Nurse-Led Large Language Model Chatbot for Predicting and Preventing Complications After Coronary Artery Bypass Grafting: Protocol for a Randomized Controlled Trial.

BACKGROUND: Thirty-day unplanned readmission following coronary artery bypass grafting (CABG) affects 10%-20% of patients and is a key quality indicator, particularly in low- and middle-income countries (LMICs) where access to cardiac rehabilitation is limited. Existing risk models are static, lack real-time engagement, and no validated large language model (LLM)-based clinical decision support (CDS) system exists for post-CABG readmission prevention. OBJECTIVE: This protocol describes the development, validation, and evaluation of Smart CABGuard, a nurse-led, LLM-based CDS chatbot with conti

Machine Learning-Based Predictive Model for Functional Independence in Spinal Cord Injury: Protocol for a Predictive Rule Development and Validation Study.

BACKGROUND: Spinal cord injury (SCI) causes substantial disability by disrupting spinal pathways, making functional independence a central rehabilitation goal. In 2024, approximately 15.4 million people worldwide were living with SCI. Despite its clinical relevance, traditional prognostic tools, notably the International Standards for Neurological Classification of Spinal Cord Injury (ISNCSCI), have limitations. Furthermore, access to advanced diagnostics constrains prediction. Existing models often fail to estimate independence, limiting goal setting and resource planning. OBJECTIVE: This st

A Ruptured Tri-Lobulated ICA-PCom Aneurysm Presenting with Preserved Neurological Function: Case Report and Clinical-Anatomical Analysis.

Background and Clinical Significance: Although rupture of aneurysms at the internal carotid-posterior communicating artery (ICA-PCom) junction accounts for a small percentage of all ruptured intracranial aneurysms, they are clinically relevant due to their proximity to perforator-rich cisterns, the optic-carotid-oculomotor pathways and flow-diverting zones, as well as their high likelihood for causing early neurological instability. Additionally, ruptured ICA-PCom aneurysms that have multiple lobulations are associated with increased variability in wall shear stress, local inflammatory remodel

Magnitude of [(11)C]PK11195 binding is related to severity of motor deficits in a rabbit model of cerebral palsy induced by intrauterine endotoxin exposure.

Intrauterine inflammation is known to be a risk factor for the development of periventricular leukomalacia (PVL) and cerebral palsy. In recent years, activated microglial cells have been implicated in the pathogenesis of PVL and in the development of white matter injury. Clinical studies have shown the increased presence of activated microglial cells diffusely throughout the white matter in brains of patients with PVL. In vitro studies have reported that activated microglial cells induce oligodendrocyte damage and white matter injury by release of inflammatory cytokines, reactive nitrogen and

Inhaled corticosteroids versus placebo  for stable chronic obstructive pulmonary disease.

BACKGROUND: The role of inhaled corticosteroids (ICS) in chronic obstructive pulmonary disease (COPD) has been the subject of much uncertainty. COPD clinical guidelines currently recommend selective use of ICS. ICS are not recommended as monotherapy for people with COPD, and are only given in combination with long-acting bronchodilators due to greater efficacy of combination therapy. Incorporating and critiquing newly published placebo-controlled trials into the monotherapy evidence base may help to resolve ongoing uncertainties and conflicting findings about their role in this population. Climate, environmental, and programmatic correlates of malaria resurgence in Amhara, Ethiopia (2018-2024): a Bayesian spatiotemporal analysis.

BACKGROUND: After substantial progress in malaria control, Ethiopia's Amhara Region experienced a marked resurgence since 2018. The relative contributions of climate variability, environmental context, intervention coverage, and unmeasured factors to this resurgence remain inadequately quantified. This study used a Bayesian spatiotemporal framework to estimate factor associations with malaria incidence, decompose spatial versus temporal climate effects, and identify persistent hotspots. METHODS: We conducted an ecological district-level panel analysis of 13,944 district-month observations fro

The arabic version of the fat phobia scale-short form: reliability and structural validity.

BACKGROUND: Weight bias, often known as fat phobia or weight stigma, refers to unfavorable attitudes and stereotypes that are associated with, and applied to, larger bodies. Fat phobia can include an unreasonable and abnormal dread of being overweight or being associated with obese people. Currently, there is no validated tool available to measure fat phobia in Arabic. Measuring fat phobia in Arabic-speaking populations is crucial, because cultural attitudes toward body weight can be quite negative and discriminatory; these negative attitudes can negatively impact mental health. The current st

Characteristics of the Gut Microbiota Composition of the Arctic Zone Residents in the Far Eastern Region.

Background. In many studies over the past decade, scientists have made a connection between the composition of gut microbiota and human health. A number of publications have shown that gut bacteria are involved in many metabolic and physiological processes of the organism. The composition of the gut microbiome is unique for each person and is formed under the influence of various factors associated with both the individual characteristics of the body and the characteristics of the environment. Different regional characteristics make it necessary for the body to adapt to certain conditions, inc

The Prevalence of Thyroid Autoimmunity in Pregnancy and Adverse Neonatal Outcomes at a Secondary Care Hospital in the Middle East.

Background and objective Among the common endocrinological disorders of pregnancy, thyroid disorders rank second after diabetes. Thyroid autoimmunity is linked to sub-fertility, miscarriages, preterm birth, gestational diabetes, and adverse neurodevelopmental sequelae in children. Peroxidase and thyroglobulin autoantibodies may be associated with enhanced global autoimmune state, which may have adverse effects on the fetal or placental development. It is the main cause of hypothyroidism in reproductive-age women and is associated with poor obstetric outcomes. In Gulf countries, a higher preval

Value of cervical electrical impedance spectroscopy to predict spontaneous preterm delivery in asymptomatic women: the ECCLIPPx prospective cohort study.

OBJECTIVES: Preterm birth (PTB) accounts for two-thirds of deaths of structurally normal babies and is associated with substantial lifetime healthcare costs. Prevention of PTB remains limited by the modest accuracy of prediction methods, namely transvaginal ultrasound (TVS) cervical length (CL) measurement and quantitative cervicovaginal fetal fibronectin (FFN) estimation. We report the first substantive study detailing the predictive performance of a cervical probe device based on electrical impedance spectroscopy (EIS) for PTB - the EleCtriCaL Impedance Prediction of Preterm birth by spectro

Single versus combination intravenous anti-pseudomonal antibiotic therapy for people with cystic fibrosis.

BACKGROUND: The choice of antibiotic, and the use of single or combined therapy are controversial areas in the treatment of respiratory infection due to Pseudomonas aeruginosa in cystic fibrosis (CF). Advantages of combination therapy include wider range of modes of action, possible synergy and reduction of resistant organisms; advantages of monotherapy include lower cost, ease of administration and reduction of drug-related toxicity. Current evidence does not provide a clear answer and the use of intravenous antibiotic therapy in CF requires further evaluation. This is an update of a previous
