Alexithymia and depression in patients with fibromyalgia: When the whole is greater than the sum of its parts.

This study investigated the link between alexithymia and depressive symptoms in Fibromyalgia (FM). 181 FM women and 181 healthy controls (HC) were compared using the Hospital Anxiety and Depression Scale and the Toronto Alexithymia Scale. A moderation analysis was performed to examine the moderation effect of the group (FM vs. HC) on the relationship between alexithymia and depression. Group was a significant moderator, highlighting a stronger relationship between alexithymia and depressive symptoms in the FM compared to HC. The study highlighted that the association between alexithymia and de

[Application of laser CO2 for the treatment of xanthelasma palpebrarum].

METHOD: We treated 12 patients with bilateral xanthelasmas using an ultra-pulse CO2 laser. They were all extirpated, with no differences between the upper and lower eyelids. There were no cases of ectropion or bleeding. All the patients expressed a high level of satisfaction. DISCUSSION: The use of the laser CO2 is a good therapeutic option for treating xanthelasmas especially those located in the lower eyelid, and in young people. It is a safe treatment for delicate periorbital regions, with a low risk of visible scars, and fewer recurrences. The technique can be performed outside the operat

Application of electroconvulsive therapy in treatment of retinitis pigmentosa comorbid with major depressive disorder and panic disorder.

A substantial proportion of retinitis pigmentosa (RP) patients have depression/anxiety and a phobic pathology that may be related to changes in melatonin secretion. We discuss electroconvulsive therapy (ECT) in a patient with RP comorbid with depression and panic disorder. A 51-year-old man was admitted because of major depression, panic disorder, and RP. Ultrabrief pulse (0.3 millisecond) right unilateral ECT was performed 9 times in total. The symptoms relieved, and patient tolerated the treatments well. Electroconvulsive therapy increases serum melatonin, providing therapeutic effects in de

[Trichotillomania: three cases presentation and diagnosis tests review].

Trichotillomania represents a chronic disorder in which patients traumatically remove their own hair in a bizarre pattern. Like obsessive-compulsive disorder, the hair-pulling behavior is recognized as senseless and undesirable, but is performed in response to several emotions and affects, such as increasing anxiety, or unconscious conflicts with a resultant tension relief. The condition may be episodic, but is usually chronic and difficult to treat. We present three cases of trichotillomania and review common clinical and pathologic findings of this, often chronic and socially debilitating, d

Generalized Social Phobia and social judgments: the salience of self- and partner-information.

Individuals with Generalized Social Phobia (GSP, N = 25) and community controls (N = 26) participated in a social interaction task. They then rated how well they had performed in the situation and assigned weights and valences to the different types of information they used to arrive at this judgment. The GSP group weighed anxiety-related self-information more heavily than did the Control group. In contrast, their conversational partners weighed verbal behavior more heavily. These findings are generally supportive of cognitive models of social phobia and point to the negative interpersonal imp

Mitochondrial dysfunction in the spinal cord contributes to peripheral hypersensitivity in a nitroglycerin-induced migraine model.

BACKGROUND: Mitochondrial dysfunction was reported as a potential factor that contributes to the development of migraine and peripheral hypersensitivity. The recent accumulation of knowledge on mitochondrial involvement in migraine paved the way for a new non-pharmacological-therapeutic strategy like hyperbaric oxygen therapy (HBOT), which targets mitochondrial dysfunction. Yet, the efficacy of HBOT in modaulting migraine associated pain remains unclear. METHODS: Rats were injected with nitroglycerin (NTG, 10 mg/kg), a human migraine trigger, every other day for 9 days. Mechanical and thermal

Pheochromocytoma: A Case Report.

Pheochromocytomas are rare tumors located in the adrenal medulla, that derives from the chromaffin cells and produce catecholamines. They are an uncommon cause of hypertension, and only 50% of the patients present symptoms compatible with this pathology. Here we describe the case of a 70-year-old woman with a history of anxiety, hypertension and palpitation, who had an unspecified nodule in the right adrenal gland. Laboratory studies revealed an elevated urinary metanephrines secretion. A diagnosis of pheochromocytoma was made and an adrenalectomy was performed. Our aim is to highlight the dia

Resuscitating the Crashing Pregnant Patient.

Care of the critically ill pregnant patient is anxiety-provoking for those unprepared, as the emergency physician must consider not only the welfare of the immediate patient, but of the fetus as well. Familiarity with the physiologic changes of pregnancy and how they affect clinical presentation and management is key. Although some medications may be safer in pregnancy than others, stabilizing the pregnant patient is paramount. Emergency physicians should target pregnancy-specific oxygen and ventilation goals and hemodynamics and should be prepared to perform a perimortem cesarean section, sho

The detection of mental disorders in the community using a 20-item interview version of the General Health Questionnaire.

A 20-item interview version of the General Health Questionnaire (GHQ) was used to presumptively identify cases with mental disorders in a sample of 810 community residents. Subjects also were examined by psychiatrists using the Present State Examination (PSE). The screening instrument performed well in detecting PSE-CATEGO syndromes related to anxiety and depression, but less well in detecting psychotic syndromes and cognitive impairment. The overall sensitivity of the instrument for any PSE-CATEGO syndrome was 48%. The study elucidates the specific nature of the psychopathology that this vers

The clinical management of repeated early pregnancy wastage.

A rational systematic evaluation is essential to the management of a couple with repeated early pregnancy wastage. Psychologic support in the form of frequent discussions and sympathetic counseling are crucial to the successful evaluation and treatment of the anxious couple. A prompt and orderly evaluation will relieve anxiety. When no etiologic factor is identified, a 60% to 80% fetal salvage rate may be expected. Once a patient conceives, serial ultrasonography, beta-hCG determination, and estradiol determination may be useful in detecting the stage of the embryonic death if subsequent abort

Behavior of children undergoing hypospadias repair.

We performed a prospective study on 10 boys between 2 and 6 years old undergoing repair for hypospadias. Clinical observation in the hospital ward rooms and play environment demonstrated a distinctive postoperative course of withdrawal and aggressive behavior with resultant increased parental anxiety, which results in strained staff-family relations. We speculate that this distinctive behavior may be owing to an interaction among parental attitudes, the pain and immobilization from operation and the level of the child's sexual development. Intervention was designed to give the boys behavioral

Evaluation of psychosocial benefits of pulmonary rehabilitation in chronic obstructive pulmonary disease patients.

Chronic obstructive pulmonary disease (COPD) is associated with significant symptom burden, impaired quality of life, and psychosocial morbidity. Pulmonary rehabilitation (PR) is a recommended non-pharmacological intervention, but evidence focusing on its psychosocial benefits remains limited. The objective of this study was to assess the effect of hospital-based outpatient PR on psychosocial parameters and health-related quality of life in patients with COPD receiving standard pharmacotherapy. This before-after interventional study included 50 male patients with stable COPD who underwent a st
