Technical communication: percutaneous radiofrequency mandibular nerve rhizotomy guided by high-speed real-time computed tomography fluoroscopy.

We present a new method of percutaneous radiofrequency mandibular nerve rhizotomy for pain relief in the mandibular region, in which needle placement is guided by high-speed real-time computed tomography (CT) fluoroscopy. Eleven patients (13 procedures) with idiopathic trigeminal neuralgia underwent the procedure. CT fluoroscopy simultaneously provided 3 slices (1-mm interval series, craniocaudally) in 1 fluoroscopic view, allowing for accurate needle placement. Trigeminal neuralgia improved in all patients without severe complications. The mean numerical rating scales of pain intensity (+ or

Very distal radial osteotomy for Madelung's deformity.

Madelung's deformity results from arrested or slowed growth of the volar-palmar portion of the distal radial epiphysis. The deformity is characterized by increased volar and ulnar tilt of the articular surface of the radius and a bow of the distal radius in a palmar direction. When Madelung's deformity produces pain and interferes with function, correction of the deformity of the radius and secondary overgrowth of the ulna is desirable. Although osteotomy of the radius near the junction of the middle and distal thirds allows correction of the bow of the radius and limited correction of palmar

Risk, benefits and complications of epidural steroid injections: a case report.

Acute low back pain, radiculopathy, and associated disabilities have a prevalence of 2% in the United States, which represents a significant health problem and a major cause of workplace absence. The condition primarily affects 25- to 45-year-old men and women. Epidural steroid injections (ESIs) are commonly administered to relieve this pain and improve mobility without surgery. This case report presents issues related to the treatment of an epidural hematoma in an 87-year-old man receiving long-term anticoagulant therapy. It reviews the risks, benefits, side effects, complications, and contra

Assessment of women's satisfaction with medical termination of pregnancy.

This was a prospective study involving 205 women seeking medical abortion with gestational age<or=63 days. Questionnaires regarding expectations, experience and satisfaction with medical abortion were self-completed at the end of the procedure. Women's satisfaction was assessed using the visual analogue scale from 0 to 10. Satisfaction and acceptability parameters were correlated with socio-demographic and obstetric data. The mean degree of overall satisfaction was 8.3 (range 1-10). Around 90% of women would select the method again and would recommend it to a friend or relative. The degree of

[Surgical treatment of chronic pancreatitis].

The surgical treatment of chronic pancreatitis has benefited from the progress in imaging, with subsequent improvement of preoperative investigations but the main factor has been the development of surgical procedures. The pancreatectomy with the preservation of the pylorus or duodenum completes the armamentarium of by-pass and resective techniques. The classical surgical indications are the local evolutive complications: obstruction of pancreatic duct, common bile duct stenosis, duodenal stenosis, splenic complications and localized extra-hepatic portal hypertension. Other indications are int

Emotional and physical discomfort in urodynamic studies: A questionnaire-based assessment in a Middle Eastern population.

INTRODUCTION: Urodynamic study (UDS) is a valuable diagnostic test in urological practice. Due to the nature of the test, patients may experience emotional and physical discomfort. Mixed information exists on the degree of emotional and physical discomfort and their association with age, sex, body mass index, prior explanation, use of clean intermittent catheterization (CIC), and presence of a neurogenic bladder. METHODS: Over a period of 7 months, all patients undergoing UDS in our department were included in this study. Two self-completed anonymous questionnaires were distributed before and

Subacute and Chronic Low-Back Pain: From MRI Phenotype to Imaging-Guided Interventions.

Low-back pain (LBP) is a leading cause of disability worldwide. When symptoms persist beyond 4-6 weeks, when red flags are suspected, or when precise patient selection for procedures is needed, imaging-primarily MRI (Magnetic Resonance Imaging)-becomes pivotal. The purpose is to provide a pragmatic, radiology-first roadmap that aligns an imaging phenotype with anatomical targets and appropriate image-guided interventions, integrating MRI-based phenotyping with image-guided interventions for subacute and chronic LBP. In this narrative review, we define operational MRI criteria to distinguish ra

Short-Term Impact of Radiofrequency Ablation on Esophageal Functional Quality of Life in Patients With Barrett's Neoplasia.

BACKGROUND AND AIM: Endoscopic treatment is accepted as mainstream therapy to treat dysplastic Barrett's esophagus (BO) or early esophageal adenocarcinoma (OAC) to prevent disease progression. Radiofrequency ablation (RFA) is widely used for treating flat, noncancerous Barrett's mucosa but may cause significant short-term pain and dysphagia. This study aimed to assess the short-term impact of RFA on esophageal functional quality of life (QOL), compared to surveillance endoscopy. METHODS: This prospective, observational study was conducted at a tertiary hospital in Brisbane, Australia, and enr

Comparative approach for abdominal wall reconstruction after ventral hernia: open versus minimally invasive surgery.

INTRODUCTION: The classic open ventral hernia repair provides excellent results in recurrences. However, wound complications are the Achilles heel for a good overall clinical outcome. Laparoscopic surgery is in general associated with less pain, better esthetic results, faster recovery, and lower incidence of wound complications. Robotic procedures provide increased degrees of freedom, may improve ergonomics, and allow scaling and performance of finer movements deemed difficult through alternate surgical approaches. PURPOSE: The aim of this study is to compare outcomes between open and minima

Advancing frameless stereotactic navigation for precise targeting of the foramen ovale during radio-frequency thermal ablation (RFTA) for trigeminal neuralgia in anesthetized patients using triggered electromyography.

PURPOSE: Radio-frequency thermal ablation (RFTA) is an effective method for treating trigeminal neuralgia (TN). However, challenges persist in locating the foramen ovale (FO) and awake surgery has its limitations. Here we evaluate a novel neurophysiologic technique using dynamic mapping with triggered electromyography (T-EMG) to guide surgeons in identifying the FO under general anesthesia. METHODS: We prospectively collected clinical data from all patients undergoing RFTA for TN with dynamic T-EMG between September 2022 and December 2024. We subsequently reviewed the data within this databas

Hyperbaric Oxygen Therapy for Enhanced Postoperative Recovery in Hair Transplantation.

Postoperative recovery following hair transplantation procedures often involves prolonged healing periods, which can delay patients' return to normal activities. The objective of this work is to evaluate the efficacy of hyperbaric oxygen therapy (HBOT) as an adjunctive treatment in accelerating wound healing, reducing scab permanence, and enabling earlier resumption of standard hygiene practices and daily activities in patients undergoing hair transplantation procedures. Five male patients (age range 28-56 years) undergoing follicular unit excision (FUE) hair transplantation were enrolled in t

A Case of Acute Pancreatitis Developing After Upper Gastrointestinal Endoscopy: A Rare Post-Procedural Complication.

Acute pancreatitis is a rare but clinically significant complication of upper gastrointestinal endoscopy. Its diagnosis can be easily overlooked if abdominal symptoms are associated with the primary indication for the procedure. Although endoscopy is generally considered a reliable diagnostic tool, postprocedural pancreatitis has rarely been described in the literature, and its underlying mechanisms are not yet fully understood. Possible explanations include mechanical irritation from gastric or duodenal manipulations, increased intraluminal pressure due to air inflation, or induction of proce

A case series of fluoroscopy-guided neurolytic splanchnic nerve block for chronic pancreatitis pain.

CONTEXT: Splanchnic nerve block (SPN) with local anesthetic and steroid is used to relieve the pain of chronic pancreatitis. However, it does not provide long-lasting relief. We hypothesize that the use of 70% alcohol will give adequate analgesia for more than months. AIMS: The primary objective was to find out the analgesic efficacy of the use of 70% alcohol with SPN. Secondary objectives included the incidence of side effects, analgesic consumption postintervention, quality of life (QOL) assessed via a self-reported quality of life scale, and repeat block during the 1-year follow-up period.

Percutaneous ultrasound-guided A1 pulley release utilizing a modified 20-gauge spinal needle.

BACKGROUND: Trigger finger is a common cause of hand pain. Though multiple techniques for percutaneous A1 pulley release have been described in the literature, there is a continued need for safe and effective techniques using inexpensive, familiar, and commonly found instruments. This study evaluated outcomes of percutaneous A1 pulley release performed using a novel technique with a modified 20-gauge spinal needle and ultrasound guidance, with follow-up outcomes at least 6 months after the procedure. OBJECTIVE: To evaluate the efficacy and safety of a novel percutaneous A1-pulley release tech

Effect of lesion temperature on the durability of percutaneous radiofrequency rhizotomies to treat trigeminal neuralgia.

OBJECTIVE: Percutaneous radiofrequency rhizotomy is a common procedure for trigeminal neuralgia (TN) that creates thermocoagulative lesions in the trigeminal ganglion. Lesioning parameters for the procedure are left to the individual surgeon's discretion, and published guidance is primarily anecdotal. The purpose of this work was to assess the role of lesioning temperature on long-term surgical outcomes. METHODS: This was a retrospective analysis of patients who underwent percutaneous radiofrequency rhizotomy from 2009 to 2020. Patient data, including demographics, disease presentation, surgi
