Minimally invasive anterior approach to upper cervical spine: surgical technique.

OBJECTIVE: The transoral approach of Spetzler is the classic anterior access to the upper cervical spine that provides direct exposure for anterior decompression of the spinal cord. The risks of infection, the limits in extension, and the postoperative recovery difficulties of transmucosal access suggest the use of an alternative anterior extraoral approach in upper cervical surgery. However, this approach results in complications from nerve palsy because of excessive retraction of the hypoglossal and the superior laryngeal nerves. The goal of this work was to provide anatomic data for an ante

Brachial diplegia as a result of cervical cord injury.

OBJECTIVE: To report a case of brachial diplegia with normal lower limb power, after a cervical cord injury. DESIGN: Case report of a 63-year-old man who sustained a neck trauma following a fall. SETTING: Department of Neurology, University Hospitals of Leicester, Royal Infirmary, Leicester, UK. CASE REPORT: The mechanism of the injury was probably one of hyperflexion resulting in central cervical cord contusion. Initial neurological examination revealed brachial diplegia, with posterior column sensory involvement of the lower limbs which had normal power. The patient was also in urinary re

Central cord syndrome after total hip arthroplasty: a patient report.

STUDY DESIGN: Case report. OBJECTIVE: To present a patient with central cord syndrome injury after total hip arthroplasty performed under general endotracheal anesthesia. SUMMARY OF BACKGROUND DATA: Central cord syndrome, a common injury usually sustained as a result of an extension injury to the cervical spine, often occurs in geriatric patients with underlying spondylotic changes. The injury results in weakness and sensory changes, which are more pronounced in the upper than in the lower extremities. Patients with this syndrome experience variable return of function, but some degree of res

Pathological cervical fracture after spinal manipulation in a pregnant patient.

OBJECTIVE: To present the rare case of a displaced odontoid fracture after manipulative treatment. CLINICAL FEATURES: A 37-year-old, 15-week pregnant patient was referred with acute neck pain and a diffuse paravertebral swelling that started after cervical manipulation performed by her general medical practitioner 5 days before. Because of pregnancy, a cervical spine radiographic series was not obtained before treatment. Magnetic resonance imaging revealed a displaced odontoid fracture associated with a pathological process in the vertebral body of C2 and a paravertebral hematoma on the left

Cervical disc arthroplasty: general introduction.

Cervical disc arthroplasty is a relatively new motion-preserving technique for the treatment of symptomatic degenerative disc disease of the cervical spine. Unlike anterior discectomy and fusion, cervical disc replacement restores normal motion and a physiologic biomechanical environment to adjacent disc levels, thereby decreasing the incidence of adjacent segment disease. To date, cervical disc arthroplasty has been at least as effective as cervical fusion in providing symptomatic relief while reducing perioperative morbidity, eliminating the need for postoperative external immobilization, an

[Diagnostic image (183). A man with pain in his neck after a fall off his bike. Burst fracture of C1 and a dens axis fracture].

A 24-year-old man presented with neck pain without neurological symptoms, after a fall on his head from his bike. Radiographs showed a combination of a burst fracture of C1 and a dens axis fracture.

Complete recovery of severe quadriparesis caused by stab wound at the craniocervical junction.

Non-missile penetrating spinal cord injuries are uncommon, and involvement of the craniocervical junction is even less frequent. The author reports a case of 42-year-old male who presented with quadriparesis immediately following stab injury inflicted with a kitchen knife to the back of his neck. The knife was retained in the patient's neck. Neurological examination revealed spastic paraplegia and severe weakness of the left upper limb and the right-hand grip, and sensory disturbance from C2 and downwards. In addition to these symptoms, cerebrospinal fluid (CSF) was leaking from the wound. Com

Goals of cervical disc replacement.

The goals of cervical arthroplasty are reviewed against a backdrop of adjacent segment disease, restoring normal kinematics to the motion segment, and avoidance of fusion in situations where fusion may be difficult to obtain or unwanted.

Analysis of a school bus collision: mechanism of injury in the unrestrained child.

INTRODUCTION: The most common type of school bus crash resulting in injury and death involves the "rollover" mechanism, which may be linked to bus design. To investigate this possibility, we carried out a detailed investigation of a severe school bus crash. METHODS: The crash involved 12 children, passengers in the school bus. Analysis included the determination of crash dynamics by examination of physical evidence at the crash site and deformation sustained by the structure of the bus and the other vehicle involved. The mechanism of injury was determined by comparing physical evidence collec

Transient quadriparesis in the athlete.

A considerable amount of controversy persists regarding return-to-play criteria and the risk for more severe injury after an athlete experiences an episode of transient quadriparesis. Similarly, the implication of the presence of congenital stenosis in an athlete participating in contact sports elicits great debate in the literature in terms of the athlete's risk for neurologic injury. The relatively infrequent occurrence rate of both transient quadriparesis and permanent cervical cord injury make it difficult to predict with certainty whether or not an episode of transient quadriparesis is a

[Schwannoma of the cervical spinal cord with cervical angina: a case report].

A case of schwannoma of the cervical spinal cord presenting with cervical angina is reported. A 49-year-old man was admitted to our hospital with severe chest pain, cold sweats, and unconsciousness. Extensive cardiac examination showed no abnormal findings. Neurological deficits were muscular weakness and atrophy of the left arm, bilateral hypersthesia of the arms, and hyporeflexia of the left biceps. MRI revealed a tumor in the left side of the spinal canal between C4 and C5. The diagnosis was neurinoma of the left nerve root in C5. The tumor was completely removed surgically by laminectomy.

[Thoracic outlet syndrome].

Thoracic outlet syndrome (TOS) is due to compression/irritation of brachial plexus elements ("neurogenic TOS") and/or subclavian vessels ("vascular TOS") in their passage from the cervical area toward the axilla. The usual site of entrapment is the interscalenic triangle. TOS is a highly controversial subject in regard to its incidence, diagnostic criteria and optimal treatment. Constitutional factors--osseous or more often fibromuscular--and external factors such as trauma predispose to the development of TOS. Various clinical pictures include pain in the cervical region and arm, paresthesias
