[Opportunistic infections of the central nervous system in patients with AIDS].

Between August 1985 and March 1989 we had evaluated 70 AIDS patients. We had identified 7 opportunistic Central Nervous System infections, others toxoplasmosis and tuberculosis: A case of progressive multifocal leukoencephalopathy presented with aphasia as their initial manifestation, a probable herpesvirus type 1 polyradiculoneuropathy, four cryptococcal meningitis and syphilitic neuroretinitis.

Hepatitis in vervet monkeys caused by Fusarium moniliforme.

The fungus Fusarium moniliforme Sheldon is a common contaminant of maize (Zea mays L.) intended for human and animal consumption throughout the world. Culture material of F. moniliforme MRC 826, isolated from home-grown maize in an area in Transkei, southern Africa, with a high rate of human oesophageal cancer, was highly toxic to vervet monkeys (Cercopithecus pygerythrus). Ten monkeys were fed a standard primate diet which contained various amounts of culture material for 180 days. Two control monkeys received the standard diet without culture material. Pathological changes observed in liver

Norfloxacin versus vancomycin/polymyxin for prevention of infections in granulocytopenic patients.

Selective antimicrobial decontamination with norfloxacin was compared with vancomycin/polymyxin for prophylaxis of bacterial infections in granulocytopenic patients. In the group of patients receiving norfloxacin, there were a lower number of acquired gram-negative bacillary organisms per patient (0.88 versus 1.86, p = 0.002), fewer patients with documented infection (16 of 36 versus 20 of 30, p = 0.12), and fewer cases of gram-negative septicemia (0 of 36 versus five of 30, p = 0.02). Norfloxacin was better tolerated (30 of 36 versus 16 of 30 patients highly compliant, p = 0.02), and associat

Pathogens in children with severe combined immune deficiency disease or AIDS.

We evaluated the frequency and severity of illnesses caused by various microbial pathogens in 15 children with severe combined immune deficiency disease (SCID) and 8 with acquired immune deficiency syndrome (AIDS). There were 35 viral, 23 bacterial, 19 mycotic and 13 parasitic infections. Nineteen of the 23 patients died of infection; Pneumocystis carinii pneumonia, giant-cell pneumonia due to paramyxoviruses and various disseminated viral infections were responsible for most deaths in both groups. The emerging role of paramyxoviruses was illustrated by the fact that they were responsible for

C-reactive protein for detection and follow-up of bacterial and fungal infections in severely neutropenic patients with acute leukaemia.

To evaluate the aetiology of febrile episodes and to rationalize our politics with antibiotics, C-reactive protein (CRP) was determined immunoturbidimetrically in 20 consecutive neutropenic adults with acute leukemia. They had 35 febrile episodes, 89% of which were infectious. Twenty per cent of infections were fungal. A similar CRP response was seen both in bacterial and in fungal infections. In 84% of infections the peak value for CRP rose greater than 100 mg/l. Thirty-five apyrexial patients with acute leukaemic and 20 healthy adults served as controls. Their CRP was less than 10 mg/l in 87

Surgical treatment for fungal infections in the central nervous system.

The hospital records of 78 patients who underwent surgical therapy for fungal infections of the central nervous system (CNS) between 1964 and 1984 are summarized. Nine different fungal types were identified, but Coccidioides immitis and Cryptococcus neoformans accounted for most (67.1%) of the infections. A variety of clinical syndromes were seen, including chronic basal meningitis (45 patients), intracranial mass lesions (12 patients), and communicating hydrocephalus (six patients). Thirteen patients had rhinocerebral forms of fungal infection, and two presented with spinal involvement. Delay

Neutropenia, fever, and infection.

With the advances in the management of various neoplastic diseases and subsequent improvement in "disease-free" states, complications of therapy--particularly, infectious complications--have evolved as stumbling blocks to survival. Among neutropenic (absolute neutrophil count below 1,000/mm3) patients with cancer, infection is the major autopsy-determined cause of death. With expected "cure rates" of childhood leukemia approaching 60 to 70 percent, it seems unreasonable to lose such patients to an infectious cause of death, yet this, indeed, happens. The purpose of this review is to (1) define

Infective endocarditis at autopsy in northern India. A study of 120 cases.

Pathologic data on 120 autopsied cases of infective endocarditis are presented. They constitute 1.8% of total 6,700 and 6.3% of 1,900 cardiac autopsies over 16 years. Ninety % of patients were below the age of 40 years, 42.5% had no pre-existing heart disease, 33.3% had previous valvular disease, mainly rheumatic, and 24.2% had congenital heart disease. Mitral and aortic valves were each the sites in one third cases, mitral slightly more than aortic, twenty-six point six % of the cases had lesions on the right side of the heart, 16.6% exclusively so. Cardiac complications were infrequent while

Infections in patients with Hodgkin's disease: a clinical study of 300 consecutive adult patients.

Patterns of infection were investigated in 300 consecutive adult patients with Hodgkin's disease who were followed for an average of 5.7 years after diagnosis. Serious infection other than cutaneous herpes zoster infection developed in 21% of the 300 patients. Sixty percent of the episodes of serious infection were microbiologically documented (MDSI). Bacteremia was the most frequent MDSI, and Streptococcus pneumoniae was the most common organism causing MDSI. Bacteremia due to S. pneumoniae occurred in 3% of the 300 patients, was rarely fatal, and almost always was associated with prior relap

Fungus infections after liver transplantation.

The problem of fungus infections after liver transplantation was studied. In 100 consecutive recipients of orthotopic liver homografts there were 10 and 8 examples, respectively, of localized and disseminated infections caused by Candida species. Candidemia was demonstrated in 8 of these 18 patients. One patient who had a localized Candida infection also had disseminated cryptococcosis. An additional 31 patients were infested in that Candida could be cultured from sites where it is not normally found, such as the blood (8 examples), urine (8), ascitic fluid (8), and wounds (22). This exorbitan

Recurrent keratitis due to Acremonium potronii.

In a 15-year-old boy a culture-proved keratitis after a corneal perforation healed without antifungal agents after corneal suturing and application of tissue glue. Eight months later a posterior corneal abscess developed. Diagnostic and therapeutic penetrating keratoplasty was performed when the lesion failed to respond to pimaricin. Cultures were positive for Acremonium potronii, the same fungus isolated from the original corneal laceration eight months previously. To our acknowledge, this is the first case report of a central corneal ulcer or abscess due to this specific organism.
