BACKGROUND: Posttraumatic endophthalmitis is a rare complication of penetrating eye injuries that results in blindness in potentially salvageable eyes.
METHODS: 36 cases of posttraumatic endophthalmitis were evaluated retrospectively to determine which factors were associated with visual loss. All cases were treated with intravenous and intraocular antibiotics. Pars plana vitrectomy was performed in 15 (41.7%) cases as an adjunct to treatment.

BACKGROUND: Intravitreal administration of ganciclovir has been used to treat cytomegalovirus (CMV) retinitis in patients with acquired immune deficiency syndrome (AIDS) who are unable to tolerate systemic antiviral therapy. Although retinal toxicity from intravitreal ganciclovir has been observed in animal experiments, to our knowledge, retinal toxicity from intravitreal ganciclovir has not been reported in humans.

A group of 25 active, asymptomatic, amateur boxers were examined to evaluate the nature and incidence of ocular pathologic conditions in amateur boxing. An approximately age-matched group of 25 men who were not boxers was used as a control group. The number of fights varied from 1 to 220 (mean 39.4), and the number of spar rounds from 20 to 600 (mean 192.5). Visual acuity was 20/20 in all subjects of both groups, except for one boxer with keratoconus. Intraocular pressure was below 20 mmHg in all eyes. A 48-year-old woman with a previous a liver transplant and a cataract extraction developed a necrotizing retinochoroiditis in her right eye. The base values of electrooculograms (EOGs) of 39 patients with new, unilateral rhegmatogenous retinal detachment and clear media were studied. None of the patients had a history of ocular surgery. Ophthalmoscopic examinations revealed choroidal detachment in 11 eyes. The clinical and histologic features of an intraocular parasite from a 29-year-old woman are reported. The live organism was located in the posterior vitreous near the optic disc and was successfully removed by vitreous aspiration. The parasite was identified as an advanced stage larvae of Gnathostoma spinigerum. This is the first case report of the scanning electron microscopic findings of intraocular gnathostomiasis. The features of nine previously reported cases of intraocular gnathostomiasis are reviewed.

The clinical and pathologic features of an unusual retinal glioma that was the only clinically overt manifestation of tuberous sclerosis in a 27-year-old woman are reported. The tumor was composed predominantly of large pleomorphic cells with glassy eosinophilic cytoplasm. The results of vitrectomy in 240 consecutive cases of ocular trauma were reviewed. Of these cases, 71.2% were war injuries. Intraocular foreign bodies were present in 155 eyes, of which 74.8% were metallic and 61.9% ferromagnetic. Multivariate analysis identified the prognostic factors predictive of poor visual outcome, which included: (1) presence of an afferent pupillary defect; (2) double perforating injuries; and (3) presence of intraocular foreign bodies. At CPMC routine mammographic screening was introduced in late 1975. The total volume of breast cancers, other than outside consultations, seen in the department increased from 71 in 1975 to 164 by 1988, an increase of 230%. This expansion in volume was due largely to surgical removal of mammographically detected occult, in situ duct and relatively small invasive duct carcinomas. The clinical features of an infective embolic choroidopathy are described, from its early onset to late complications in a 45-year-old man with acute Staphylococcus aureus endocarditis of the aortic valve.
