Laparoscopy: its selected use in patients with unilateral nonpalpable testis after human chorionic gonadotropin stimulation.

Laparoscopy has been used to help evaluate patients with a unilateral nonpalpable testis. This procedure can be performed quickly just before exploration with the patient under the same anesthetic. With laparoscopy it has been possible to localize either a testis or the course of the spermatic vessels in 100 per cent of the patients. Preoperative knowledge of testis location is helpful to plan the location of the incision as well as the type of repair. The incidence of vanishing testis in our series is much higher than that reported previously. This difference is attributed to a careful examin

Thoracic epidural morphine in the palliation of chest wall pain secondary to relapsing polychondritis.

Relapsing polychondritis is a rare disease of unknown etiology characterized pathologically by degeneration of the chondrocyte and replacement with fibrous connective tissue. The following case report presents the pain management of a 34-yr-old man suffering from intractable pain secondary to relapsing polychondritis. Systemic narcotic analgesics, adjunctive drugs, and peripheral nerve blocks with local anesthetic and steroid failed to adequately control the patient's pain. Thoracic epidural morphine was used to provide excellent relief of pain. Factors in the selection of an implantable narco

Immunoreactive substance P release from skin nerves in the rat by noxious thermal stimulation.

We have examined by radioimmunoassay the substance P (IR-SP) content of superfusate obtained from skin blisters in the anesthetized rat. The blisters were produced on the footpad using a vacuum pump, and the blister base was superfused with tyrode solution. Continuous release of IR-SP was approximately 100 pg/h from the innervated footpad but was considerably reduced when the footpad had been surgically denervated 10-14 days previously. The IR-SP content was markedly increased in the innervated footpad following a local noxious heat stimulus of 50 degrees C applied for 30 s at 5-min intervals

Effects of phenothiazines on phosphate incorporation and chloride uptake in isolated intestinal epithelial cells.

Isolated, intact epithelial cells from rat ileum synthesized a variety of radiolabeled compounds when 32Pi was added to the incubation medium. One of these, referred to as P-0.7 because of its Rf value in the thin layer chromatography system used to separate phosphorylated compounds, was found in the extracellular medium but not in extracts of cells. Synthesis of P-0.7 was inhibited by phenothiazines, with an EC50 value of 205 microM for trifluoperazine. Phenothiazines also inhibited uptake into isolated cells, with EC50 values of 288 and 359 microM for trifluoperazine and chlorpromazine, resp

Effectiveness and safety of nurse-led different epidural analgesia methods based on symptom management theory in patients with severe acute pancreatitis: A single-center RCT protocol.

BACKGROUND: Patients with severe acute pancreatitis (SAP) often experience intense pain that significantly impacts their quality of life and may hinder recovery. Current pain management approaches have limitations. Although continuous thoracic epidural analgesia (TEA) is generally effective, some patients still need additional intermittent injections of local anesthetics for sufficient pain relief. In the ICU, nurses are the primary providers responsible for pain management. Symptom Management Theory (SMT), though used in various fields, has not yet been fully validated for nurse-led pain mana

Modified Single-Side-Lying Quadratus Lumborum Block Combined with Liposomal Bupivacaine versus Transversus Abdominis Plane Block for Post-Cesarean Analgesia: A Randomized Controlled Trial.

BACKGROUND: Inadequate post-cesarean analgesia remains a common clinical issue, associated with increased opioid use and risks of chronic pain and postpartum depression. Although the anterior quadratus lumborum block (QLB III) is more effective than transversus abdominis plane (TAP) block, it requires repeated positioning and may cause quadriceps numbness. To address these limitations, we designed a modified QLB to overcome these drawbacks. This study evaluated its analgesic efficacy and safety when combined with liposomal bupivacaine in cesarean delivery. METHODS: This single-center, randomi

Successful cutaneous sensory blockade following single-injection and double-injection techniques of ultrasound-guided superficial parasternal intercostal plane block: a randomized clinical trial.

BACKGROUND: While superficial parasternal intercostal plane blocks can improve analgesia after cardiac surgery, the optimal site and the number of injections remain uncertain. This study aimed to compare the efficacy of single versus double injections of superficial parasternal blocks, hypothesizing that double injections would achieve superior cutaneous sensory blockade. METHODS: 70 cardiac patients undergoing median sternotomy were randomly assigned to receive either single or double injections of superficial parasternal blocks bilaterally. Each patient received 40 mL of 0.25% bupivacaine w

Evaluation of Postblock Hypersensitivity Using Quantitative Sensory Testing before, during, and after Axillary Brachial Plexus Block Resolution in Healthy Volunteers.

BACKGROUND: The problem of a sharp increase in pain after resolution of a nerve block, known as rebound pain, has been increasingly recognized. It is currently unknown whether rebound pain simply reflects the unmasking of untreated, residual surgical pain, or if it represents a de novo hyperalgesic state driven by a mechanistic resetting of pain thresholds in response to regional anesthesia. In this study, the authors assessed nociceptive processing after a single-injection axillary brachial plexus block in healthy volunteers to detect whether a previously blocked arm displayed objective hyper

Efficacy of using intrathecal mu-receptor antagonist in preventing pruritus and PONV after spinal anesthesia - A systematic review and meta-analysis.

INTRODUCTION AND OBJECTIVE: Different adjuvants are used in combination with intrathecal local anesthesia to improve the quality of spinal anesthesia. Opioids are most used for this purpose, but these are associated with side effects such as pruritus and postoperative nausea and vomiting. Various medications have been utilized for prophylaxis or treatment of these side effects especially pruritus. Although mu-receptor antagonists are generally used intravenously or subcutaneously for the treatment of pruritus, few studies have investigated the role of intrathecal mu-receptor antagonists. So, t

Ultrasound-Guided Superficial Versus Deep Continuous Serratus Anterior Plane Block in Patients Undergoing Thoracotomy for Lung Surgery: A Randomized Controlled Trial.

BACKGROUND: Thoracic surgery is considered to be one of the most painful surgical procedures; therefore, providing adequate analgesia in the perioperative period is crucial. Inadequate pain control impedes deep breathing, which further leads to atelectasis and pneumonia. The optimal analgesic protocol that allows adequate postoperative pain control and facilitates fast recovery of this population has yet to be determined. The serratus anterior plane (SAP) block is a new procedure that is relatively easier to perform and safer, compared with other modalities. MATERIALS AND METHODS: Patients un

Paracervical block in office hysteroscopy with vaginoscopic approach to reduce patients' pain perception: a propensity score matching analysis.

BACKGROUND: Pain perception is a major limitation of office hysteroscopy and may lead to premature suspension of the procedure. Current guidelines do not routinely recommend local analgesia, and the effectiveness of paracervical block during office hysteroscopy performed with a vaginoscopic approach remains insufficiently investigated. OBJECTIVES: The primary objective was to assess whether paracervical block is associated with reduced pain perception during office hysteroscopy with a vaginoscopic approach. The secondary objective was to evaluate its impact on procedure suspension. MATERIALS

Nebulization with differently sized 2% lidocaine atomized particles in awake tracheal intubation by video laryngoscopy: a proof-of-concept randomized trial.

BACKGROUND: Optimization of the individual topicalization technique during awake tracheal intubation (ATI) is critical, but how to accomplish this is still unknown. We hypothesized that there would be a difference between nebulization with different sizes of 2% lidocaine atomized particles in ATI by video laryngoscopy. METHODS: A total of 230 patients scheduled for ATI were recruited from September 2024 through January 2025. Forty-six participants were excluded, and 184 participants were randomly assigned to 4 treatment groups according to the size of the atomized particles of 2% lidocaine: A

Comparison of effects of 0.5 % levobupivacaine with buprenorphine and nalbuphine in ultrasound-guided supraclavicular brachial plexus block- A Randomised Control Trial.

INTRODUCTION: The brachial plexus block is one of the most commonly used anaesthesia techniques for upper limb surgeries. The supraclavicular brachial plexus block (SCBB) has been a frequently performed technique because it provides excellent quality of blockade, faster onset, and dense blockade. Different adjuvants and local anaesthetics are used to improve the onset and quality of the blockade. In our study, the local anaesthetic used was 0.5% levobupivacaine, and the adjuvants used were buprenorphine and nalbuphine. MATERIALS AND METHODS: Our study, a randomised, prospective double-blinded
