[Nocturnal long-term monitoring of lung sounds in patients with gastro-oesophageal reflux disease].

Gastro-oesophageal reflux disease (GERD) is one of the most common clinical conditions in the developed countries. Particular interest in pulmonary manifestations of this disease has arisen over the last few years. Although the high coincidence between reflux and chronic cough is unquestioned, the proof of a causal correlation is still lacking. In this paper we present the Marburger Lung-Sound-Monitoring as a new method for the detection of nocturnal respiratory symptoms such as cough, wheezing and throat clearing and their temporal correlation with reflux. This method will in future allow us

Polysomnographic features in infants with early diagnosis of congenital hypothyroidism.

Thyroid hormones play a major role in the maturation process of the brain. Currently, congenital hypothyroidism is detected by mass screening. The impact of this early hormonal deficiency on the organization of the sleep pattern is not known. In this study, the polysomnographic features in children diagnosed with congenital hypothyroidism were analyzed. Children were detected by mass population screening and the hormonal replacement therapy starts immediately. Children's age ranged between 1.5 and 18 months of age. The duration of hormonal treatment before sleep recordings varied between 8 day

Severity of obstructive sleep apnoea/hypopnoea syndrome and subsequent waking EEG spectral power.

It is well known that most patients with obstructive sleep apnoea/hypopnoea syndrome (OSAHS) suffer sleepiness, although the underlying mechanisms of this relationship remain unclear. The present study examined the relationship between nocturnal variables and the subsequent waking electroencephalogram (EEG), in order to determine if sleepiness was related to OSAHS severity and due to sleep fragmentation or to nocturnal hypoxaemia. In total, 12 moderate-to-severe OSAHS patients underwent a total sleep night followed by a 24-h period of sustained wakefulness where the waking EEG was measured eve

Obstructive sleep apnea in older adults.

The "typical" presentation of obstructive sleep apnea (OSA) is chronic loud snoring and excessive daytime sleepiness in middle-aged obese men. OSA can result in increased risk for cardiovascular morbidity and mortality. The diagnostic features of OSA in older adults are similar to those in younger adults; however, the older adult may be less likely to seek medical attention or have the sleep disorder recognized because symptoms of snoring, sleepiness, fatigue, nocturia, unintentional napping, and cognitive dysfunction may be ascribed to the aging process itself or to other disorders. This arti

Oculoauriculovertebral complex with an atypical cause of obstructive sleep apnea.

Oculoauriculovertebral spectrum (OAVS) is a birth defect of unknown etiology, often causing obstructive sleep apnea, due to unilateral retrognathia. We describe an adolescent sleep apnea patient, with usual and unusual signs of OAVS. Apart from mandibular hypoplasia, microtia, external auditory canal atresia and cervical vertebrae anomalies, skull base asymmetry was also noted, resulting in aberrant anatomy of the tympanic cavity, and nasopharyngeal obstruction, which was the main source of the patient's apneas. The extended craniofacial abnormalities manifested here, suggest a broader develop

Noninvasive ventilation in children with spinal muscular atrophy types 1 and 2.

OBJECTIVE: Our aim was to assess the efficacy of noninvasive ventilation (NIV) for the treatment of thoracoabdominal asynchrony during sleep in children with spinal muscular atrophy (SMA) types 1 and 2. DESIGN: Nine subjects underwent assessment for sleep apnea/hypopnea index (AHI), mean oxyhemoglobin saturation (SpO2), oxygen desaturation index, transcutaneous carbon dioxide tension (tcpCO2), and mean phase angle during sleep as a measure of thoracoabdominal coordination. A second sleep study was performed with use of NIV. RESULTS: The nine patients (7 mos of age, range 2-33) had a baseline

Relationship between cardiac arrhythmias and sleep apnoea in permanently paced patients with type I myotonic dystrophy.

The long-term relationship between cardiac arrhythmias and sleep apnoea in myotonic dystrophy (DM1) is unknown. Pacemakers enabling the long-term monitoring of electrocardiographic and ventilation parameters were implanted in 20 patients with DM1 (mean age = 42+/-11.6 years), followed 40+/-12.3 months. Arrhythmias were recorded by the pacemaker in 17 patients (85%): 14 developed arrhythmic episodes that occurred either in absence or in presence of concomitant sleep apnoea. Conversely, among these 14 patients, the majority of sleep apnoea episodes were not associated with concomitant arrhythmia

Daytime sleepiness and polysomnographic variables in sleep apnoea patients.

Excessive daytime sleepiness (EDS) is not invariably present in patients with obstructive sleep apnoea syndrome (OSAS). The aim of the present study was to investigate polysomnographic determinants of EDS in patients with OSAS. EDS was assessed using the Epworth Sleepiness Scale (ESS) and the multiple sleep latency test (MSLT). Patients showed EDS whenever the ESS score was >10 and the MSLT score <5 min. Absence of EDS was defined as having an ESS score of <10 and an MSLT score of >10 min. In total, 23 male patients with EDS (mean+/-sd ESS and MSLT score 17+/-3 and 4+/-1 min, respectively) and

Prostaglandin D synthase (beta trace) levels in sleep apnea patients with and without sleepiness.

BACKGROUND: Excessive daytime sleepiness (EDS) occurs often in patients with obstructive sleep apnea syndrome (OSAS). However, not all patients present EDS. We hypothesized that the prostaglandin D2 system (PGD2) may be involved in the pathogenesis of EDS associated with OSAS. METHODS: We measured the levels of lipocalin-type PGD synthase (L-PGDS), the enzyme that produces PGD2, in the serum of 47 patients with OSAS (26 with and 21 without EDS) and 18 healthy controls. RESULTS: Patients with EDS had higher levels of L-PGDS (0.73+/-0.06 mg/L) than patients without EDS (0.58+/-0.03 mg/L, p<0.0

[The role of functional nasal surgery in the treatment of obstructive sleep apnea syndrome].

INTRODUCTION: Nasal obstruction is widely mentioned among the factors influencing pathophysiology of obstructed breathing disorders during sleep. However numerous observations confirms such relation, influence of nasal surgery on frequency and intensity of obstructive episodes during sleep remains modest and unpredictable in vast majority of cases. The aim of this study was to evaluate the role of functional nasal surgery in the treatment of Obstructive Sleep Apnea Syndrome (OSAS). MATERIAL AND METHODS: The study included 83 subjects with OSAS diagnosed by polysomnography and elevated nasal r…

Awareness of sleepiness and ability to predict sleep onset: can drivers avoid falling asleep at the wheel?

OBJECTIVES: Regarding the causes of sleep-related accidents, this study assesses whether individuals can anticipate sleep onset accurately and how individuals acknowledge and use physiological and cognitive cues to make judgments related to sleep onset. METHODS: A group of 41 partially sleep-deprived subjects predicted the likelihood of sleep in 30 consecutive two-minute intervals and noted physiological and cognitive signs of sleepiness, including involuntary eye closure, head-nodding, wandering thoughts, yawns, and instances of sleep, collectively referred to as "sleep complaints". Surgical orthognathic management of sleep apnea.

Management of obstructive sleep apnea syndrome (OSAS) requires a clear understanding of both the medical and surgical components of the treatment and interpretation of the subjective and objective data such as the polysomnagram. Medical management is primarily by continuous positive airway pressure (CPAP) and is the first line of treatment. Surgical management consists of several phases based on the severity and location of the anatomical abnormalities. The last phase of surgical management consists of maxillo-mandibular advancement by a Le Fort I osteotomy and bilateral sagittal split ramus o

Topographic differences in mean computational sleep depth between healthy controls and obstructive sleep apnoea patients.

In this work, topographic differences in computational sleep depth between healthy controls and obstructive sleep apnoea syndrome (OSAS) patients have been examined. Sleep depth estimation was based on continuous monitoring of the mean frequency of the EEG. During the experiments, all-night sleep EEG recordings of carefully age and gender matched sets of 16 healthy controls and 16 OSAS patients were compared on six electrode locations (Fp1-M2, Fp2-M1, C3-M2, C4-M1, O1-M2, and O2-M1). To optimise the diagnostic ability of the method, we examined the influence of 45 sets of adjustable analysis p

Sleep architecture in homicidal women with antisocial personality disorder--a preliminary study.

The aim of the present study was to characterize sleep in severely violent women with antisocial personality disorder (ASP) as the primary diagnosis. Participants for this preliminary study were three drug-free female offenders ordered to undergo a forensic mental examination in a maximum security state mental hospital after committing homicide or attempted homicide. Ten healthy age- and gender-matched controls consisted of hospital staff with no history of physical violence. The most striking finding was the increased amount of slow wave sleep, particularly the deepest sleep stage, S4, in wom

Effect of continuous positive airway pressure on ventricular ectopy in heart failure patients with obstructive sleep apnoea.

BACKGROUND: Obstructive sleep apnoea (OSA) elicits a number of cardiovascular perturbations that could lead acutely or chronically to increased ventricular ectopy in patients with heart failure (HF). We tested the hypothesis that treatment of OSA with continuous positive airway pressure (CPAP) in patients with HF would reduce the frequency of ventricular premature beats (VPBs) during sleep in association with reduced sympathetic nervous system activity. METHODS: Following optimisation of medical treatment, 18 HF patients with OSA and >10 VPBs per hour of sleep were randomised to a control gro
