The arterial stiffness and pulse pressure are predictors of intraocular pressure in a middle-aged population.

The vascular theory posits that glaucoma is associated to ocular blood flow alterations. However, there is uncertainty regarding the association of arterial stiffness index (ASI) and pulse pressure (PP) to intraocular pressure (IOP). This study investigates the relationship between ASI and PP, and IOP, using the UK Biobank population. Among 89,930 participants of the UK Biobank population, ASI and PP were assessed, and their correlations with IOP were estimated using multiple linear and logistic regressions adjusted for age, sex, BMI, diabetes, dyslipidemia, mean blood pressure, heart rate, to

Evaluating the Prognostic Value of Kansas City Cardiomyopathy Questionnaire (KCCQ) Scores for 6-Month Readmissions in Southeast Asian Populations With Heart Failure.

BACKGROUND: Heart failure (HF) is a prevalent cause of hospital readmissions. Our study aims to determine the correlation between the Kansas City Cardiomyopathy Questionnaire (KCCQ) scores and 6-month readmission in our Southeast Asian population. METHODS: We evaluated KCCQ-12 in a cohort of 180 patients at first post-discharge visit after a recent hospitalization for HF with reduced ejection fraction (HFrEF). Logistic regression was used to determine the predictive significance of the KCCQ scores for 6-month HF readmission. The selection of predictive parameters was performed using Stepwise

Resting Hyperinflation Predicts Incremental Shuttle Walk Distance in Chronic Obstructive Pulmonary Disease.

Rationale: The incremental shuttle walk test (ISWT) correlates closely with peak oxygen uptake in COPD and relates to important outcomes such as mortality, response to treatment and hospital readmission. Despite this, there is limited data on the physiological determinants of ISWT distance (ISWD) in COPD. Methods: In this exploratory, prospective observational study, spirometry, lung volumes, diffusion capacity (DLCO) and oscillometry were performed in patients with confirmed COPD. Patients then completed two ISWT with the results of the best test, measured by distanced walked taken. The deter

Predicting Risk of Cardiovascular Disease EVENTs (PREVENT) Equations: What Clinicians Need to Know?

PURPOSE OF REVIEW: This review aims to examine the rationale, development, and implications of the newly developed Predicting Risk of CVD EVENTs (PREVENT) equations for cardiovascular disease (CVD) risk assessment. RECENT FINDINGS: The PREVENT equations were developed from diverse, contemporary, real-world datasets and offer accurate discrimination for predicting risk of total CVD and separately, atherosclerotic CVD (ASCVD) and heart failure (HF). It addresses the nearly twofold overprediction of ASCVD risk with PCEs and includes risk factors related to cardiovascular-kidney-metabolic (CKM) s

Physiological responses to acute warming at the agitation temperature in a temperate shark.

Thermal tolerance and associated mechanisms are often tested via the critical thermal maximum (CTmax). The agitation temperature is a recently described thermal limit in fishes that has received little mechanistic evaluation. The present study used a temperate elasmobranch fish to test the hypothesis that this thermal tolerance trait is partially set by the onset of declining cardiorespiratory performance and the cellular stress response. Pacific spiny dogfish (Squalus suckleyi) were screened for cardiorespiratory and whole-organism thermal limits to test for associations between thermal perfo

Orthostatic systemic and cerebral hemodynamics in newly injured patients with spinal cord injury.

BACKGROUND: Low blood pressure (BP) may lead to reduced cerebral blood flow velocity (CBFv) during an orthostatic challenge in newly injured patients with traumatic spinal cord injury (SCI), which, may relate to the neurological level of injury (NLI) as documented on the International Standards for the Neurologic Classification of SCI (ISNCSCI), or to evidence of cardiovascular autonomic impairment as determined by the International Standards to document remaining Autonomic Function after SCI (ISAFSCI). OBJECTIVE: To examine the influence of patient demographics, ISNCSCI and ISAFSCI scores on

Nitroxyl Donor CXL-1020 Lowers Blood Pressure by Targeting C195 in Cyclic Guanosine-3',5'-Monophosphate-Dependent Protein Kinase I.

BACKGROUND: We previously demonstrated that nitroxyl causes vasodilation, at least in part, by inducing the formation of an intradisulfide bond between C117 and C195 in the high affinity cyclic guanosine monophosphate-binding site of PKGI (cyclic guanosine monophosphate-dependent protein kinase I). The aim of this study was to determine whether nitroxyl donors lower blood pressure via this novel PKGI activation mechanism in vivo. METHODS: To determine this, a C195S PKGI knock-in mouse model was generated that ubiquitously and constitutively expresses a mutant kinase resistant to nitroxyl-indu

Autonomic dysfunction and cardiovascular risk in post-traumatic stress disorder.

BACKGROUND: Patients with post-traumatic stress disorder (PTSD) have an increased risk for cardiovascular disease. The underlying mechanisms are unclear but impaired autonomic function may contribute. However, research in this field has shown contradictory results and the causal links between PTSD, autonomic dysfunction, and cardiovascular risk remain unknown. This brief review summarizes the current knowledge on alterations in autonomic function and cardiovascular risk in patients with PTSD. LITERATURE SEARCH STRATEGY: A PubMed search of the literature was performed using the following keywo

Quantitative analysis of dysautonomia in patients with autonomic dysreflexia.

Autonomic dysreflexia (AD) is a life-threatening condition for individuals with cervical or high-thoracic spinal cord injury (SCI). The profile of autonomic dysfunction in AD using validated clinical autonomic tests has not been described so far, although it could be useful to identify SCI patients at greater risk of developing AD non-invasively. With this objective, 37 SCI patients (27% female) were recruited, and hemodynamic and cardiac parameters were continuously monitored to determine the presence of AD, defined as an increase of systolic blood pressure of 20 mmHg or higher after bladder

The First Lactate Threshold Is a Limit for Heavy Occupational Work.

Long-term heavy physical work often leads to early retirement and disability pension due to chronic overload, with a need to define upper limits. The aim of this study was to evaluate the value of the first lactate threshold (LTP1) as a physiological marker for heavy occupational work. A total of 188 male and 52 female workers performed an incremental cycle ergometer test to determine maximal exercise performance and the first and second lactate (LTP1; LTP2) and ventilatory thresholds (VT1; VT2). Heart rate (HR) recordings were obtained during one eight-hour shift (HR8h) and oxygen uptake was

Transcatheter Closure of Perimembranous Ventricular Septal Defects Using Different Generations of Amplatzer Devices: Multicenter Experience.

OBJECTIVES: To demonstrate safety and efficacy of using different generations of softer Amplatzer™ devices for ventricular septal defect (VSD) closure to avoid serious complications at follow-up. BACKGROUND: Transcatheter closure of perimembranous ventricular septal defects (PmVSD) is a well-established procedure; however, it is associated with unacceptable incidence of complete heart block. Great advantages have been achieved by using softer devices for VSD transcatheter closure. The first and second generation of Amplatzer™ occluders (AVP II, ADO, and ADO II) seem to offer a safe and attrac

Maternal Cardiovascular Disease 3 Decades After Preterm Birth: Longitudinal Cohort Study of Pregnancy Vascular Disorders.

Women who deliver preterm are at risk of cardiovascular disease, but the reason for the association is unclear. We determined whether vascular disorders during pregnancy explain the association between preterm delivery and future maternal cardiovascular disease. We analyzed a longitudinal cohort of 1 199 364 pregnant women with 19 186 983 person-years of follow-up in Quebec between 1989 and 2017. We calculated incidence rates of myocardial infarction, ischemic stroke, and other cardiovascular hospitalizations. We used multivariable Cox regression to estimate adjusted hazard ratios and 95% CIs

Association of the Dundee severity classification with mortality, length of stay and readmission in adult inpatients with cellulitis.

BACKGROUND: The Dundee classification is a simple severity assessment tool that could optimize treatment decisions and clinical outcomes in adult patients with cellulitis; however, it has not been validated in a large cohort. OBJECTIVES: To determine whether the Dundee classification reliably identified those patients with cellulitis who had a higher mortality, a longer length of hospital stay or an increased risk of readmission. METHODS: We performed a retrospective study of all adults with a primary discharge diagnosis of cellulitis admitted to Auckland City Hospital from August 2013 to Ju

Determinants of the high admission blood pressure in mild-to-moderate acute intracerebral hemorrhage.

BACKGROUND AND PURPOSE: An early elevation in blood pressure (BP) is common after spontaneous intracerebral hemorrhage (ICH), has various potential causes, and is predictive of poor outcome. We aimed to determine the predictors of this phenomenon, in pooled analyses of the Intensive Blood Pressure Reduction in Acute Cerebral Hemorrhage Trials [INTERACT1 (n = 404) and INTERACT2 (n = 2829)]. METHODS: INTERACT trials were international, open, blinded endpoint, randomized controlled trials of patients with spontaneous ICH (<6 h) and elevated SBP (150-220 mmHg) assigned to intensive (target SBP <

Central Arterial Function Measured by Non-invasive Pulse Wave Analysis is Abnormal in Patients with Duchenne Muscular Dystrophy.

Duchenne muscular dystrophy (DMD) is an X-linked recessive disorder caused by mutation of dystrophin. Cardiovascular involvement includes dilated cardiomyopathy. Non-invasive assessment of vascular function has not been evaluated in DMD. We hypothesize arterial wave reflection is abnormal in patients with DMD. Pulse wave analysis was performed on DMD patients with a SphygmoCor SCOR-PVx System to determine central blood pressure and augmentation index (AIx) as an assessment of arterial wave reflection. Results were compared to a control group. A total of 43 patients with DMD were enrolled, and

Hemodynamic characterization of hypertensive patients with an exaggerated orthostatic blood pressure variation.

UNLABELLED: Exaggerated orthostatic blood pressure variation (EOV) is a poorly understood phenomenon related to high cardiovascular risk. We aimed to determine whether hypertensive patients with EOV have a distinct hemodynamic pattern, assessed through impedance cardiography. METHODS: In treated hypertensive patients, we measured the cardiac index (CI), systemic vascular resistance index (SVRI), blood pressure (BP), and heart rate (HR) in the supine and standing (after 3 minutes) positions, defining three groups according to BP variation: 1) Normal orthostatic BP variation (NOV): standing sys
