Diagnosis and treatment of infraspinatus tendon-bursa ossification in a Eurasian Dog.

A 4-year-old male Eurasian Dog presented at our veterinary clinic with a history of perpetual forelimb lameness in both thoracic limbs. In the clinical exploration, direct pressure over the infraspinatus tendon of insertion caused pain in both thoracic forelimbs and a firm band-like structure was palpable. No improvement was observed after treatment with rest, non-steroidal anti-inflammatory drugs and an intralesional injection of a long-acting glucocorticoid. Radiographic examination, ultrasonographic exploration and computed tomography were performed, identifying ossified structures lateral

Shoulder problems in overhead sports. Part I - biomechanics of throwing.

The article discusses the biomechanical processes that occur during an overhead throw. This activity is highly specialized and requires full and proper function from the shoulder joint. It consists of active and passive stabilization and synchronous work of the accelerating and decelerating muscles. The process of the overhead throw can be divided into several phases that differ from each other in biomechanical parameters and involvement of specific muscles.

[Open reconstruction of the rotator cuff].

Success rates relating to relief from pain and improved function following open reconstruction of the rotator cuff have been documented in many studies. At least for small to medium-size tears the question currently arises of whether an arthroscopic repair would not also give good treatment results. At present, however, the results of arthroscopic repair are not yet comparable to those yielded by the open techniques, at least in terms of recurrent defects especially in the case of massive tears (affecting more than two tendons). Open surgical repair is currently still recommended for all patie

Analysis of the distribution of cutaneous perforators in cutaneous flaps.

An anatomic and statistical analysis was performed on the distribution of cutaneous perforators that perfuse the scapular, radial forearm, and lateral arm cutaneous flaps. Perforators were categorized as direct, terminal, and intransitive, depending upon perforator origin and termination site relative to the source artery. Statistical cluster analysis of perforator distributions was performed to determine the regions in which cutaneous perforators are consistently found. The scapular and radial forearm flaps could be divided into up to three well-perfused segments. The analysis predicted the p

Return to sport after a Bankart operation of the shoulder using the Mitek anchor system.

Sixteen athletes suffering anterior instability of the shoulder after primary or recurrent traumatic anterior dislocation were followed for a mean of 23.9 months after an open Bankart operation using Mitek anchors. Nine patients resumed sports an average of 4.4 months postoperatively, while the remaining seven patients did not return to sports at all. By comparing the group of athletes who had resumed sports and the group who had not, we found in the former group a lower mean age, a higher number of athletes who had inflicted their first anterior dislocation during sports and a larger number o

Partial repair of irreparable rotator cuff tears.

This study analyzes the results of a previously unreported technique of reconstruction for the massive irreparable rotator cuff tear. The technique involves repair of the margins of the tear to restore the force couples and "suspension bridge" system of force transmission in the shoulder. Complete coverage of the defect was not considered to be essential as long as the normal mechanics of the shoulder were restored and the rotator cuff tear was converted to a "functional cuff tear." This procedure was performed on 14 patients. Improvement in function was dramatic. Active elevation (elevation d

[Isolated lesions of the subscapularis muscle. Apropos of 21 cases].

PURPOSE OF THE STUDY: The goal of this retrospective study is to describe the different anatomic lesions of the subscapularis and to precise the diagnostic value of the clinical and imaging tests. MATERIAL: Twenty-one cases of isolated tear of the subscapularis were operated on between 1989 and 1992. Affecting both sex, this lesion happens in younger patients than the rotator cuff tear concerning supra or infra-spinatus. The onset was most often traumatic but not specific. Four patients had no traumatic history. RESULTS: In 16 cases, the complaint was an unspecific chronic painful shoulder.

Patterned injuries from a modified sickle: Forensic observations and insights-Case report.

Sharp weapons are common homicide weapons. However, in rare cases, unusual weapons are used for crimes and can pose a challenging task to the forensic pathologist during autopsy. Darat is an indigenous agricultural tool used in the hilly regions of India. Darat is a modified sickle, and literature on injury patterns caused by it is exceptionally rare. This case report describes a double homicide involving a 72-year-old man and his 65-year-old wife, who were killed by a Darat. Autopsy findings revealed multiple chop, stab and incised wounds to the head, neck, and shoulders, consistent with heav

A Limb-Threatening Long Arterial Dissection Caused by Humerus Neck Fracture: A Case Report.

Proximal humerus fracture is a common arm trauma and rarely occurs with vascular injury which however is a serious complication. In this case report, we present a long segment dissection of the axillary and brachial arteries as a rare complication due to fragmented proximal humerus fracture and shoulder dislocation. An 80-year old female patient was seen at the emergency department. Radiograph examination has revealed a fragmented proximal humerus fracture besides dislocation of the head of humerus towards the axillary area. On vascular examination, acute arterial occlusion such as absence of

Operative management of children's fractures of the shoulder region.

Fractures about the shoulder in children rarely require operative treatment. Exceptions include open fractures and those associated with neurovascular compromise. Fractures of the proximal humerus in older children that cannot be adequately reduced and maintained should be treated with open reduction and internal fixation. Interposition of periosteum and biceps tendon can lead to difficulty in fracture reduction. Irreducible displaced fractures of the clavicular shaft, fractures that develop nonunion, and congenital pseudarthrosis of the clavicle can be treated by an intramedullary pin techniq

Revisiting the incidence of brachial plexus birth injury: a 2021-2024 prospective surveillance study.

OBJECTIVE: By implementing a new, real-time reporting surveillance system to capture patients born in an extensive regional healthcare system, the authors evaluated the incidence of brachial plexus birth injury (BPBI). They hypothesized that the true incidence of BPBI is higher than previously reported. METHODS: A prospective surveillance system was established in obstetric units and affiliated tertiary-level neonatal ICUs (NICUs) at 2 sites within the authors' healthcare system to ensure the capture of every BPBI. If the infant was born at either site, this reporting system was used so that

Chronic Irreducible Anterior Shoulder Dislocation With Axillary Vein Compression: A Case Report Highlighting Multidisciplinary Management With Reverse Total Shoulder Arthroplasty.

INTRODUCTION: Anterior shoulder dislocations are the most common major joint dislocation encountered in clinical practice. Although typically reducible, chronic irreducible anterior dislocations are rare and pose significant diagnostic and management challenges. Contributing factors include rotator cuff pathology, soft tissue interposition, and bony fragments. Vascular complications, while infrequent, must also be considered, particularly in chronic presentations. CASE REPORT: We report the case of a 61-year-old female who sustained an anterior shoulder dislocation after falling on ice 8 week

Concurrent Long Thoracic and Lateral Cutaneous Intercostal Nerve Entrapment After Chest Tube Thoracostomy Causing Scapular Winging and Chest Wall Pain: Diagnosis With Ultrasound and Treatment With Hydrodissection.

BACKGROUND: Chest tube thoracostomy may cause iatrogenic nerve injury, resulting in persistent pain and functional impairment. Long thoracic nerve (LTN) palsy after chest tube placement has been described, but concurrent involvement of the LTN and the lateral cutaneous branch of an intercostal nerve (LCIN) appears to be rarely reported. We describe a patient with persistent post-thoracostomy chest wall pain and scapular winging in whom musculoskeletal ultrasonography identified entrapment of both nerves. CASE PRESENTATION: A 43-year-old man presented with severe left shoulder and chest wall p
