A feasibility study of psychosocial group intervention for breast cancer patients with first recurrence.

GOALS OF WORK: The effects of psychosocial group interventions on improving quality of life (QOL) for patients with recurrent breast cancer are not well known. The objective of this study was to assess the feasibility of a psychosocial group intervention in Japanese women with first recurrence of breast cancer. PATIENTS AND METHOD: The subjects were consecutively selected from among patients who were diagnosed with a first recurrence of breast cancer. We conducted a 6-week psychosocial group intervention. QOL was assessed using the Profile of Mood States (POMS), the Impact of Event Scale-Revi

Heart rate variability and baroreflex sensitivity are reduced in chronically undernourished, but otherwise healthy, human subjects.

Alterations in autonomic nerve activity in subjects in a chronically undernourished state have been proposed, but have been inadequately documented. The present study evaluated heart rate and systolic blood pressure variability in the frequency domain in two underweight groups, one of which was undernourished and recruited from the lower socio-economic strata [underweight, undernourished (UW/UN); n =15], while the other was from a high class of socio-economic background [underweight, well nourished (UW/WN); n =17], as well as in normal-weight controls [normal weight, well nourished (NW/WN); n

Human rights, environmental justice, and the health of farm workers in South Africa.

Despite the democratization of South Africa in 1994, which brought the agricultural sector within the ambit of legal protection, farm workers remain vulnerable to an undue burden of social and health problems. Alcohol abuse due to the DOP system, pesticide poisonings, and other occupational hazards illustrate that the likely success of efforts at redress depends on a greater awareness of the rights and justice dimensions of the health problems facing these workers. International trade policies may exacerbate inequalities that deprive them of opportunities to realize their rights at national le

Drug dependence, parenting responsibilities, and treatment history: why doesn't mom go for help?

Despite longstanding concern that the presence of children deters drug-dependent women from entering treatment, there have been few empirical tests of the relationship between parenting responsibilities and treatment-seeking behavior. In this study, the relationship between number of biological children and treatment history was examined in a cohort of 153 women seeking methadone maintenance treatment. In a standard multiple regression analysis that also allowed for the potential influence of (a) age, (b) education, (c) ethnic minority status, (d) cohabitation with a sexual partner, (e) chroni

Discovering how urban poverty and violence affect health: development and validation of a Neighborhood Stress Index.

Health problems of the urban poor have been attributed to psychosocial effects of environmental stress. Testing such models requires an ability to measure neighborhood characteristics that make life stressful. The City Stress Inventory (CSI) uses self-report to assess perceived neighborhood disorder and exposure to violence. Data from an interracial sample of urban adolescents show the CSI to be internally consistent, stable, and correlated with census indices of social disadvantage. Validity for stress research is indicated by correlations with trait depression, anger, hostility, self-esteem,

[Speech development and intelligence in autism. How uniform is Asperger syndrome?].

Since the introduction of a separate diagnosis for Asperger's syndrome in the ICD-10 and DSM-IV classification systems, a controversial debate has continued on whether Asperger's syndrome is a specific, clearly distinguishable disorder within the autistic spectrum or whether it represents a milder phenotypical variation of autism. The effect on the amount of autistic symptoms of the variables language delay and level of intelligence was analyzed within a sample of individuals exhibiting autism diagnosed by standardized methods. Both variables showed a significant effect on the degree of autist

A preliminary investigation of the construct of psychopathic personality (psychopathy) in chimpanzees (Pan troglodytes).

Although the construct of psychopathy has received considerable attention in humans, its relevance to other animals is largely unknown. We developed a measure of psychopathy for use in chimpanzees (Pan troglodytes), the Chimpanzee Psychopathy Measure (CPM), and asked 6 raters to complete this index on 34 chimpanzees. The CPM (a) demonstrated satisfactory interrater reliability and internal consistency; (b) exhibited marginally significant sex differences (males > females); (c) correlated positively with measures of extraversion, agreeableness, and observational ratings of agonism, sexual activ

Residential transitions from nursing homes for adults with cerebral palsy.

PURPOSE: The impact of moving out of nursing homes into community-based settings for adults with cerebral palsy was assessed by comparing the health and community functioning of movers and non-movers at time 1 and 3 years later at time 2. SUBJECTS: The sample included 83 non-movers and 28 movers age 30 years and older who initially were living in nursing homes. The majority of residents had severe to profound mental retardation. The movers transferred to 15 community-based settings between 1 and 3 years (mean of 2 years) prior to the time 2 assessment. Assessments of residents at baseline and

Development and validation of measures of social phobia scrutiny fear and social interaction anxiety.

The development and validation of the Social Phobia Scale (SPS) and the Social Interaction Anxiety Scale (SIAS) two companion measures for assessing social phobia fears is described. The SPS assesses fear of being scrutinised during routine activities (eating, drinking, writing, etc.), while the SIAS assesses fear of more general social interaction, the scales corresponding to the DSM-III-R descriptions of Social Phobia--Circumscribed and Generalised types, respectively. Both scales were shown to possess high levels of internal consistency and test-retest reliability. They discriminated betwee

Patients' experiences of Parkinson's disease.

Patients' (n = 101) experiences of Parkinson's disease (PD) were studied through structured interviews. Oblique factor analysis produced three moderately intercorrelated clusters of items reflecting reported severity of motoric, cognitive, and psychological problems, respectively. Scales formed from the factors were correlated with demographic, disease-related, and psychosocial variables. The demographic variables were not significantly correlated with the scales or with any other variables in the set. Hoehn and Yahr staging was significantly related to scores only on the motoric severity scal

[Epidemiologic studies of oral mucosa changes occurring in children, adolescents, and adults 13-24 years of age in Warsaw].

The aim of the study was the analysis of RAS occurrence in the population and a comparison to other oral mucosa lesions as well as an analysis of the influence of various factors on RAS occurrence: age, social status of the subject's, parents' education, CPITN index. In the school year 1994/95 a questionnaire concerning the frequency of RAS was distributed to children and adolescents (13-18y) from two Warsaw schools and a group of soldiers of the Polish Army serving in Warsaw. Additionally, all study participants were checked towards RAS by the use of the clinical methods according to WHO meth…

Natural history study of pseudoachondroplasia.

Pseudoachondroplasia (PSACH) is a well-characterized autosomal dominant dwarfing condition. A great deal of information is available about orthopedic complications, but little is known about extraskeletal complications in adulthood. This study was undertaken to delineate the natural history of PSACH at all ages. Seventy-nine individuals responded to an extensive questionnaire that included information about deformities, operations, general health, chronic diseases, and reproduction. PSACH individuals were ascertained through the University of Texas Medical Genetics patient population, a geneti

Acute and transient psychotic disorder: comorbidity with personality disorder.

A study sample of 51 patients with acute and transient psychotic disorder (ATPD) (ICD-10) is presented. The findings suggest that, in hospital settings, ATPD is a non-frequent condition with onset in early adult life and most often associated with female sex, good premorbid social functioning and no or minor/moderate psychosocial stressors. The DSM-IV criteria distribute the patients into three diagnostic categories: schizophreniform disorder (41%), brief psychotic disorder (33%) and psychotic disorder not otherwise classified (25%). A high prevalence (63%) of personality disorders (PD) is rev

Premorbid adjustment as a predictor of phenomenological and neurobiological indices in schizophrenia.

Previous studies have demonstrated relationships between poor premorbid adjustment and a variety of phenomenological and neurobiological indices in schizophrenic patients. Using the Modified Premorbid Adjustment Scale we re-examined these relationships in a large sample (n = 131) of schizophrenic patients. Subjects were evaluated with the Comprehensive Assessment of Symptoms and History (CASH) and magnetic resonance imaging. Multiple correlation indicated that poor premorbid adjustment was significantly associated with prominence of negative symptoms, early age of onset, educational problems,

Chronic fatigue in primary care attenders.

From 686 patients attending primary care physicians, 77 were identified by a screening procedure as having chronic fatigue. Of these, 65 were given a comprehensive psychological, social and physical evaluation. Seventeen cases (26%) met criteria for the chronic fatigue syndrome. Forty-seven (72%) received an ICD-9 diagnosis of whom 23 had neurotic depression, with a further 5 meeting criteria for neurasthenia. Forty-nine were 'cases' as defined by the revised Clinical Interview Schedule (CIS-R), and 42 if the fatigue item was excluded. Psychiatric morbidity was more related to levels of social

[The effectiveness of ambulatory occupational therapy measures for patients with schizophrenia].

This study evaluates the concept of occupational therapy for schizophrenic outpatients. Preliminary results from 18 patients suggest that occupational therapy for outpatients might contribute to an improvement in cognitive-adaptive functions and open up ways for further integration on the job market. The prognosis was favourable when the outset of occupational therapy was not equated with a rise in the level of professional adaptation and a certain approachability and flexibility were to be observed in cognitive, affective and intentional aspects among the chronic schizophrenic patients. Motor, volitional and behavioural disorders in schizophrenia. 1: Assessment using the Modified Rogers Scale.

The assessment of catatonic symptoms, unlike that of other schizophrenia symptoms, has not acquired accuracy and phenomenological rigour, and their distinction from extrapyramidal side-effects is not always easy. The Modified Rogers Scale can rate abnormalities in movement, volition, speech, and overall behaviour in schizophrenia. It is detailed, reliable and valid, and permits the isolation of a score for non-extrapyramidal and hence presumably catatonic phenomena.

Social activities after stroke: measurement and natural history using the Frenchay Activities Index.

A method of assessing general (i.e. other than personal care) activities of stroke patients is described: the Frenchay Activities Index (FAI), a scale comprising 15 individual activities summed to give an overall score from 0 (low) to 45 (high). Data from 976 patients with acute stroke were analysed to establish the validity and reliability of the index, and to give information on the level of activities seen before stroke, at 6 months and 1 year post-stroke. Factors related to a lower overall score included loss of functional (ADL) ability, more depression, a lower IQ and, at 1 year, being fe
