Sleep and hypnotic drugs.

In recent years the effectiveness of hypnotic drugs has had to be assessed in terms of a greatly increased knowledge of the physiology and pathology of sleep. The normal pattern of sleep and wakefulness involves a cyclic alternation between three rather than two basically dissimilar states of the brain and body - alert wakefulness, rapid-eye-movement (REM) sleep and non-rapid-eye-movement (NREM) sleep. The pattern of this alternation in individual people results from the interaction of many influences - biological (including genetic, early developmental and later degenerative influences), psyc

Laryngoscopic evaluation of arytenoid movements in pure Arabian horses.

BACKGROUND: The equine larynx can be affected by a variety of diseases that may interfere with upper respiratory tract patency. Laryngeal hemiplegia is a well-known disease in large-sized equine breeds and its occurrence in Arabian horses has not been thoroughly investigated. AIM: To endoscopically assess arytenoid movements in pure Arabian horses. METHODS: A total number of 38 pure Arabian horses were examined. When the horse was unsedated, the laryngeal arytenoid movement was assessed using a laryngoscope, and a grading score was assigned accordingly. RESULTS: Evidence of laryngeal moveme

Treatment of Chorea in Childhood.

Chorea is a movement disorder characterized by ongoing random-appearing sequences of discrete involuntary movements or movement fragments. Chorea results from dysfunction of the complex neuronal networks that interconnect the basal ganglia, thalamus, and related frontal lobe cortical areas. The complexity of basal ganglia circuitry and vulnerability of those circuits to injury explains why chorea results from a wide variety of conditions. Because etiology-specific treatments or effective symptomatic treatments are available for causes of chorea, defining the underlying disease is important. Th

EMDR: a putative neurobiological mechanism of action.

Numerous studies have provided evidence for the efficacy of eye movement desensitization and reprocessing therapy (EMDR) in the treatment of posttraumatic stress disorder (PTSD), including recent studies showing it to be more efficient than therapist-directed flooding. But few theoretical explanations of how EMDR might work have been offered. Shapiro, in her original description of EMDR, proposed that its directed eye movements mimic the saccades of rapid eye movement sleep (REM), but provided no clear explanation of how such mimicry might lead to clinical improvement. We now revisit her origi

Saccade responses to dopamine in human MPTP-induced parkinsonism.

Depletion of dopamine content in the substantia nigra resulting from 1-methyl-4-phenyl-1,2,5,6-tetrahydropyridine (MPTP) toxicity produces parkinsonism. Management of 3 patients with MPTP-induced parkinsonism required drug holidays during which there was a state of dopamine depletion followed by dopamine replacement. We used this opportunity to study the effect of the selective loss of pars compacta dopaminergic cells on vertical and horizontal saccade (fast) eye movements. During the drug holidays, visually guided saccades were hypometric and had long latencies but retained a normal saccade v

Epsilon-sarcoglycan mutations found in combination with other dystonia gene mutations.

Myoclonus-dystonia is a movement disorder associated with mutations in the epsilon-sarcoglycan gene (SGCE) in most families and in the DRD2 and DYT1 genes in two single families. In both of the latter families, we also found a mutation of SGCE. The molecular mechanisms through which the detected mutations may contribute to myoclonus-dystonia remain to be determined.

Periodic limb movements in Parkinson's disease: a critical review of pathophysiology and a framework for clinical management.

Periodic limb movements (PLM) refer to periodic episodes of repetitive and stereotypical limb movements, predominantly affecting the lower extremities. The prevalence of PLM is estimated at 17.6% to 86.7% in patients with Parkinson's disease (PD), which is much higher than 4% to 11% in the general population. PD is a chronic neurodegenerative movement disorder characterized by both motor and non-motor symptoms (NMS), and PLM represents one of the common non-motor manifestations. PLM in PD has garnered increasing attention due to its high prevalence, association with reduced sleep quality, elev

Изучение нейродегенеративных заболеваний: преодоление разрыва между моделями in vitro и in vivo.

Genetic mutations disrupt the coordinated mode of tyrosinase intra-melanosomal domain.

Oculocutaneous albinism type 1 is a genetic disorder caused by the disruption of tyrosinase activity in the melanogenesis pathway. The tyrosinase's intramelanosomal domain can be subdivided into the catalytic and Cys-rich subdomains, integral for protein stability and catalytic activity. To understand the motions in the tyrosinase intra-melanosomal subdomains and their link to its catalytic activity, we perform essential dynamics on homology models for tyrosinase and the mutant variants R217Q, R402Q, and R217Q/R402Q. Dimensional reduction techniques, such as Principal Component Analysis (PCA),

A Novel Feature from Instrumented Utensils for Clinical Assessment of Friedreich Ataxia.

Friedreich Ataxia (FRDA) is an inherited disorder that affects the cerebellum and other regions of the human nervous system. It causes impaired movement that affects quality and reduces lifespan. Clinical assessment of movement is a key part of diagnosis and assessment of severity. Recent studies have examined instrumented measurement of movement to support clinical assessments. This paper presents a frequency domain approach based on Average Band Power (ABP) estimation for clinical assessment using Inertial Measurement Unit (IMU) signals. The IMUs were attached to a 3D printed spoon and a cup

Tardive Dyskinesia Following Low-Dose Risperidone.

Tardive dyskinesia is an involuntary athetoid or choreiform movement lasting a minimum of a few weeks. It is associated with the use of neuroleptic medication for at least three months and persists beyond four to eight weeks. Tardive dyskinesia usually occurs as a result of the long-term use of dopamine receptor-blocking agents, mainly first-generation antipsychotics or a high-dose, second-generation antipsychotic. We present a case of a 28-year-old female with osteogenesis imperfecta presented later with major depressive disorder with psychotic features. She was given a low-dose second-genera

Quantitative evaluation of posture control in rats with inferior olive lesions.

Impairment of inferior olivary neurons (IONs) affects whole-body movements and results in abnormal gait and posture. Because IONs are activated by unpredicted motion rather than regular body movements, the postural dysfunction caused by ION lesions is expected to involve factors other than simple loss of feedback control. In this study, we measured the postural movements of rats with pharmacological ION lesions (IO rats) trained to stand on their hindlimbs. The coordination of body segments as well as the distribution and frequency characteristics of center of mass (COM) motion were analyzed.

An Expert System for Quantification of Bradykinesia Based on Wearable Inertial Sensors.

Wearable sensors and advanced algorithms can provide significant decision support for clinical practice. Currently, the motor symptoms of patients with neurological disorders are often visually observed and evaluated, which may result in rough and subjective quantification. Using small inertial wearable sensors, fine repetitive and clinically important movements can be captured and objectively evaluated. In this paper, a new methodology is designed for objective evaluation and automatic scoring of bradykinesia in repetitive finger-tapping movements for patients with idiopathic Parkinson's dise

DPAGT1-CDG: report of a patient with fetal hypokinesia phenotype.

Congenital disorders of glycosylation (CDG) are due to either defects in the synthesis of the glycan moiety of glycoproteins or glycolipids and in the attachment of the glycans to proteins and lipids. Some 50 CDG have been identified. They represent a challenge for clinicians because most are multisystem diseases with a heterogeneous spectrum of clinical manifestations with involvement of any organ and system. We report on a patient with a mutation in the glycosyltransferase encoded by the DPAGT1 gene, an infrequent CDG. He showed severe fetal hypokinesia phenotype with decreased fetal movemen

[Cervical dystonia--diagnosis and treatment].

BACKGROUND: Cervical dystonia is a focal dystonia of the neck muscles, leading to involuntary head movement, abnormal postures and pain. As effective treatment is available, all doctors should be able to recognize this potentially disabling disorder. MATERIAL AND METHODS: The manuscript is based on literature retrieved from searches in Medline databases, and the author's own clinical experience and research. RESULTS AND INTERPRETATION: Diagnosis is based on identification of typical symptoms and signs. Cervical dystonia is most often primary, with unknown cause, but can also be secondary to

Open reduction of pediatric atlantoaxial rotatory fixation: long-term outcome study with functional measurements.

OBJECT: Atlantoaxial rotatory fixation (AARF) is an uncommon disorder of childhood in which resolution usually occurs spontaneously or after traction therapy. In a minority of children irreducible or chronic fixation develop, and the natural history then usually involves restriction of head on neck movement, abnormal head position, and progressive facial asymmetry. The conventional management in these cases has been a posterior fusion. METHODS: The authors performed an open reduction via the extreme-lateral approach without adjunctive fixation surgery in 13 children who ranged in age from 4 t

Vertigo in virtual reality with haptics: case report.

A researcher was working with a desktop virtual environment system. The system was displaying vector fields of a cyclonic weather system, and the system incorporated a haptic display of the forces in the cyclonic field. As the subject viewed the rotating cyclone field, they would move a handle "through" the representation of the moving winds and "feel" the forces buffeting the handle as it moved. Stopping after using the system for about 10 min, the user experienced an immediate sensation of postural instability for several minutes. Several hours later, there was the onset of vertigo with head

Carpal tunnel syndrome (CTS) and exposure to vibration, repetitive wrist movements, and heavy manual work: a case-referent study.

Possible connections between carpal tunnel syndrome (CTS) and exposure to vibrating handheld tools, repetitive wrist movements, and heavy manual work were examined in a case-referent study. The cases were 38 men operated on for CTS between 1974 and 1980. For each case, two referents were drawn from among other surgical cases (hospital referents) and two further referents from the population register and telephone directory, respectively (population referents). Thirty four of 38 cases (89%) and 143 of 152 referents (94%) were interviewed by telephone. An increased prevalence of obesity, rheumat
