The influence of functional electrically stimulated ankle plantar flexors on the swinging lower extremity was studied in incomplete spinal cord injured persons. Stimulation sequences with different time and frequency parameters were delivered to ankle plantar flexors and knee extensors and to the peroneal nerve. The results of kinematic assessment showed that stimulated calf muscles provide noticeable forward and upward propulsion to the swinging leg.

The successful management of an extensive non-atherosclerotic ilio-femoro-popliteal aneurysm is described. This very rare aneurysm developed at the site of an old penetrating lower thigh wound caused by an infected arrow. To the best of our knowledge this is the first reported case in English literature.

Physical rehabilitation after lower limb traumatic amputation due to acts of war has not yet been investigated. It has been shown that patients suffering non-war injuries rehabilitate well. This study shows that despite the difficult circumstances and limited resources available in an area of conflict, good rehabilitation of war victims is possible. However, economic rehabilitation is worse than that seen in non-war populations. Given that this type of injury is very common in war zones worldwide, this failure of economic rehabilitation may represent a major healthcare issue.

Between 1983 and 1993, 16 children with 18 lower extremity power lawn mower-related injuries were treated at Children's Hospital of Philadelphia. Eleven of 16 patients (69%) were bystanders or nonoperators. The average age at injury was 4 years 9 months. Length of follow-up averaged 3 years 10 months. Calcific myonecrosis of lower extremity muscles has been identified as an uncommon late sequela of posttraumatic compartment syndromes or ischemic myonecrosis. A 5-year-old girl had a traumatic amputation of her left thigh and on the right side loss of the posterior part of the talus and parts of the posterior tibial nerve and artery. These defects were treated with free transplantation of the corresponding part of the talus from the amputated left leg and a vascularized graft from the left posterior tibial nerve and artery. 2 years later the girl was able to walk without crutches, the ankle mobility was good, and so was the sensibility in the sole of the foot.

Operations were conducted on 22 patients with defects in soft tissues and trophic ulcers of cicatrix in the region of the calcaneal tendon. The walking cast was used to repair 46 fractures of long bones (distal radius and tibia, metacarpus and metatarsus) and 10 fractures of first phalanx of 35 horses and 21 ponies. The walking cast proved to be highly effective. Lower extreemity injury complicated by limb-threatening vascular injuries of the popliteal artery present an infrequent but difficult management problem. We report clinical and bone morphometric findings in 18 osteoporotic patients who experienced stress fractures during fluoride therapy. Patients were treated with either sodium fluoride (n = 15), or sodium monofluorophosphate (n = 3). Oral calcium supplementation was given in 11 patients, and vitamin D in 13. Eighty-seven lower extremity fractures in 71 paraplegics were retrospectively evaluated to determine adequacy of treatment, functional result, and range of motion of the hip and knee. Most fractures treated closed healed without difficulty, but with a complication rate of 19%.
