need for amputation. METHODS: All diabetic patients with bone biopsy-confirmed osteomyelitis were included into the study: biopsies were performed either during surgical removal of infected bone or percutaneously under guided fluoroscopy through non-infected tissue. The depth and extent of the ulcer was assessed using a sterile blunt metal probe. Deep wound cultures were taken …

Minimally invasive soft tissue release of foot and ankle contracture secondary to stroke.

Lower extremity contracture associated with stroke commonly results in a nonreducible, spastic equinovarus deformity of the foot and ankle. Rigid contracture deformity leads to gait instability, pain, bracing difficulties, and ulcerations. The classic surgical approach for stroke-related contracture of the foot and ankle has been combinations of tendon lengthening, tendon transfer, osteotomy, and joint fusion procedures. Recovery after traditional foot and ankle reconstructive surgery requires a period of non-weightbearing that is not typically practical for these patients. Little focus has be…

Effects of a foot bath containing green tea polyphenols on interdigital tinea pedis.

BACKGROUND: It has been shown that green tea polyphenols (GTP) can directly kill Trichophyton in vitro; however, there are no published clinical studies that show anti-fungal activity of GTP. OBJECTIVES: To identify the effects of GTP on interdigital tinea pedis in elderly patients. METHOD: Ninety-four patients with interdigital tinea pedis were enrolled and were either given a lukewarm water foot bath containing GTP or placebo treatment. Effects of GTP were assessed based on changes in the size of the affected area, the rate of recurrence, microscopy findings, and overall assessment of skin…

Changing aetiology of hand, foot and mouth disease in Linyi, China, 2009-2011.

Millions of hand, foot and mouth disease cases occur annually and the disease is considered a significant public health threat in China. We have focused on Linyi for 3 years to analyse the aetiological spectrum and epidemic changes. The reported number of cases of HFMD in Linyi declined in 2011,with the percentage of HEV71 dropping to 11%. Although CVA16 rose to 51% in 2011, the number of CVA16 cases did not change significantly compared with the previous 2 years. Other types of enteroviruses were detected in 2011 and might play more important roles in future epidemics.

[Relationship between G6PD deficiency and hand-foot-mouth disease induced by enterovirus 71].

OBJECTIVE: To study the influence of glucose-6-phosphate dehydrogenase (G6PD) deficiency on hand-foot-mouth disease (HFMD) induced by enterovirus 71 (EV71) , and possible mechanisms. METHODS: A total of 220 boys with HFMD induced by EV71 were classified into two groups based on disease severity: mild/moderate (n=145) and severe HFMD groups (n=75), and 132 healthy boys were selected as the control group. The activity of G6PD and levels of reduced glutathione (GSH) and malonaldehyde (MDA) in blood were measured using the automatic biochemical analyzer. RESULTS: The percentage of G6PD deficienc…

Correction of a severe poliomyelitic equinocavovarus foot using an adjustable external fixation frame.

One-stage surgical correction of severe equinocavovarus deformity can result in complications ranging from skin necrosis to tibial nerve palsy. Fewer complications have been reported when severe deformities were treated by gradual correction using external frames such as the Ilizarov external fixator or the Taylor Spatial Frame™. We describe a case of a 64-year-old female patient with severe poliomyelitic equinocavovarus whose deformity required her to ambulate using the dorsum of her right foot as a weightbearing surface. We treated the deformity with gradual correction using a Taylor Spatial…

Symmetry of foot alignment and ankle flexibility in paediatric Charcot-Marie-Tooth disease.

BACKGROUND: Charcot-Marie-Tooth disease is the most common inherited nerve disorder and typically presents with pes cavus foot deformity and ankle equinus during childhood. Level in the variation of symmetry of musculoskeletal lower limb involvement across the clinical population is unknown, despite early reports describing gross asymmetry. METHODS: We measured foot alignment and ankle flexibility of the left and right limbs using accurate and reliable standardised paediatric outcome measures in 172 patients aged 3-20 years with a variety of disease subtypes recruited from the United States, …

Enteroviruses isolated from herpangina and hand-foot-and-mouth disease in Korean children.

Hand-foot-and-mouth disease (HFMD) and herpangina are commonly prevalent illness in young children. They are similarly characterized by lesions on the skin and oral mucosa. Both diseases are associated with various enterovirus serotypes. In this study, enteroviruses from patients with these diseases in Korea in 2009 were isolated and analyzed. Demographic data for patients with HFMD and herpangina were compared and all enterovirus isolates were amplified in the VP1 region by reverse transcription-polymerase chain reaction and sequenced. Among the enterovirus isolates, prevalent agents were cox…

Effect of ankle-foot orthosis on postural control after stroke: a systematic review.

INTRODUCTION: Stroke is currently the main cause of permanent disability in adults. The impairments are a combination of sensory, motor, cognitive and emotional changes that result in restrictions on the ability to perform basic activities of daily living (BADL). Postural control is affected and causes problems with static and dynamic balance, thus increasing the risk of falls and secondary injuries. The purpose of this review was to compile the literature to date, and assess the impact of ankle-foot orthosis (AFO) on postural control and gait in individuals who have suffered a stroke. DEVELO…

Rpl27a mutation in the sooty foot ataxia mouse phenocopies high p53 mouse models.

Ribosomal stress is an important, yet poorly understood, mechanism that results in activation of the p53 tumour suppressor. We present a mutation in the ribosomal protein Rpl27a gene (sooty foot ataxia mice), isolated through a sensitized N-ethyl-N-nitrosourea (ENU) mutagenesis screen for p53 pathway defects, that shares striking phenotypic similarities with high p53 mouse models, including cerebellar ataxia, pancytopenia and epidermal hyperpigmentation. This phenocopy is rescued in a haploinsufficient p53 background. A detailed examination of the bone marrow in these mice identified reduced n…
