The history and development of the fanciful terminology concerning the structures of the limbic system are discussed. The diseases involving the limbic system are divided into three groups; (1) diseases in which the limbic system is more or less selectively involved, such as limbic encephalitis, herpes simplex encephalitis, cerebral confusions by the falx and tentorium, and internal herniations through the falx and tentorium; (2) diseases in which the limbic system is predominantly involved, such as arhinencephalia, holoprosencephaly, cyst of cavum septi pellucidi, Pick's disease, Alzheimer's …

Congenital gingival granular cell tumours are rare lesions which have only occasionally been reported in the UK. Clinical features are of a benign lesion which occurs almost exclusively in newborn, Caucasian females and the anterior maxilla is the commonest site. Treatment consists of local excision and is curative. The terminology concerning this condition has been rather confused because of uncertainty regarding the histogenesis of these tumours and the similar histological appearance to adults granular cell myoblastoma occurring at other intraoral sites. The exact histogenesis of these tumo

Raeder's paratrigeminal syndrome is an uncommon neurologic disorder characterized by oculosympathetic paralysis, including ptosis and miosis. In the ED, this syndrome can be easily confused with Horner's syndrome, although with Raeder's syndrome trigeninal nerve irritation and preservation of facial sweating are noted. This report reviews a case of a 62-year-old man who presented to the ED with signs and symptoms consistent with Raeder's syndrome. MRI with angiography revealed a carotid artery dissection, and the patient was admitted and anticoagulated. The syndrome has been associated with he

A 10-yr-old gymnast, training well below the elite level, reported a 3-wk history of bilateral dorsal wrist pain. Physical examination and radiographic findings were consistent with stress injury to the distal radial growth plates. The patient's radiographs also displayed rare findings including cleft radial and ulnar epiphyses, as well as epiphyseal spur formation. Magnetic resonance imaging (MRI) demonstrated evidence of metaphyseal and epiphyseal ischemia of the growth plate. The patient responded well to conservative treatment, including ice, splinting, and activity modification. This case

OBJECTIVE: To present several case reports of children with 'pseudoureteroceles'. Familiarization with this entity should help to avoid an error in diagnosis and possible improper therapy.
PATIENTS AND METHODS: Three girls with ectopic ureters entering mesonephric duct cysts are presented for review.
RESULTS: Misdiagnosis of the pseudoureterocele as an ectopic ureter was made in two children. The 'pseudoureterocele' may lie dormant for many years and often presents with acute urinary incontinence and/or onset of urinary tract infections. Resection of the dysplastic kidney and ipsilateral ure

Erythema induratum, previously uncommon, is appearing more frequently coincident with the recrudescence of tuberculosis. It is a type of panniculitis often confused clinically with erythema nodosum. We report two patients who were both initially believed to have erythema nodosum and a multisystem disorder. However, in each case, the lesions appeared atypical due to suppuration or distribution of the lesions. This led to skin biopsies, which showed the presence of erythema induratum and excluded the panniculitis of erythema nodosum or leukocytoclastic vasculitis. This lesion heightened our susp

The porphyrias are a group of metabolic disorders of heme biosynthesis genetically determined defects. Acute intermittent porphyria is the most common form of porphyria found in the United States. It is caused by a genetic defect in chromosome 11, where one of two genes for porphobilinogen deaminase is defective. Acute intermittent porphyria is characterized by intermittent, acute, occasionally fatal attacks of abdominal, neurologic, psychiatric, and renal symptoms. Attacks are often confused with acute abdomen or bowel obstruction. A variety of drug, hormonal, nutritional, and infectious fact

Cerebral in vivo proton magnetic resonance spectroscopy of 13 newborn infants displaying seizures and receiving phenobarbitone, in one case supplemented by phenytoin, showed signals from propan-1,2-diol (the injection vehicle for both these anticonvulsants). Subsequent in vitro spectroscopy of cerebro spinal fluid (CSF) from one of these infants also showed signals from this substance. The estimated in vivo propan-1,2-diol concentration (approximately 3 mM) was less than that measured in the CSF sample (14.4 mM). These observations suggest that propan-1,2-diol may accumulate in cerebral tissue

Bacterial meningitis presenting with normal initial CSF findings is rare and could be confusing to the physicians. Such an entity was observed in this first case report from Lebanon on an 8-month-old female febrile infant whose initial CSF studies were normal despite the culture yielding Streptococcus pneumoniae. Thus, this case emphasizes the need that physicians should start antimicrobial therapy, pending culture results, whenever bacterial meningitis is clinically suspected, even if initial CSF investigation of cellular, protein, sugar and gram-strain results do not reveal abnormal findings

Langerhans cell histiocytosis (LCH) is regarded as a disorder of histiocytic cell proliferation of the Langerhans type, probably resulting from altered immunoregulation. Cutaneous involvement is frequent and often appears as a scaling papular eruption that resembles seborrheic dermatitis; when limited to the scalp, scaling and erythema may also be confused with tinea capitis. We describe four patients who had LCH and documented dermatophyte infection versus colonization of the scalp. Fungal cultures should be considered in patients who have LCH with scalp involvement who do not respond to stan

Increasing utilization of chorionic villus sampling (CVS) has lead to the discovery that the placenta can karyotypically be a very heterogeneous organ, and chromosomal mosaicism within the placental can confuse cytogenetic interpretation. Recently, confined placental mosaicism (confined regions of aneuploidy in the otherwise normal diploid placental and fetus) has been described involving a number of chromosomal abnormalities. Fetal trisomy 16 is considered uniformly lethal early in gestation. However, we present 3 cases of nonmosaic trisomy 16 confined regionally to the placenta. We discuss t

Chronic anal fissures have a varied and often confusing clinical presentation. To clarify this picture, we retrospectively studied 172 patients to determine associations between patient characteristics and manifestations of these lesions. We found that anterior fissures and sentinel piles were significantly more common in women; pruritus and rectal drainage occurred significantly more often in men; and patients with a longer duration of symptoms and those with pruritus and drainage were more likely to have a fistula. Younger patients were more likely to report rectal bleeding. Patients with ch

The management of an HIV-infected patient with syphilis is an evolving and difficult area of clinical medicine. Many such patients initially present with dermatologic problems, and the practicing dermatologist must be alert to the variety of presentations syphilis may take in these patients. The role of biopsy with immunofluorescent or Warthin-Starry silver staining is often crucial in correctly diagnosing confusing cases. The treatment of early syphilis in HIV-infected patients may need to be more intensive than has previously been recommended, because the immunosuppression induced by HIV can

Rheumatoid arthritis is being increasingly treated with sulphasalazine. We report the first case of a patient being treated for rheumatoid arthritis who developed severe dyspnoea with chest radiograph shadowing, reversible on discontinuation of sulphasalazine and subsequent steroid therapy. An histological diagnosis of fibrosing alveolitis was made. Thirteen cases of similar reactions to sulphasalazine, all in patients treated for inflammatory bowel disease, can be found in the literature. We identify two types of pulmonary reaction, an eosinophilic pneumonitis and a fibrosing alveolitis. Adeq

Myiases constitute a parasitic disease always secondary to the presence of the larvae of certain insects, particularly diptera, in skin wounds ("cutaneous myiases") or in the body ("deep myiases") of man or vertebrates. Larvae or maggots are able to invade natural cavities and induce myiasis. They can invade the nasal fossae causing "nasomyiases", the eyes causing "ophthalmomyiases" and the ears causing "otomyiases". They can penetrate further, causing urinary tract "cystomyiases", vaginal or gastrointestinal myiases. They can invade the scalp or complicate surgical wounds "surgical myiases".

An infected urachal cyst classically presents with a tender lower midline abdominal mass and systemic signs of infection, including fever, malaise, and leukocytosis. At times, the findings may be clinically confused with those of acute appendicitis, Meckel's diverticulitis, or peritonitis. Sonography aids in differentiating these entities by identifying the localized cystic mass containing debris, located anteriorly in the low mid-abdomen, extending from the region of the bladder to the umbilicus. We present an unusual case of an infected urachal cyst in a 6-year-old boy who presented with low

The classical triad of hemolytic uremic syndrome (microangiopathic hemolytic anemia, severe thrombopenia, and renal failure) developed de novo in three of our renal transplanted patients under cyclosporin A treatment. The predominant morphologic findings in the grafts consisted of glomerular and arteriolar thrombosis as well as arteriolonecrosis, all features of the syndrome. In one instance, ischemic bowel disease supervened after graft removal and was associated with persistent low grade microangiopathic process. De novo hemolytic uremic syndrome has been reported in patients treated with cy

Sixteen cases of clinically and serologically diagnosed canine ehrlichiosis were studied retrospectively. Findings not previously reported included diffuse, pulmonary interstitial radiopacities; weak-positive results for blastomycosis, using agar gel immunodiffusion; normal platelet counts; hemorrhage despite platelet counts greater than 100,000/mm3, and positive results for platelet factor 3 and antinuclear antibody assays. These atypical findings showed that the differential diagnosis of canine ehrlichiosis, blastomycosis, immune-mediated thrombocytopenia, and primary nasal lesions may be co

Fifteen pregnancies occurring in 13 patients with congenital uterine anomalies were analyzed. The anomalies included bicornuate uteri (nine cases), subseptate uteri (two), septated uteri (one), and uterus didelphys (one). Identification of a bilobed uterine contour with an anterior and/or posterior indentation, an eccentrically located gestational sac, and echogenic endometrial debris in the nongravid horn, reliably indicated a coexistent uterine anomaly. Uterine anomalies became difficult or impossible to identify with increasing gestational age. Entities potentially confused with uterine ano

We are reporting the third case of the Olmsted syndrome, an entity that has received little attention in the medical literature. The syndrome consists of congenital diffuse, sharply marginated keratoderma of the palms and soles, onychodystrophy , constrictions of digits, and periorificial keratoses. Our patient, in addition, had congenital universal alopecia, absence of a premolar tooth, leukokeratosis of oral tissue, and hyperlaxity of the joints. The differential diagnosis includes hidrotic ectodermal dysplasia of the Clouston type, pachyonychia congenita, acrodermatitis enteropathica, kerat
