Donkey hair cortisol concentrations are associated with carrying heavy load and being beaten at work.

BACKGROUND: Cortisol is often considered an indicator of stress. However, it is not a straightforward indicator of stress in animals and is more closely linked to arousal and hypothalamic-pituitary-adrenal axis (HPA) axis activation. To better understand what animals are experiencing, other stressors, e.g., donkeys carrying heavy loads and being beaten, should be considered when interpreting cortisol levels. Our research aimed to assess how carrying mounted loads and working in challenging conditions impact donkey hair cortisol concentrations (HCC). For this purpose, thirty-five working donkey

Economic burden of patients with post-surgical chronic and transient hypoparathyroidism in the United States examined using insurance claims data.

BACKGROUND: Hypoparathyroidism (HP) is a rare endocrine disease commonly caused by the removal or damage of parathyroid glands during surgery and resulting in transient (tHP) or chronic (cHP) disease. cHP is associated with multiple complications and comorbid conditions; however, the economic burden has not been well characterized. The objective of this study was to evaluate the healthcare resource utilization (HCRU) and costs associated with post-surgical cHP, using tHP as a reference. METHODS: This analysis of a US claims database included patients with both an insurance claim for HP and th

Insights into relationship of environmental inequalities and multimorbidity: a population-based study.

BACKGROUND: Substantial inequalities in the overall prevalence and patterns of multimorbidity have been widely reported, but the causal mech … tamination was associated with a 3.92%, 5.17% and 5.54% higher probability, respectively, of having multiple chronic conditions. There was strong evidence that air pollutants, as well as chemical and microbial water contamination, were significantly associated with higher odds of the most common clusters of multimorbidity identified by LCA. CONCLUSION: There is compelling e …

Childhood maltreatment and high dietary fat intake behaviors in adulthood: A birth cohort study.

Childhood maltreatment has been associated with a wide range of chronic medical conditions including obesity, other metabolic events and eating disorders. However, little is known about the association between childhood maltreatment and high dietary fat intake. This study addresses the extent to which co-occurring and specific forms of substantiated childhood maltreatment are associated with self-reported high dietary fat intake in adulthood and whether there is a gender-childhood maltreatment interaction in predicting this association. The study also examines the association between age at su

Myoclonic seizure prior to diagnosis of chronic toxic encephalopathy: a case report.

BACKGROUND: "Thinner" is a widely used industrial mixture of organic solvents. Exposure to organic solvents is usually not considered to be a possible cause of epilepsy, despite descriptions of toxic effects on the central nervous system. There are only a few reports about a possible epileptogenic effect of organic solvents exposure. We report a case of myoclonic seizure at the workplace that shows a remarkable coincidence between exposure to a thinner mixture and the occurrence of an epileptic seizure. CASE PRESENTATION: We present the case of a 50-year-old Belgian woman exposed to organic s

Brain and cardiovascular diseases: common neurogenic background of cardiovascular, metabolic and inflammatory diseases.

In spite of significant progress in pharmacotherapy the incidence of newly diagnosed cases of cardiovascular diseases and cardiovascular morbidity is alarmingly high. Treatment of hypertension or heart failure still remains a serious challenge. Continuous attempts are made to identify the mechanisms that decide about susceptibility to pathogenic factors, and to determine effectiveness of a specific therapeutic approach. Coincidence of cardiovascular diseases with metabolic disorders and obesity has initiated intensive research for their common background. In the recent years increasing attenti

Sympathetic over activity in the etiology of hypertension of obstructive sleep apnea.

Beginning with modest clinical observations in 1984, a picture has evolved suggesting that sympathetic nervous system over activity may be responsible in part for the elevated blood pressure seen in obstructive sleep apnea patients. Early studies of urinary and plasma catecholamines indirectly suggested sympathetic over activity carried to daytime, non-apneic conditions. Later intra-neuronal recordings of muscle sympathetic nerve activity directly demonstrated both acute and diumal (non-apneic) sympathetic over activity. Most importantly, diurnal sympathetic over activity has been shown to dim

[Treatment of generalized anxiety: new pharmacologic approaches].

First defined as a residual diagnostic category in the third edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM), Generalized Anxiety Disorder (GAD) was until recently one of the least studied and least clearly conceptualized of the anxiety disorders. The clinical definition of GAD has however improved up to the fourth edition of the DSM where the disorder is now characterized as a chronic state of apprehensive expectation and uncontrollable worry concerning multiple daily life events or activities and accompanied with at least 3 symptoms belonging to a list of six commo

Scrub typhus complicated by cholecystitis: a case report.

OBJECTIVE: Scrub typhus complicated by acute cholecystitis is rare. Scrub typhus lacks specific clinical manifestations in its early stage, especially in patients without eschars, requiring comprehensive diagnosis based on epidemiological history, clinical presentation, and laboratory results. This case report describes a patient with scrub typhus complicated by acute cholecystitis, ultimately confirmed by tNGS, reminding surgeons to emphasize physical examination and avoid diagnostic anchoring. METHODS: We report a case of a 74-year-old female who presented with right upper quadrant pain and

COVID-19-Related Bilateral Avascular Necrosis of the Femoral Head.

Osteonecrosis is a pathologic process that involves focal bone infarction and death of bone tissue caused by trauma, infections, autoimmune conditions, and chronic steroid use; however, most cases go undiagnosed. The link between bilateral osteonecrosis and coronavirus disease 2019 (COVID-19) infections has not been fully investigated. This is the case of a 42-year-old Caucasian woman who presented to the emergency department for bilateral hip pain, which started three months prior. Initially, the pain was mild; however, her symptoms worsened, causing her to have difficulty ambulating. Co-inci

Anesthetic management of lung transplantation in a patient with end-stage COVID-19 pneumonia: A case report.

RATIONALE: The COVID-19 pandemic is spreading around the world and the leading cause of death is rapidly progressive respiratory failure because of lung damage and consolidation. Lung transplantation is the last line of treatment for chronic end-stage lung diseases. There were several cases of lung transplantation reported in patients with COVID-19 pneumonia. However, anesthetic management of lung transplantation in this subpopulation is rare. We report the anesthetic and perioperative management of lung transplantation in a patient with COVID-19 pneumonia. PATIENT CONCERNS: A 70-year-old man

Challenging Case: Navigating End-of-life in Neuro-inclusive Cancer Care.

Roy is a 13-year-old boy with autism spectrum disorder who has a recurrent glioblastoma. He initially underwent a surgical resection followed by 5 months of intensive systematic chemotherapy that required multiple hospitalizations. Although a clinical trial was considered, Roy was excluded from enrollment because of concerns around his ability to consent and adhere to a complex protocol. Imaging soon revealed significant disease progression and his care plan shifted toward palliative therapies.Roy was the only child of his aging parents. Roy's mother was navigating her own chronic medical cond

[Pulmonary amyloidosis in Sjögren's syndrome: a case report].

Primary Sjögren's syndrome (pSS) is a chronic inflammatory autoimmune disease characterized by lymphocyte proliferation and progressive exocrine gland damage, which can involve multiple organs, including the lungs. However, secondary pulmonary amyloidosis is rare. This study summarizes the clinical features of this condition through the diagnosis and treatment of a 44-year-old female patient and a literature review. The patient was admitted due to "bilateral pulmonary nodules discovered over 4 years ago." Chest CT revealed scattered multiple nodules and mass shadows in both lungs, some with ca

Associations between baseline level and clinical, anthropometric and behavioral changes in adults and the elderly diagnosed with hypertension and/or diabetes: a prospective cohort study.

BACKGROUND: Multimorbidity is defined as the presence of two or more multifactorial diseases or conditions. In this context, the focus of this study is on arterial hypertension (AH) and diabetes, two chronic diseases with high global prevalence and incidence, whose clinical, physical, and behavioral factors must be critically analyzed over the long term, particularly within the primary care setting, where interventions are most effective and impactful. The objective of the study was to prospectively assess baseline levels of clinical, physical, and behavioral parameters and their changes over

Effectiveness and Safety of Ozone Therapy in Humans: An Umbrella Review of Systematic Reviews with Meta-Analyses of Randomized Clinical Trials.

BACKGROUND/OBJECTIVES: Ozone therapy has been proposed across multiple clinical conditions based on hormetic, antioxidant, and immunomodulatory effects, but its efficacy and safety remain controversial. We conducted an umbrella review to appraise the effectiveness and safety of ozone therapy using evidence from meta-analyses of randomized controlled trials (RCTs). METHODS: We searched MEDLINE, Web of Science, Embase, and the Cochrane Library from inception to 14 February 2025, with an updated search performed on 9 May 2026. Eligible studies were systematic reviews with meta-analyses comparing

Clinical studies on the effect of type 1 diabetes on oral health in children and adolescents aged 6-17 years. A literature review.

BACKGROUND AND AIMS: Type 1 diabetes mellitus is a chronic metabolic disease of autoimmune origin with early manifestations, which appear predominantly in childhood. Its incidence has increased in most European countries mainly due to refined diets and increased consumption of sweets. Diabetes is known as a predisposing factor for the development of oral diseases; therefore, prevention at an early age is essential. Genetic factors and environmental factors contribute to the risk of disease, indicating a multifactorial etiology. Along with chronic periodontitis and other oral manifestations, su

PoWeR elicits intracellular signaling, mitochondrial adaptations, and hypertrophy in multiple muscles consistent with endurance and resistance exercise training.

Physical activity guidelines recommend both endurance and resistance exercise to improve and maintain overall health. Recently, progressive weighted wheel running (PoWeR), a voluntary, progressive, and high-volume exercise paradigm, was posited as a singular prototype of combined endurance and resistance exercise in mice as evident by enhanced capillarization and hypertrophy of select plantar flexor muscles. Despite growing interest in this model, it remains incompletely characterized if PoWeR resembles the acute and chronic responses to resistance and/or endurance exercise in humans. Therefor
