Non-psychostimulant drugs of abuse and anxiogenic drugs activate with differential selectivity dopamine transmission in the nucleus accumbens and in the medial prefrontal cortex of the rat.

In rats vertically implanted with concentric dialysis probes in the medial prefrontal cortex and in the medial nucleus accumbens, morphine, ethanol and nicotine failed to modify extracellular dopamine in the medial prefrontal cortex at doses that were fully effective in raising extracellular dopamine in the nucleus accumbens. Conversely, the aversive/anxiogenic drugs picrotoxin, pentylenetetrazol and FG 7142, administered at subconvulsant doses, increased extracellular dopamine in the medial prefrontal cortex but failed to do so in the nucleus accumbens. Systemic administration of low doses of

Anticonvulsant activity of the diaryltriazine, LY81067: studies using electroencephalographic recording and positron emission tomography.

It is reported that LY81067, a new diaryltriazine, possesses anticonvulsant properties against grand mal status epilepticus induced by intravenous administration of picrotoxin binding site ligands (Ro 5-4864 and pentylenetetrazole) in the baboon. Intravenous administration of LY81067 during the seizures blocked grand mal type electroencephalographic (EEG) paroxysmal discharges and led to a long electrical silence, progressively replaced by spike-and-wave discharges of low frequency (2 c/sec). A transient blocking effect was also observed when LY81067 was injected during grand mal status epilep

Effect of pneumatic trousers on intracranial pressure in hypovolemic dogs with an intracranial mass.

Thirty-six dogs under chloralose anesthesia were studied in groups of six. Arterial pressure (BP), central venous pressure (CVP), intracranial pressure (ICP), and arterial blood gases were monitored. The group underwent: 1) anesthesia only; 2) phlebotomy (P) (40% of blood volume) and reinfusion (RI) of half the shed blood plus Ringer's lactate solution; 3) P followed by pneumatic trousers (PT), and then RI; 4) inflation of an epidural 2-cc Foley catheter (EFC); 5) P followed by EFC and then RI; and 6) P followed by EFC and then by PT and RI. Group 1 was stable. In Group 2, P lowered BP (49 tor

Shock therapy and psychiatric malpractice: the legal accommodation to a controversial treatment.

Shock therapy has developed over the years into an accepted form.of psychiatric treatment for relieving the symptoms of certain mental illnesses. The development of shock therapy did and still does take place in a legal environment which leaves the reasonable psychiatrist free to experiment. The discovery of muscle-relaxing drugs and their introduction into the treatment procedure were undoubtedly encouraged by the possibility of malpractice suits arising from bone fracture caused by rapidly contracting muscles. Today the incidence of malpractie suits involving shock therapy has been reduced,

Neurological causes of cardiac arrest in adults: Epidemiology, mechanisms, diagnosis, and clinical implications.

Neurological conditions account for a small but poorly quantified proportion of cardiac arrests in adults. Recognising them means separating the disease that triggered the arrest from the hypoxic-ischaemic injury that follows it, a distinction that the initial presentation often obscures. Electrocardiographic abnormalities suggestive of a coronary origin, non-epileptic convulsive movements, and injury sustained during the fall are all confounders. Reported proportions, approximately 3-8% across series, depend on recruitment, on access to investigations, and on the attribution rules applied. Th

Neuro-oncological emergencies requiring critical care intervention.

Patients with primary or metastatic brain tumours can deteriorate abruptly, and the acute physician is often the first to respond. This review covers the recognition and initial management of the neurological emergencies most likely to need critical care: raised intracranial pressure from peritumoural oedema and herniation; seizures and status epilepticus, including non-convulsive status epilepticus (NCSE); intracranial haemorrhage; acute hydrocephalus; and venous thromboembolism. It also covers two treatment-related emergencies that the general physician must now recognise: immune effector ce

Brain Abscess Caused by Streptococcus intermedius and Aggregatibacter aphrophilus Secondary to Exfoliation of a Deciduous Tooth in a Previously Healthy Child: A Case Report.

This report describes the case of a previously healthy nine-year-old girl who developed a left temporal lobe brain abscess following spontaneous exfoliation of a healthy deciduous tooth. She presented in convulsive status epilepticus without fever or focal neurological symptoms, and the initial MRI revealed a ring-enhancing lesion with adjacent dural inflammation. Surgical drainage and intravenous antibiotic therapy led to a full recovery with no neurological sequelae after six weeks of treatment. Pus culture revealed Streptococcus intermedius and Aggregatibacter aphrophilus, both of which are

Antibiotic-associated encephalopathy induced by cefmetazole in a hemodialysis patient: Case report.

Antibiotic-associated encephalopathy (AAE) is a rare but significant neurological complication of β-lactam antibiotics, particularly in patients with renal impairment. While cefepime, ceftazidime, and ceftriaxone are well-documented causes, cefmetazole (CMZ) has not been widely recognized for neurotoxicity. We report an 82-year-old Japanese man with end-stage renal disease on chronic hemodialysis who developed altered mental status and myoclonus five days after starting intravenous CMZ for prostatic abscess. Electroencephalography (EEG) revealed triphasic waves consistent with non-convulsive s

[A case of young woman with intracranial gumma developed within 1 year after Treponema pallidum infection].

A 17-years-old woman visited the hospital due to convulsions. T2-weighted images showed high intensity areas in right temporal lobe and left frontal lobe. Enhanced T1-weighted images showed mass-like lesions on the dura mater. Based on mononuclear pleocytosis and a reactive fluorescent treponemal antibody-absorption test in the cerebrospinal fluid, neurosyphilis and intracranial gumma were diagnosed, and antibiotic therapy was initiated. After treatments, the high intensity areas improved, and she had no recurrence of symptoms or MRI images. Intracranial gumma usually develops in tertiary syph

Atypical Rickettsia japonica Infection Involving Critical Illness Diagnosed by Metagenomic Next-Generation Sequencing: A Case Report.

BACKGROUND: Rickettsia japonica infection is a rare disease, it is rare to report critical and severe case caused by this disease in Zhejiang Province, China. PATIENT CONCERNS: We report a patient who initially sought medical attention due to fever and developed coma and convulsions during treatment. The patient did not develop typical eschar and rash. Eventually, the patient needed to be treated in the intensive care unit due to acute respiratory failure. DIAGNOSES: The patient was diagnosed with Rickettsia japonica bloodstream infection by metagenomic next-generation sequencing (mNGS). OU

[A review of functional electrical stimulation based on brain-computer interface].

Individuals with motor dysfunction caused by damage to the central nervous system are unable to transmit voluntary movement commands to their muscles, resulting in a reduced ability to control their limbs. However, traditional rehabilitation methods have problems such as long treatment cycles and high labor costs. Functional electrical stimulation (FES) based on brain-computer interface (BCI) connects the patient's intentions with muscle contraction, and helps to promote the reconstruction of nerve function by recognizing nerve signals and stimulating the moving muscle group with electrical im
