The management of extensor mechanism lesions after total knee replacement is still associated with an unacceptably high complication rate, especially in its chronic setting. In this study, we report on 4 patients with chronic (> 3 months) patellar tendon rupture after knee arthroplasty, who were treated with a novel procedure of staged patella advancement prior to reconstruction with autografts. Initially, a unilateral frame was applied connecting the patella with the tibial shaft. The construct allowed for gradual distal advancement of the patella based on the Ilizarov principles. After achie

The purpose of this study was to explore the difference in clinical outcome of 2-strand and 4-strand flexor tendon repairs in a single unit in adult population. A total of 109 complete divisions of a single flexor tendon were analyzed from 2016 to 2018 retrospectively. Thirty flexor tendons were repaired with 2-strand and 79 tendons were repaired with 4-strand technique. There was no significant difference in the complication rate including rupture, infection, and adhesions. These results support that 4-strand is not superior than 2-strand and that lower volume type of repair would be preferab

The extensor mechanism of the knee plays an important role in many movements of the lower extremity. Although its main function is to extend the knee, it also stabilizes the patellofemoral joint and resists passive flexion of the knee when landing from jumping. Pathologic conditions of the extensor mechanism can be acute or chronic, and they can negatively affect daily activities. Fortunately, many surgical treatments are available aimed at restoring function to the injured extensor mechanism. Understanding the surgical techniques and common complications of these procedures enables the radiol

Magnetic resonance imaging (MRI) is a noninvasive modality of choice for the detection of static peroneal tendon pathologic features. The depiction of peroneal tendon subluxation using real-time kinematic MRI has not been previously reported. We describe the MRI and intraoperative correlation of peroneal tendon and retinacular pathologic findings, and the novel use of kinematic MRI in the illustration of peroneal tendon subluxation.

A case of a latissimus dorsi myotendinous junction strain in an avid CrossFit athlete is presented. The patient developed acute onset right axillary burning and swelling and subsequent palpable pop with weakness while performing a "muscle up." Magnetic resonance imaging examination demonstrated a high-grade tear of the right latissimus dorsi myotendinous junction approximately 9 cm proximal to its intact humeral insertion. There were no other injuries to the adjacent shoulder girdle structures. Isolated strain of the latissimus dorsi myotendinous junction is a very rare injury with a scarcity

BACKGROUND: The tissue-suture interface remains the weakest aspect of a rotator cuff repair, highlighting the importance of identifying techniques to improve stitch strength. Choice of suture-passing devices, size of the tissue bite, and stitch configuration are variables that may influence stitch strength and therefore repair stability.
PURPOSE: To evaluate the effect that size of the tissue penetrator device and tissue bite size have upon the holding strength of commonly used stitches.
STUDY DESIGN: Controlled laboratory study.
METHODS: Three different-sized tissue-penetrating devices, sm

A reconstructive technique and physical therapy protocol is presented for the treatment of extensor hallucis longus (EHL) lacerations with critical size defects caused by tendon retraction. The primary goal of treatment was to restore EHL structure and function without the use of a bridging allograft or tendon transfer. The technique is performed by split lengthening the distal segment of the lacerated EHL and rotating the lengthened segment proximally 180° to bridge the tendon defect. The lengthened tendon is then sutured to the proximal segment of the EHL. The EHL is then tubularized with an

PURPOSE: This study aimed to evaluate the diagnostic possibilities of MR arthrography in the correct identification of complex tears of the biceps pulley and their possible correlation with anterosuperior impingement (ASI) development.
MATERIALS AND METHODS: MR arthrography examinations of 23 athletes with clinical suspicion of ASI were reviewed. All examinations were obtained with a 1.5-T unit (Signa Horizon, GE Healthcare). The shoulders were studied with a dedicated surface coil with the patient's arm in the neutral position and in internal and external rotation. In five patients, images i

Internal causes of impingement are secondary to rotator cuff and capsular dysfunction and are categorized by the location of the impingement and the underlying pathophysiological or mechanical cause of the impingement. These include posterosuperior impingement, anterosuperior impingement, anterior impingement, and entrapment of the long head of the biceps tendon. The objective of this article is to review magnetic resonance imaging findings of each of the 4 types of internal impingement syndromes and discuss the pathophysiology behind the impingement.

AIM: A degenerative tear of the anterior tibial tendon is a rare event compared to other tendons. The purpose of this study was to evaluate the functional results after surgical refixation.
METHOD: In a retrospective study, we report the functional outcome of five consecutive operatively treated patients suffering from a tear close to the insertion site of the anterior tibial tendon. All patients were assessed postoperatively, the AOFAS and Richter scores were obtained and the range of motion in the ankle joint was evaluated.
RESULTS: Preoperatively all patients presented with a significant

False tendons are fibromuscular intracavitary bands that can mimic pathologic structures such as intraventricular chorda rupture, vegetation, or thrombus. False tendon rupture is a rare echocardiographic finding, reported in only a few case reports. In this case report, the authors present a patient with false tendon rupture that mimicked intraventricular chorda rupture.

A 20-year-old white man presented with a localized unilateral swelling in the popliteal fossa. Ultrasound (US) showed the presence of an accessory muscle, the tensor fasciae suralis. The muscle was located in the proximal portion of the popliteal fossa, superficial to the medial head of the gastrocnemius. Its long tendon extended inferiorly to join the Achilles tendon. Magnetic resonance images correlated well with the US findings, confirming the diagnosis. Tensor fasciae suralis muscle is a rare cause of popliteal swelling and must be differentiated from other masses. Both US and magnetic res

We retrospectively reviewed 13 patellar tendon repairs done over 32 months at a tertiary care, military medical center. Early mobilization was initiated within 2 weeks postoperatively. Clinical and functional results were statistically examined with relation to age, timing of surgery, length of follow-up, quadriceps atrophy, extensor lag, patella position, and time to full duty. At an average of 24 months' follow-up, six patients (46%) had thigh girth atrophy, and one patient (8%) had an extensor lag >5 degrees. Mean Lysholm score was 84 (range, 57 to 100). Maximum postoperative Tegner activit
