Scintigraphy with 67Ga-DFO-DAS-fibrinogen (67Ga-fibrinogen) was performed for the positive delineation of thrombi in patients with suspected venous or arterial thrombus formation. Five out of 17 patients showed 67Ga-fibrinogen accumulation corresponding to thrombus formation, as confirmed by other examination including radionuclide venography, contrast angiography, and CT scan. There was some difference in 67Ga-fibrinogen uptake between venous and arterial thrombi. A 29 year-old-man presenting with advanced metastatic malignant melanoma was successfully imaged using carbon-11 (11C) labeled alpha-aminoisobutyric acid (AIB), a synthetic, non-metabolized amino acid transported into viable cells by the A-type, or alanine-preferring, amino acid transport system. Tumor located in the hilum of the lung was well visualized with 11C-AIB prior to chemotherapy. A gallium image with liver subtraction using 99mTc-sulfur colloid demonstrated regions of increased activity in liver which correlated with regions of increased activity on the 11C-AIB liver image.

The motility of the ileocaecal region of the gut was studied in 10 women with irritable bowel syndrome (IBS) and bloating and in 8 normal women. Bran labelled with 37 MBq 99mTc was administered after fasting, and a dynamic scan was done after a standard meal 3 h later. Time-activity curves were plotted for the ileum and caecum. In controls, ileal emptying was faster, peak % counts in the caecum were higher, and ileocaecal clearance was greater than in those with IBS. Thyroid carcinoma is not a common disorder but the metastatic thyroid carcinoma manifesting thyrotoxicosis is exceedingly rare. Since Leiter et al. described the first patient with adenocarcinoma of the thyroid with functioning metastases and postoperative thyrotoxicosis in 1946, about 20 such cases have been reported. We have examined two cases of metastatic follicular thyroid carcinoma with T3-hyperthyroidism after surgical ablation of the gland. The thyrotoxicosis was induced by excessive iodine contamination during preoperative radiological examination.

Tc-99m labeled iminodiacetic acid (Tc-99m IDA) studies were performed in three patients with bile leaks. The radiotracer distribution was different in all three. A diffuse peritoneal pattern was seen in one patient. Localized radioactivity was seen in two; in one in the gallbladder bed and in the other in the perihepatic region.

The Hyper IgE syndrome is a rare disease consisting of recurrent sinusitis and pneumonia, pneumatoceles, chronic dermatitis, and elevated serum levels of IgE. The primary radiographic abnormalities are recurrent alveolar lung disease and pneumatoceles. Pneumothorax may occasionally occur as in one of our cases. Other causes of pneumatoceles are usually easily excluded by the history and other clinical data. Pulmonary scintigraphy and computed tomography may add information valuable to the management of these patients.

Radionuclide bone scintigraphy is highly sensitive and specific for diagnosing the musculoskeletal disorders of childhood. Conditions such as neonatal osteomyelitis, septic arthritis, diskitis of childhood, Legg-Calvé-Perthes disease, the osteochondroses, the "toddler's fracture," sports injuries, spondylolysis, myositis ossificians, and reflex sympathetic dystrophy are readily defined. High-quality state-of-the-art scintigraphy is essential in infants and young children.

Emergency room physicians frequently are faced with the evaluation of the patient with acute testicular pain. The rapid differentiation between testicular torsion and acute epididymitis is necessary although frequently difficult due to an atypical presentation.
