Structure-guided discovery of 1H-pyrazolo[3,4-c]pyridin-3-amines as a novel class of hematopoietic progenitor kinase 1 (HPK1) inhibitors.

Hematopoietic progenitor kinase 1 (HPK1) is an intracellular negative regulator of immune responses, particularly in T cell receptor (TCR) signaling. Compelling genetic evidence indicates that HPK1 impairs multiple stages of antitumor immunity largely via its kinase activity, thereby positioning it as a promising target for cancer immunotherapy. Starting from two 3-aminopyrazole hits identified through in-house screening, we initiated a structure-guided medicinal chemistry campaign. Through multi-stage optimization to enhance potency by engaging the Asp101 residue and to improve kinase selecti

Changes in sputum viscoelastic properties and airway inflammation in primary ciliary dyskinesia are comparable to cystic fibrosis on elexacaftor/tezacaftor/ivacaftor therapy.

BACKGROUND: Primary ciliary dyskinesia (PCD) and cystic fibrosis (CF) are muco-obstructive lung diseases that are caused by distinct genetically determined defects in mucociliary clearance; however, knowledge on the relative severity of airway mucus dysfunction and chronic inflammation remains limited. The aim of this study was therefore to compare sputum viscoelastic properties, inflammation markers and the proteome between patients with PCD and patients with CF before and under elexacaftor/tezacaftor/ivacaftor (ETI) therapy. METHODS: We compared sputum rheology, inflammation markers and the

Drug-Drug Interaction Signals for Major Bleeding in Nursing Home Residents: High-Throughput Screening of Clopidogrel and Oral Anticoagulants.

PURPOSE: Nursing home (NH) residents are at high risk for major bleeding and are frequently exposed to complex medication regimens that may increase the likelihood of clinically important drug-drug interactions (DDIs). However, DDI evidence from community-dwelling populations may not generalize to NH residents, and real-world signals involving clopidogrel and oral anticoagulants remain incompletely characterized in this setting. We aimed to identify and interpret candidate DDI signals for major bleeding involving clopidogrel and oral anticoagulants among NH residents. METHODS: Using linked Mi

Triple combination cystic fibrosis transmembrane receptor modulator effects on glycaemia and insulin kinetics in cystic fibrosis with and without diabetes.

BACKGROUND: Greater insight into the effects of cystic fibrosis (CF) transmembrane modulators such as elexacaftor-tezacaftor-ivacaftor (ETI) on glucose metabolism can support a more dynamic and individualised approach to CF-related dysglycaemia. AIM: To assess changes in glycemia, body weight and pulmonary function in adults with CF with and without cystic fibrosis related diabetes (CFRD) following ETI. METHODS: This single-centre retrospective cohort study included adults with at least one F508del mutation (23 with CFRD, 38 without CFRD). Exploratory outcomes were HbA1c and total daily insu

Clinical Impact of Gleason Pattern 5 on Doublet Versus Triplet Therapy in Metastatic Hormone-Sensitive Prostate Cancer.

BACKGROUND: Upfront treatment intensification with androgen deprivation therapy (ADT) plus an androgen receptor pathway inhibitor has become a standard approach for metastatic hormone-sensitive prostate cancer (mHSPC). However, prognosis remains poor for some patients despite second-generation androgen receptor antagonist-based doublet therapy. Although triplet therapy with darolutamide and docetaxel is available, its added clinical value over apalutamide- or enzalutamide-based doublet therapy in real-world practice remains unclear. Gleason pattern 5 (GP5) reflects aggressive tumor biology and

Exploring the lived experiences of individuals with cystic fibrosis after the implementation of elexacaftor/tezacaftor/ivacaftor for treatment.

BACKGROUND: CFTR modulators are a class of medications prescribed to people with cystic fibrosis (pwCF) who have certain genetic mutations. In 2019, elexacaftor/tezacaftor/ivacaftor (ETI) was approved by the FDA and led to significant health improvements for pwCF. Despite ETI showing significant health improvements compared to prior modulators, little is known about how ETI impacts the lived experience of pwCF. OBJECTIVES: This study explored how the implementation of ETI in cystic fibrosis treatment has impacted the lived experience of pwCF and what additional resources healthcare providers

Prolonged progression-free survival with zanubrutinib in relapsed/refractory CLL: an indirect treatment comparison versus other BTK inhibitors using multilevel network meta-regression.

BACKGROUND: Bruton tyrosine kinase inhibitors (BTKis) are therapeutic agents for relapsed/refractory chronic lymphocytic leukemia (R/R CLL). Previous indirect treatment comparisons are limited in simultaneously comparing multiple interventions and adjusting for population differences. This study aimed to use a more rigorous approach called multilevel network meta-regression (ML-NMR) to estimate the relative treatment effects of zanubrutinib compared to acalabrutinib and ibrutinib in two target populations: a general R/R CLL population similar to the phase 3 ALPINE trial's intention-to-treat (I

Targeting of ibrutinib resistance-driving pathways by miR-28 in ABC-DLBCL.

Diffuse large B-cell lymphoma (DLBCL) is the most common aggressive B-cell lymphoma. Although many patients respond well to R-CHOP immunochemotherapy, those with the activated B-cell (ABC) subtype are often refractory or relapse. Bruton tyrosine kinase (BTK) inhibitors such as ibrutinib have improved outcomes, but acquired resistance limits their long-term efficacy. Here, we modeled the development of ibrutinib resistance in ABC-DLBCL and investigated whether the BCR-signaling regulator microRNA-28 (miR-28) can block this process. Using flow cytometry-based competition assays, multicolor clona

Zanubrutinib versus fludarabine, cyclophosphamide and rituximab in fit, treatment-naïve patients with chronic lymphocytic leukemia: a matching-adjusted indirect comparison.

Aim: Fludarabine, cyclophosphamide and rituximab (FCR) is a first-line therapy for fit treatment-naive patients with chronic lymphocytic leukemia (CLL); however, its hematotoxicity and related infections necessitate more efficacious, safer treatments. Zanubrutinib is a highly potent and selective next-generation Bruton tyrosine kinase inhibitor approved for treatment-naive patients with CLL and small lymphocytic lymphoma. Given the absence of clinical trials providing head-to-head comparisons, the aim of this analysis was to conduct a matching-adjusted indirect comparison between zanubrutinib

Management Strategies for Hepatotoxicity Associated With Elexacaftor/Tezacaftor/Ivacaftor: Results of a Provider Survey.

BACKGROUND: Elexacaftor/tezacaftor/ivacaftor (ETI) has significantly improved lung function and the clinical course of people with cystic fibrosis (PwCF) but also can cause elevations in liver function tests (LFTs). While the prescribing information (PI) provides some recommendations for holding ETI or hepatically adjusting doses based on certain criteria, clinicians are often faced with clinical scenarios that do not meet those criteria (e.g. PwCF with CF liver disease [CFLD, now termed CF hepatobiliary involvement (CFHBI)] that do not meet Child-Pugh Class B or C criteria). The purpose of th

Efficacy and safety of ruxolitinib combined with steroids for first-line treatment of acute graft-versus-host disease after hematopoietic stem cell transplantation: a single-center, real-world experience.

INTRODUCTION: Despite the increasing use of allogeneic hematopoietic stem cell transplantation (allo-HSCT), graft-versus-host disease (GVHD) remains the main cause of morbidity and mortality, significantly impacting HSCT outcomes. Steroids are the standard first-line treatment for acute GVHD (aGVHD); however, standardized treatment algorithms for patients who do not respond to steroid therapy are lacking. Ruxolitinib is the most promising second-line therapy for steroid-refractory (SR)-GVHD, but data on its first-line use for aGVHD are limited. METHODS: In this retrospective study, we analyze

Inhibiting the TGF-β1 Pathway Reduces the Aggressiveness of Intrahepatic CCA HuCCT1 CD90-Positive Cells.

Molecular mechanisms responsible for the poor prognosis in patients with intrahepatic cholangiocarcinoma (CCA) are still unknown, but stem cell marker Cluster Differentiation 90 (CD90) has been reported to be associated with a more aggressive cancer phenotype. In this scenario, the TGF-β1 signaling pathway likely has a role as master gene regulator. Aim of the study is to investigate the role of CD90 in iCCA aggressiveness. The molecular profile of HuCCT1/CD90+ and HuCCT1/CD90- cells was obtained through transcriptomic analysis (NGS). Bioinformatic data were confirmed in both cell lines by qRT

Comparative study of ensitrelvir and symptomatic therapy in healthcare workers with mild COVID-19: A single center retrospective analysis in Chiba, Japan.

BACKGROUND: Ensitrelvir, a novel oral antiviral targeting the 3CL protease of SARS-CoV-2, has demonstrated efficacy in reducing viral load in clinical trials. Rapid viral clearance in healthcare workers (HCWs) with mild COVID-19 is critical for symptom relief and minimizing secondary transmission. However, real-world evidence on ensitrelvir's effectiveness in this population is limited. METHODS: This single-center retrospective study analyzed 407 HCWs with mild COVID-19 treated with either ensitrelvir or symptomatic therapy. Viral load was assessed using reverse transcription-polymerase chain
