Contemporary management of chronic rhinosinusitis with nasal polyposis in aspirin-exacerbated respiratory disease: an evidence-based review with recommendations.

BACKGROUND: Chronic rhinosinusitis (CRS) in aspirin-exacerbated respiratory disease (AERD) represents a recalcitrant form of sinonasal inflammation for which a multidisciplinary consensus on patient management has not been reached. Several medical interventions have been investigated, but a formal comprehensive evaluation of the evidence has never been performed. The purpose of this article is to provide an evidence-based approach for the multidisciplinary management of CRS in AERD. METHODS: A systematic review of the literature was performed and the guidelines for development of an evidence-

Current trends in topical therapies for chronic rhinosinusitis: update and literature review.

Chronic rhinosinusitis (CRS) affects millions of patients worldwide. The disease is multifactorial with influences including anatomic factors, immunological disturbances, and altered sinonasal microbiome. Although oral medications are effective in controlling some symptoms, they are associated with side effects and long-term use is not ideal. Thus, topical therapies have emerged as an alternative delivery method for localized, high-concentration medication with less side effects. Areas covered: This is a review of the various topical therapies available or under investigation for the managemen

A new method of identifying the posterior inferior nasal nerve: implications for posterior nasal neurectomy.

INTRODUCTION: Posterior nasal neurectomy is an effective way of treating recalcitrant rhinitis. The aim of this study is to describe the anatomic relationship between the posterior inferior nasal nerve (PINN) and the structures that might be important for posterior nasal neurectomy. MATERIALS AND METHODS: An anatomic study was conducted in a university hospital dissection laboratory with 15 formalin-fixed, sagittally cut adult cadaver heads. The distance between PINN and (1) nasal sill, (2) maxillary sinus ostium, (3) posterior fontanel, (4) torus tubarius, and (5) crista ethmoidalis was meas…

Management of chronic rhinosinusitis in asthma patients: is there still a debate?

The united airway concept in which upper and lower respiratory conditions are present in one patient requires special consideration. There is some evidence linking chronic rhinosinusitis and asthma, but a good understanding of the pathophysiology and combined management is still lacking, a fact that leads to discussion. Bronchial asthma is more prevalent in patients who suffer chronic rhinosinusitis. On the other hand, patients with asthma have a greater prevalence of rhinosinusitis than patients without asthma. The effect of chronic rhinosinusitis in patients with or without nasal polyps on a

Measuring the breath of life.

The inability to breathe freely and clearly is a major source of concern to patients and a common symptom in many sinonasal conditions (1). … of conditions from the common cold and acute bacterial rhinosinusitis to allergic and non-allergic rhinitis, septal deformity or a tumour. Paradoxically, many people, myself included, go through life largely oblivious to a significant septal deflection. It therefore begs the question as to why an individual with a long-standing anatomical variant such as th …

Evidence-based treatment of chronic rhinosinusitis with nasal polyps.

BACKGROUND: Chronic rhinosinusitis with nasal polyps (CRSwNPs) is the most challenging form of CRS to treat. Clinical practices vary widely among physicians around the world, often with little evidence to support current therapies. METHODS: This study evaluates various medical and surgical treatments with a focus on the highest levels of evidence. Recommendations for various therapies are provided based on consensus panels and evidence-based reviews. RESULTS: Therapies with the strongest evidence and recommendations for CRSwNPs include oral steroids and topical nasal steroid sprays. There is

Treatment and outcomes of chronic rhinosinusitis in children with primary ciliary dyskinesia: where is the evidence? A qualitative systematic review.

BACKGROUND: Consensus on treatment of chronic rhinosinusitis (CRS) in children with primary ciliary dyskinesia (PCD) is limited. We sought to synthesize the best available evidence pertaining to treatment and outcomes of CRS in children with PCD. METHODS: This work is a systematic review of PubMed and EMBASE for studies pertaining to treatment and outcomes of CRS in children with PCD in a 20-year period. Two reviewers independently extracted information on study design, setting, medical or surgical therapy for CRS, and treatment outcomes. RESULTS: Twelve studies from 8 countries were include

Cerebro-rhino orbital mucormycosis: an update.

Mucormycosis is an uncommon fungal infection which can lead to fulminant necrotizing infection under optimal host condition. Fungi have the ability to invade blood vessels and can affect different parts of the body. The most common, though the most aggressive, form is cerebro-rhino-orbital mucormycosis that occurs in debilitated patients, in conjunction with sinus or para-sinus involvement. Due to increased number of newly diagnosed cases of mucormycosis world-wide resulting from uncontrolled metabolic conditions, this paper intends to widen the reader's scope and knowledge about the nature of

[Fungal rhinosinusitis - still unsolved problem].

Chronic rhinosinusitis (CRS) is an inflammatory disease affecting the nose and paranasal sinuses. Fungi are considered to be one of the pathogens responsible for some kinds of CRS. Fungal rhinosinusitis - the classifications schemes, the diagnostic criteria and methods of treatment are still being discussed. 338 patients underwent the endoscopic sinus surgery due to CRS in the Otolaryngology Department of the Medical University of Lodz in the years 2009-2011 (February). The histopathological examination revealed fungi only in three patients. The authors report three cases of patients with fung

[The dependence of Toll-receptor expression on pathomorphological features of nasal polyps in patients with polypous rhinosinusitis].

This work was designed to study expression of Toll 1-10 receptors on the surface of cells present in inflammatory infiltrate from nasal polyps and peripheral blood of the patients with polypous rhinosinusitis. It was shown that the intensity of expression depended on the pathomorphological characteristics of nasal polyps. Tissues removed from the patients with polyps of the oedematous type contained more Toll-10 positive cells and showed enhanced expression of Toll-5 receptors on monocytes and lymphocytes, Toll-3 receptors on monocytes, granulocytes, and lymphocytes, and Toll-9 receptors on gr

Global airway disease beyond allergy.

Besides the anatomic continuity of the upper and lower airways, inflammation in one part of the airway influences the homeostasis of the other. The mechanisms underlying this interaction have been studied primarily in allergic disease, showing systemic immune activation, induction of inflammation at a distance, and a negative impact of nasal inflammation on bronchial homeostasis. In addition to allergy, other inflammatory conditions of the upper airways are associated with lower airway disease. Rhinosinusitis is frequently associated with asthma and chronic obstructive pulmonary disease. The i

Up front about frontal headaches and sinusitis.

In clinical practice frontal headaches are common however are frequently incorrectly attributed to rhinosinusitis. The misdiagnosis of frontal headaches and/or facial pain has been compounded by increased accessibility and over-reliance on sinus CT imaging. We reviewed the presentation and clinical findings of 3 patients in our unit with frontal headaches misattributed to rhinosinusitis. We also reviewed the literature regarding the diagnostic criterias for rhinosinusitis and the role/limitations of CT imaging in the diagnosis of paranasal sinus disease. All 3 patients had isolated frontal hea

[Orbital complications of rhinosinusitis].

INTRODUCTION: Orbital complications were observed in 53 patients (1.35%, n=53/3912 of all treated patients; 11.04%, n= 53/480 of hospitalized patients). Complications in the orbit can occur in 3-5% of adults with the inflammatory condition of sinuses, while the percentage in children ranges from 0.5-8%. OBJECTIVE: The objective of our work was to determine the frequency of the occurence of orbital complications of the rhinosinal inflammatory origin in a group of adults and children. METHOD: The retrospective analysis of patients treated of rhinosinusitis in the period 1992 to 2007, in the Cl

A systematic review of the use of intranasal steroids in the treatment of chronic rhinosinusitis.

OBJECTIVE: To determine if intranasal steroids (INS) are useful in the management of chronic rhinosinusitis without polyps and chronic rhinosinusitis (CRS) with polyps. DATA SOURCES: Studies for inclusion were searched using Medline, EMBASE, Cochrane databases, and references of included studies. REVIEW METHODS: Initial screening of article titles and abstracts obtained from the literature search was performed independently by two reviewers (SAJ and RT) based on the research protocol criteria. These articles then underwent a second-stage review. Each article was read in detail and discussed

[Oral allergy syndrome--problem].

History of oral allergy syndrome goes back to 1987 year, when Amlot for the first time used this name for pollen-food cross-reactive reactions. Since that time many kinds of various reactions between pollens and different food had been described. Is seems that after almost 20 years the knowledge about this highly prevalent phenomenon is still incomplete and not well known among specialist other than allergologists. We try to reach consensus on several aspects of this food-induced syndrome, which may be helpful in laryngological practice. It is very important especially for more severe immediat
