Testing the incremental effectiveness of pay-for-performance to improve implementation of a motivational interviewing brief intervention for substance use disorders in HIV settings: Results of a cluster-randomized type 3 hybrid trial.

BACKGROUND: Substance use disorders (SUDs) have a serious adverse impact on people living with HIV. Previously, using a 39-site dual-randomized type 2 hybrid trial design, findings from the Substance Abuse Treatment to HIV Care Project supported the Implementation and Sustainment Facilitation (ISF) strategy to improve implementation and effectiveness of a motivational interviewing brief intervention (MIBI) for SUD within HIV service settings across the United States (US). Building on this trial, this cluster-randomized type 3 hybrid trial aimed to test the incremental effectiveness of a pay-fo

The Care of Adults with Intellectual Disabilities: Informal (Family) Caregivers' Perspectives.

CONTEXT: The care of adults with intellectual disabilities is marginalized and rarely studied in Poland. In recent years, this issue has gained particular importance, partly due to the increasing life expectancy of people with ID. This paper presents a study of the function of informal caregivers for adults with ID, comprising parents who provide regular, constant, physical and emotional support and assistance with everyday activities to their adult children. Due to cultural and institutional conditions, Polish society sets high expectations for families regarding the care of their dependent m

Working toward sustainability: Transitioning HIV programs from a USA-based organization to a local partner in Zimbabwe.

BACKGROUND: Despite the history of United States of America (USA)-based partners implementing global health programs in low- and middle-income countries (LMIC), future models for sustainable healthcare rely on local country ownership and leadership. Transition is the process of shifting programs towards country ownership, where local stakeholders plan, manage, and deliver health programs. Transition is not a singular event but a process which may include a phase where health programs are led and managed by local entities but still reliant on awards from international partners. This phase is sc

Assessment of the effectiveness of the gastropathy risk reduction program in patients with arterial hypertension.

OBJECTIVE: Introduction: For primary health care patients with concomitant morbidity are usual phenomena. Combination of gastropathy with arterial hypertension is increasingly being studied. However, the assessing of the medical and economic effectiveness of treatment of patients with concomitant morbidity still methodologically challenging. The issue aggravated by different cushion programs aimed to alleviate financial burden to indigent population. These cover non-expensive drugs with probable hazard to concomitant morbidity. The aim: to evaluate the effectiveness of the gastropathy risk red

Better than nothing? Patient-delivered partner therapy and partner notification for chlamydia: the views of Australian general practitioners.

BACKGROUND: Genital chlamydia is the most commonly notified sexually transmissible infection (STI) in Australia and worldwide and can have serious reproductive health outcomes. Partner notification, testing and treatment are important facets of chlamydia control. Traditional methods of partner notification are not reaching enough partners to effectively control transmission of chlamydia. Patient-delivered partner therapy (PDPT) has been shown to improve the treatment of sexual partners. In Australia, General Practitioners (GPs) are responsible for the bulk of chlamydia testing, diagnosis, trea

Cerebellar Ataxia.

There is nothing more discouraging than for a patient to be given a specific diagnosis, then to be told that there is nothing that can be done. Physicians are equally disheartened to see exponential progress being made in the understanding of the pathophysiology of a complex disorder but few direct benefits resulting for their patients. Over the past 5 years, molecular genetic research has completely revolutionized the way in which the progressive cerebellar ataxias are classified and diagnosed, but it has yet to produce effective gene-based, neuroprotective, or neurorestorative therapies. The

Barriers to accessing cancer care services in Eastern Cape and Mpumalanga provinces of South Africa: cancer survivors' perspectives.

BACKGROUND: The World Health Organization’s 2022 estimates reported 20 million new cancer cases and 9.7 million deaths, reaffirming cancer as the second leading global cause of mortality. Equitable care requires access to prevention, early detection, treatment, and follow-up for all populations. In rural South Africa, access is shaped by structural, economic, systemic, and sociocultural factors. This study explored cancer survivors’ experiences in the Eastern Cape and Mpumalanga to understand the barriers they face in obtaining care. METHODS: A qualitative study was conducted between November

Clinical Decision Support Tool for Early Pancreatic Cancer Detection in Primary Care: Simulation Study.

BACKGROUND: Early detection in primary care could improve pancreatic cancer survival, but diagnosis is often delayed due to the low prevalence of the disease, the nonspecific nature of early symptoms, and the broad range of conditions and volume of consultations managed by general practitioners (GPs). In Australia, improving pancreatic cancer outcomes, including via earlier diagnosis, is a priority being progressed under the National Pancreatic Cancer Roadmap developed by Cancer Australia. Computerized clinical decision support systems (CDSSs) have shown promise in aiding timely cancer diagnos

Efficacy of a small-volume blood culture diversion device across three wards: a 6-month retrospective review.

Blood culture contamination (BCC) is a challenging issue that can negatively impact patients and healthcare costs. The Clinical and Laboratory Standards Institute (CLSI) historically encouraged a BCC rate of <3%, although recent evidence suggests <1% is achievable. One approach to reducing BCC is initial specimen blood diversion devices (ISDDs). ISDDs prevent the initial blood, which may contain contaminating microorganisms, from inoculating the blood bottles. The ISDD literature remains limited, particularly for evaluating performance across different wards. Here, the Kurin Lock ISDD was eval

Pushed, Pulled, and Moored: A Systematic Review and Meta-analysis of Migration Intentions Among Medical Trainees in the Arab Region.

INTRODUCTION: The Arab region is experiencing substantial outward migration amid persistent healthcare workforce shortages. However, reported prevalence estimates and reasons vary considerably across studies. Therefore, we aimed to estimate the pooled prevalence of migration intentions among medical trainees in the Arab region and summarize the reported drivers of migration using the Push-Pull-Mooring (PPM) framework. METHODS: We searched PubMed, Scopus, Embase, and Web of Science in April 2026, supplemented by backward and forward citation searching. Studies were eligible if they reported di

Income Insufficiency, Type 2 Diabetes, and Perceived Stress in Cardiac Arrhythmia Outpatients: A Cross-Sectional Study.

Background and Objectives: Atrial fibrillation (AF) and related arrhythmias carry a substantial psychosocial burden that current cardiology guidelines do not systematically address. Whether economic strain and type 2 diabetes mellitus (T2DM) cluster with high perceived stress in arrhythmia outpatients remains untested in Arab populations. The objective of this study was to estimate the prevalence of high perceived stress and to identify its independent sociodemographic and clinical correlates in Saudi outpatients with cardiac arrhythmia, with an a priori focus on income insufficiency and type

Felt and expressed needs regarding occupational health services from the perspective of micro-, small and medium-sized enterprises: a scoping review.

BACKGROUND: In the context of the changing need for health care and prevention due to climate, digital and demographic change, occupational health services are of enormous relevance, as many people can be reached. Of particular interest are the occupational health services in micro-, small- and medium-sized enterprises (MSMEs), as smaller companies are experiencing an unequal distribution of resources. To optimise the implementation in MSMEs, the aim of this scoping review is to present an overview of the MSMEs' needs regarding occupational health services. METHODS: The literature search was

Optimizing Transitions of Care: The Impact of Pharmacist-Led Discharge Interventions on Readmissions and Healthcare Costs.

PURPOSE: Medication discrepancies at hospital discharge contribute to readmissions and adverse outcomes. Although national groups endorse pharmacist-led discharge medication reconciliation, implementation varies. We examined the effect of pharmacist-led discharge medication reconciliation on 30-day hospital readmission odds and financial outcomes after launching a transitions-of-care discharge service at our quaternary medical center. METHODS: This retrospective observational study evaluated discharged in-patients who received pharmacist-led discharge medication reconciliation (study interven
