The roles of folate, MTHFR genetics, vitamin B12 in pregnancy outcomes.

Folate is essential for fetal development, and periconceptional folic acid (FA) supplementation is well-established for preventing neural tube defects. However, evidence regarding its role in other pregnancy outcomes, such as gestational diabetes mellitus, hypertensive disorders of pregnancy, fetal growth, miscarriage, and preterm birth, remains inconsistent. Current knowledge indicates that the effects of FA are not uniform but significantly influenced by the timing, dose, and duration of supplementation, frequently exhibiting U-shaped or timing-dependent relationships. Furthermore, methylene

Editorial: Improving Evidence-Based Mental Health Care for Refugee and Internally Displaced Children: What Works and Why?

There has been a rapid increase in the number of displaced children over the past decades due to armed conflicts, violence, and natural disasters. In 2024 alone, the United Nations Children's Fund (UNICEF) estimated that 53.2 million children were forced to flee their homes, which is about triple the estimate of 2010.1 These children experience a multitude of stressors both before and during their displacement, including poor living conditions and serious risk of emotional, sexual, and physical abuse. Unsurprisingly, sustained exposure to such stressors places these children at heightened risk

Executive Summary of Evidence-Based Guidelines for the Diagnosis and Treatment of Pediatric CKD-Mineral and Bone Disorder (Version 2024).

Chronic Kidney Disease-Mineral and Bone Disorder (CKD-MBD) in children refers to the systemic mineral and bone metabolism disorders caused by CKD, including biochemical abnormalities, abnormalities in bone turnover, mineralization, quality, and ectopic calcification. Like adult patients, mineral metabolism and bone structure abnormalities are universally present in children with CKD. However, there have been no guidelines specifically for children in the past. The medication treatment and clinical practices for children are usually based on those for adults, thus lacking targeted guidance. Chi

Diet, gut microbiota, and the gut-brain axis: mechanistic interactions and therapeutic implications in neuropsychiatric disorders.

The gut microbiota is a dynamic trans-kingdom ecosystem that contributes to host immunological, metabolic, and neuroendocrine homeostasis through the microbiota-gut-brain axis (MGBA). Diet is one of the major environmental factors shaping this axis, as it influences microbial composition, microbial production of neuroactive metabolites, and intestinal barrier integrity. Dysbiosis has been increasingly associated with neurological, psychiatric, and neurodevelopmental disorders, including Alzheimer's disease, Parkinson's disease, depression, autism spectrum disorder, and attention-deficit/hypera

Association Between Alpha-1-Acid Glycoprotein and Non-Alcoholic Fatty Liver Disease and Liver Fibrosis in Adult Women.

Background: Alpha-1-acid glycoprotein (AGP) is a glycoprotein synthesized mainly by the liver. Nonalcoholic fatty liver disease (NAFLD) and liver fibrosis (LF) are associated with metabolic disorders. The aim of this study was to examine the potential correlation between AGP and both NAFLD and LF. Methods: The data were derived from the 2017-2023 National Health and Nutrition Examination Survey (NHANES). The linear association between AGP and NAFLD and LF was examined by multivariate logistic regression models. Non-linear relationships were described by fitting smoothed curves and threshold ef

Genetic predeterminants and recent advancements in steatotic liver disease: A roadmap toward precision hepatology.

Steatotic liver disease (SLD) encompasses a group of disorders characterized by the excessive accumulation of fat in the liver. It is classified into four categories based on clinical manifestations: Metabolic dysfunction-associated SLD (MASLD), metabolic-alcohol-associated liver disease (ALD), ALD, and cryptogenic SLD. In the United States, its prevalence stands at 34.2%, making it the most common cause of cirrhosis and hepatocellular carcinoma (HCC). In addition to factors related to endocrine, nutrition, and medications, several genetic markers have been implicated in the disease's pathogen

Association between weekly sleep duration and gallstones in a US-based adult sample: A cross-sectional study.

Sleep disorders and gallstone disease represent significant public health concerns, yet their potential association remains poorly understood. In particular, the association between weekly sleep duration and gallstone risk in adults has not been thoroughly investigated. Data from the National Health and Nutrition Examination Survey (NHANES, 2021-2023) were analyzed in a cross-sectional study examining weekly sleep duration and gallstone risk. Advanced multivariate regression models were employed to comprehensively analyze and adjust for potential confounders. To ensure robustness, smooth curve

Abstinence-based treatment of comorbid eating disorders and ultra-processed food addiction.

While not officially recognized as a clinical diagnosis, ultra-processed food addiction (UPFA) is an increasingly observed phenomenon that frequently co-occurs with eating disorders (EDs). Yet, treatment remains both understudied and controversial. Many challenges exist when treating patients with comorbid UPFA/ED, particularly in the context of the polarizing debate between abstinence-based and moderation-based approaches to nutrition intervention. We present three vignettes illustrating diverse trajectories of recovery when an abstinence-based approach is explored by a patient presenting wit

Omega-3 fatty acids: multi-target mechanisms and therapeutic applications in neurodevelopmental disorders and epilepsy.

Neurodevelopmental disorders (NDDs), including attention deficit hyperactivity disorder (ADHD), autism spectrum disorder (ASD), and Tourette's syndrome (TS), impair brain development and function, primarily affecting cognition, behavior, and social skills in children. Epilepsy, characterized by recurrent seizures due to neuronal hyperexcitability, shares pathological mechanisms with NDDs, such as neuroinflammation, synaptic dysfunction, and oxidative stress. Omega-3 fatty acids-primarily docosahexaenoic acid (DHA) and eicosapentaenoic acid (EPA)-exert neuroprotective and neuromodulatory effect

The proteome of human adult whipworm Trichuris trichiura: A source of potential immunomodulatory molecules.

Soil-transmitted helminths (STHs), including Trichuris trichiura, pose a major global health challenge, particularly in children, causing significant morbidity. However, T. trichiura's ability to modulate host immune responses offers a unique opportunity to discover biomolecules with therapeutic potential for inflammatory, allergic, and autoimmune disorders. This study conducted a proteomic analysis of adult male and female T. trichiura using liquid chromatography-tandem mass spectrometry (LC-MS/MS), identifying 810 parasite proteins. Of these, 177 were exclusive to females, 277 to males, and

Crosstalk between adipogenesis and aging: role of polyphenols in combating adipogenic-associated aging.

In the last forty years, the number of people over 60 years of age has increased significantly owing to better nutrition and lower rates of infectious diseases in developing countries. Aging significantly impacts adipose tissue, which plays crucial role in hormone regulation and energy storage. This can lead to imbalances in glucose, and overall energy homeostasis within the body. Aging is irreversible phenomena and potentially causing lipid infiltration in other organs, leading to systemic inflammation, metabolic disorders. This review investigates various pathways contributing to aging-relat

Recent Insights and Recommendations for Preventing Excessive Gestational Weight Gain.

Recommendations for weight gain during pregnancy are based on pre-pregnancy body mass index (BMI). Pregnancy is a risk factor for excessive weight gain and many endocrine problems, making it difficult to return to pre-pregnancy weight and increasing the risk of postpartum obesity and, consequently, type 2 diabetes and metabolic syndrome. Both excessive gestational weight gain (EGWG) and obesity are associated with an increased risk of gestational hypertension, pre-eclampsia, gestational diabetes, cesarean section, shoulder dystocia, and neonatal macrosomia. In the long term, EGWG is associated

Italian Guidelines for the diagnosis and treatment of Fetal Alcohol Spectrum Disorders: epidemiology.

Fetal alcohol spectrum disorders (FASD) are a significant global challenge characterized by complex diagnosis and research. The diagnostic process is complicated due to overlapping symptoms with other conditions, as well as factors such as maternal nutrition, socioeconomic status, and mental health, which can affect the severity of FASD traits differently in individuals. Risky drinking behaviors are prevalent in young adults, especially those aged 20-24, which coincides with high rates of unplanned pregnancies, increasing the risk of FASD. Specific subpopulations, such as children in care faci

The human placenta: new perspectives on its formation and function during early pregnancy.

The placenta has evolved to support the development of the embryo and fetus during the different intrauterine periods of life. By necessity, its development must precede that of the embryo. There is now evidence that during embryogenesis and organogenesis, the development of the human placenta is supported by histotrophic nutrition secreted from endometrial glands rather than maternal blood. These secretions provide a plentiful supply of glucose, lipids, glycoproteins and growth factors that stimulate rapid proliferation and differentiation of the villous trophoblast. Furthermore, evidence fro

Progressive osseous heteroplasia: A case report with an unexpected trigger.

Progressive osseous heteroplasia (POH) is a rare genetic disorder characterised by progressive heterotopic ossification (HO) within the skin and subcutaneous tissues. The condition is caused by heterozygous inactivating mutations of the GNAS gene and usually presents in infancy. We describe the case of a white male ex-preterm who was first referred because of subcutaneous calcium deposits along the right arm after extravasation of parenteral nutrition. As these lesions progressed, a skin biopsy was undertaken which revealed intramembranous ossification. Genetic testing revealed a constitutiona
